He was indeed sealed but it's basically impossible to reform his body back, the angels alongside the daishinkan sealed him in an alternate dimension in the infinite flow of time so only few cells of him are still alive (unlike the first sealing made by ONLY the daishinkan before Archon betrayed them)

The red ssj form is something that exist back in 2009 due to the whole DB AF thing but I suppose the modder based on the Evil Goku concept for the 4 transformations
