@ErsatzMoose I agree with you in some cases, but often those mistakes were left in not for a lack of caring but rather with the animator groaning in annoyance. It's not that they don't want to clean it up, it's that they can't. As someone who has dabbled in MMD before, I can tell you that if you are using MMD as your program to animate with, cleanup animation is impossible in some cases. Hair is pre-programmed with physics, so it will go where it pleases and the same can be said for many other parts including skirts. When it comes to arms or legs intersecting with the body or boobs phasing through the shirt: yes these are things that can be fixed by editing the model or animation. Legs going through the skirt, however, are a different story. There is only so much you can do to maneuver the legs in such a way that works with what you want to do AND doesn't make the legs go through the skirt. In these cases animators have to make a tough decision: tone down movement, or have momentary bouts of legs going through the skirt. Hair has similar issues, but even more so, as phasing through the shoulders is impossible to avoid with certain hair models. In addition, Hair often has a shaking issue (You may notice this in skirts as well) this is again a physics issue. One that is near impossible to fix unless your really lucky and shrinking the physics fixes it, or you know how to completely redo the physics to make them work. Unfixable problems are just the trade off for a free animation software. You just have to accept hair and skirts that shake for no reason and body parts that go through each other as a part of the deal. In a way, I think it adds a certain charm an sense of community to MMD. Like we can all laugh and groan together at the obnoxious issues that come up, but at the same time after we accept them we can ignore them to see the beautiful video that has been created.
