The Volcano around the hole is from when the cup is cut. The cup cutter pulls the ground around the hole up when removing the plug. There is a device that goes into the hole with a large flange on it after the plastic cup has been put in the hole. It gets stepped on and the ground around the hole is leveled out, and the plastic cup is set to the desired level below the surface. Foot traffic does not cause the problem.

I'm going to start putting my ball-marker on my shoelaces like Micah. That's actually genius. I never thought of the "efficiency of movement" like that.

The volcano effect is actually from pulling out the "plug" of dirt/turf when changing pin positions. If you pull it out too quick or don't compress it back down with a roller afterward, it'll cause that 1/2" - 1" lip of raised turf around the hole!
