highest apm obviously depends on person piloting the champion but high level Ryze, Riven, Draven players comes to mind. Especially in mid to late game teamfights.

Me in low elo after bot tower falls: Can bot pls go mid My bot lane: No, we will perma push bot lane Me: Ok, I stay mid My bot: Wait dragon is coming up, we go mid Me: Ok, I go bot My adc: No, bot is my wave! Enemy team: Nice, free mid tower Though I think the mid lane macro that pisses me off even more is when my top laner gets super lost after top tower falls and they have no idea where to go but mid, since as a mid laner, I can rarely 1v1 or hold my own vs most top laners and usually also don't want to be stuck matching the enemy top laner over being present in a team fight, which becomes a direct consequence when your top laner decides to perma group. I really don't know the remedy to mid game macro errors tbh. If you split your team is likely to perma fight in dumb spots and die but if you group and no fight happens you just lose so much gold and XP, or your team decides to siege what's not possible to siege i.e. tier 2 or inhib tower at 18-20 min, then die to dumb enemy flanks or because someone gets impatient and dives, usually leading to a free enemy baron.
