This is the second in 3 videos I am going to do about globus. I will be talking about the conditions that cause this symptom and how you treat them. In the first video I spoke about the most common reason for this symptom and the diseases that are associated with it. It will primarily focus on Acid / the enzyme Pepsin and the Cricopharyngeus muscle.
In this second video I talk about sleep apnoea conditions that cause globus, eosinophilic oesophagitis, mediastinal masses, laryngeal masses, autoimmune diseases, thyroid diseases, and laryngeal problems.

Dr Veer please don’t feel you have to apologise for videos being soo long ! I enjoy the detail that you go into. The passion and enjoyment and knowledge that show through these videos is clear to see! If anything, I am sure a number of viewers will agree with me in that we want to see more long form videos where you really drill down into the nitty gritty details of the conditions that you treat and procedures that you do ! I totally agreed with everyone, these videos are amazing and the way you explain everything cannot be more perfect. You can tell the difference on a person that truly love and have passion for what he does; many people become doctors these days but only fews that are born to be one and care for people.

Please dont ever apologize for educating us. I'm in Canada and thanks to your videos, I asked for a sleep endoscopy. It revealed slight obstruction in tonsils, tongue and soft palate . I'm constantly learning through your informative videos and I'm eternally grateful that you speak about sleep apnea. I have dealt with globus, hypothyroidism and sleep apnea since I was 17 and find your "Lump/Globus series" beneficial . Can you please please please start an in depth segment on all the procedures available for sleep apnea in office and under general anaesthesia. Especially the ones you've had success with. After my DISE, the doctor recommended uvulopalatoplasty . Can you please do a video on the different ways that's its performed, their success rates and the pros and cons of each etc

I am going to try and do this - the trouble is that each operation works differently in different people. for example I would not do an anterior palatoplasty in someone with a very long palate, cleft, or any sign of lateral wall collapse - however there are work arounds that allow me to use this technique on all of those issues (sometimes!). Similarly a Barbed palatoplasty can be pretty useful in some situations mentioned above (especially in the case of the lateral collapse - as you can just continue down on either side), but it has it's issues as well with pain, muscle injury etc. I've haven't even got on to UPPP (which I hate), and other free edge surgery (which I dislike but does have it's place). Often I use these techniques together to fit the person in front of me. for example I have occasionally used a hybrid barbed and anterior palatoplasty for a hammock palate which is a bit too long for anterior palatoplasty alone. I personally like to use a host of 'tools in my armoury' to deal with different situations - so comparing one operation with another is almost impossible. My plan is to talk about each technique and their pros and cons - but even that is dangerous in that I might over simplify the situation too much. I am trying to figure this out as I think it'll help other surgeons out there. I'm not sure it'll help patients - but some people are quite interested in these things (like you i suspect?). I'll work it out - it takes me a lot of ruminating time before I'm ready for a video. sorry!

I so appreciate your informative videos! I first noticed this phenomenon years ago when positioned backward in a dentist's chair being fitted for a crown, a really scary moment, as I felt suffocated. Last week (two weeks after surgery to drain my sinuses) I suffered the same so badly while using the CPAP machine that I pulled the mask off - and left it off the rest of the night. I believe three of the issues you mentioned factored in: a mild infection with low-grade fever, moving my adjustable bed to a relatively flat position, and lying on my back instead of my side. It's so comforting to know (or at least suspect) what's happening! Blessings from the US - and best wishes with the study!

I am a retired RN and epidemiologist from the US and I have just realized the following: Medical professionals from countries that aren’t beholden to shareholders explain medical conditions to their patients in an extremely detailed and respectful way. I usually have to watch continuing medical education (CME’s in the U.S.) channels to get the same amount of information that is presented in countries that have a national health program. An additional thought I had is that our for profit system in the U.S. might improve their bottom lines by sponsoring similar videos by answering patients’ questions instead of visits to the doctor for every mole, hangnail etc. In addition, our limited government programs such as Medicare and Medicaid could potentially save taxpayer money. Thanks Doc😊

I guess it is a change of epithelium and that is the first area at the top of the oesophagus that meets those food types?

I am completely enjoying this video series. I don't think it's too long at all! I think you are giving a ton of valuable information which takes some time to explain. I have obstructive sleep apnea, which has nothing to do with why I swallow like a 90 year old with vallecula pooling and the stenotic sphincters. Because I have some form of Ehler's-Danlos (not getting diagnosed because it's expensive and insurance doesn't pay unless you have cardiac or ocular symptoms, which I don't), at some point my soft palate gave way and became floppy. I'm an obligate nose-breather like a baby, so having a floppy soft pallet obstructs my breathing. I awakened myself hundreds of times over the last 30 years because of the snoring caused by my soft palate laxity. However, what I didn't know until a few years ago is that my heart rate was 170 or better and my sats below 80 when my soft palate failed. No wonder I didn't get good sleep! The swallowing issues I have are separate, but linked because of the ED. There are also issues from intentional overdoses on old medications in which the patient believed we could "cure" the overdose with eggs and milk (because everything you read on the internet is true). He took his father's collection of Mercury pills, which were coffin-shaped for a reason. Needless to say, he will have absorption, motility, and stricture issues for the rest of his life from his mouth to his anus. Um, with the dilated aorta thing, are you describing a widow-maker? That's kinda where you indicated. I get the symptoms that you are explaining, but these are NO SHIT issues, no matter what condition you have that could have caused it. 99.99% of patients with GI issues are entirely GI. So, as a GI, stick with that.

I just wanted to say “thank you” I have started with a “lump in my throat” sensation. Like food is stuck. This happened after I did have food impact and was told to drink fizzy pop to cough the food up when I was at AandE I was left for hours siping and throwing up. But the food didn’t budge. After 12 hours it finally moved on its own. I believe all this throwing up has inflamed my voice box making me feel I have something stuck there. I had camera down week after to find out I have hiatus hernia, ulcer and oesophagusitis. So I’m on high dose lansaprozole. I have just ordered gavascon advance. Thank you I’m hopeful this will work

It's a dangerous topic because so many people are told they have it but actually they really don't. It will upset so many people and so I need to be really careful. For example you can't have menieres related vertigo for more than a few years as it burns itself out when you lose your hearing. Betahistine doesn't really work etc. It will confuse a lot of people and cause a lot of anger. Perhaps one day once I've got better at communicating at scale. Sorry!

Hi Vik, Apologies if you've addressed this query. How long would a post surgical suspended pharyngoplasty with uvuloplasty, globus sensation be expected to last? Would tightly stretching the uvula laterally cause/exacerbate this? Many thanks

In the previous video you said that it can take 4-6 weeks with Gaviscon Advanced to get rid of the globus sensation. My ENT suspects I have ETD (I have a feeling of pressure in the ears and tinnitus, both started 1-4 months after the globus). 1. *If* the ETD is caused by reflux/pepsin, will it also take 4-6 weeks to heal? 2. If I end up needing the operation with the balloon expansion in the ET, will reflux/pepsin not recreate the ETD after it again?

A speech therapist gave me a tip for temporary relief: take a wide straw (or pretend you have one) between your lips and blow through it. This relaxes your throat a bit

Hello doctor Vik, could the Maxillary sinus cyst cause all of my symptoms post-nasal drip, throat clearing, thick clear mucus, lump in the throat, tickle in the throat, alternating nostril congestion when I lay in bed AKA vasomotor rhinitis, runny nose when cold outside or eating hot or cold foods and halitosis. I would be happy to hear from you since American ENT are only telling me that this is reflux which I never had any symptoms, so does GI doctor during EGD saying that my esophagus looks good. What causes a cobblestone throat a little bumps on the back of the throat behind the uvula? I don't have any heartburn or reflux . But i have Behcet's syndrome.

If you were to Remove the entirety of the uvula as you mentioned 11:30 uvolectomy section, is it a similar principle to like empty nose syndrome caused by removing turbinates completely ? That’s how I am Interpreting it

No sadly not. I will be doing a video on empty nose syndrome which I believe is more to do with airflow, pressure and turbulence. Uvula is more to do with nerves. Sorry it's so confusing

@@VikVeerENTSurgeon Thank you for your reply Mr Veer. I’ve had a brain scan, all ok. It disappeared after starting on Nortriptyline before bed, then it returned after about 6 weeks. Maybe pressure on nerve in neck, or Fibromyalgia? No problem when active. If I ever nod off, as we older folk do in the afternoon, that results in vague headache and feeling out of sorts, if head is slumped. Thanks again. Ps. I used to be a migraine sufferer, I believe that a large percentage of migraine sufferers also have Allodynia?

I have globus so bad, dysphagia also. Esanophilic esophagitis has killed my throat. Im sure my upper sphincter is hypertensive. Im off to see an ent, in little old new Zealand

It has been about a month and the sensation still there. everytime I eat specially meat I feel like some of the meat still stuck there, and it takes sometime for the sensation to go away. ocassionaly when I swallow it hurts on the right or left bottom side of the throat. It also feels like if i had mucus stck so sometimes i might try to spit it out to try to get rid of the sensation. i am not really sure where is coming from, i do consider myself an anxious person and i tend to get acid reflux often, i also have nail psoriasis and dandruff on my scalp if that helps come up with the cause. Any thoughts, or advise?

I've suffered for probably 12-15yrs now with most if not all of the symptoms you mention. Tbh so many symptoms over the years its difficult to know where to start. The Globus symptoms you discuss are my most aggravating, constantly having to clear throat sometimes leading to a sore throat. Been confirmed as having mild sleep apnea. Im going to try the Gaviscon Advance treatment for several weeks and see if i have any improvement. I would possibly be interested in the meeting/discussion you mention in part one. Im relatively close to London, TW5 postcode.

Hi Dr. I've just subscribed. Having most of problems you spoke about. At moment i have inflamed throat with stiff neck and pain going down from neck to just under my "blades"at back. Also if i swallow to fast pain in neck at back. It stared with flu symptoms. I checked my throat and tonsils does seem bit large and red imflamed and sore. Gargle with salt water. Dont smoke or drink. Eat very healthy. Am normally a very active person but last 2months been struggling to do even just normal things then im tired. Also i know im a very emotional person. Used to doing everything around house and getting frustrated as i can't do things where i have to use arms to cut and trim trees which wasn't a problem since November 2023. Im 70years old. Is there anything else i can do to eleviate the tonsils from getting worse. 'Coz i feel that its that that might be causing my stiff neck and pain. Any other advice. Am trying the exersices. I also want to thank you for your info and help you give in your videos for free. You are one if a kind. May God Bless you abundantly.

Hi Doc. Mine started on and off during a flu/cold and went two weeks ago. The lump feeling felt like it was related to the cold/flu and lasted a good 3 days and went. Then the lump feeling suddenly came back again about 4pm today out of the blue. I do have a runny nose and cough (again) but i don't know what this lump feeling is all about when i swallow nothing but saliva/air. I dont know if its related to the cough/runny nose or something else. I'm paranoid its the C word. I dont smoke (gave up in 2014). It does hurt when i force a burp.

I have Tracheomalacia.recently started on CPAP. Still with globus sensation. Any ideas? Have very raspy throat after a bad bout with Covid. also was put on doxy for persistent cough that caused fixed drug reaction..

I have Bilateral Vascular Eagles syndrome. I've had surgery on both sides. The left has been completely removed, the right side was just shortened. I have a palpable clicking and catching in the front of my throat, where the Adams apple is, with pain this is on the right side. Feels like I have something in my throat. Camera showed nothing. My question is, can eagles regrow or can cartlidge or soft tissue around the voice box or thyroid calcify. Happy to discuss further. Surgeries were 2012, 2013

I thought I was crazy. Could this sensation arise with multiple factors? I’m on mounjaro. Horrible acid reflex. That is under control now by switching to compounded mounjaro. But I feel the need to burp often and clear my throat and even with no acid reflux, the globus sensation is worse when I’m on either shot type. Subtle when I’m not on it now. I take many nsaids for back pain and I am chronically low on iron. I’m ordered the gaviscon advanced to start.

When i eat a pill it sometimes gets struck in my throat .... When i eat things that are dry i usually get a hickup . For the past few weeks i am feeling that something is struck in my throat . Initially i thought that it will be all right like it always used to be . But this time this feeling is not going away i am constantly feeling about this even when when i am swallowing the saliva . I am having negative thoughts and i think i need help.

Sadly I don't have access to royalty free images. Every time I try to use an image I am threatened with a strike. Plus I'm a one man band and I don't have the time to make really good drawings. I was diagnosed with a Thornwald cyst, as well as a deviated septum of the nose. Mucus constantly accumulates in the throat, cough in the morning. Maybe it's because of this cyst. But I doubt, suddenly this is not the main problem. This cyst, as I understand it, is congenital, but I have only had problems with post-nasal drip for the last 4-5 years, which have intensified in the last couple of years, to which was added a feeling of mucus in my throat and a constant desire to clear my throat and cough, especially in the morning. Before deciding on an operation to remove the cyst, I want to test myself for allergies, so I started a diet without wheat and dairy, I hope it will get better. Could you shed light on the topic of Thornwald's cyst, how does it affect such symptoms, are there other factors, such as allergies or something else, that causes an exacerbation and activates a copious secretion of mucus from this cyst.

Thornwalds cyst is 'sort of' congenital. It grows as a consequence of infection - so you aren't really born with it. it can certainly cause globus, and post nasal drip / catarrh. trying other things first makes sense like the allergy tests you mention. I would follow the advice of your surgeon.

I used to vape and I also still play saxophone.. i can swallow but it feels like food sticks and my throat alwayd feels like tight. Ive had an upper endoscope and 2 barrium swallos.. they said no reflux. I wonder what it is. Im from Texas.

In cases of cysts, polyps around the voice box, and issues surrounding GORD/GERD, can smoking irritate these issues and cause the Globus feeling to act up right after smoking? For context, the globus sensation is right near the space in-between my clavicle bones, I'm diagnosed with GERD, and I smoke weed but not cigarettes (though I used to heavily for years. 3 years w/out tobacco now), and the sensation does occur when I don't smoke. Trying to rule out throat cancer 😅
