At a guess, a good chunk of it is compressed muscle, never stated, but I always imagined people who get the symbiote out would have an extremely sore neck immediately afterword, and possible spinal issues, such is would be caused by minor atrophy of neck muscles, there just were never enough people who did so successfully, and lived long enough for it be an issue
