Accident Review of a 1977 Mooney M20J. This was a difficult process for us as we started to become connected the the events as they unfolded. The Thumbnail shows the incorrect "N-Number" of N210HF vs the correct "N-Number" N201HF, my apologies as we didn't notice until it was pointed out after uploading to KZfaq.
NTSB issued safety recommendations A-11-32 to A-32-34 to the FAA to address identified ATC deficiencies.
FAA order 7110.65,
“Air Traffic Control,” provided guidance to controllers regarding route and altitude assignments for IFR aircraft. The order stated that controllers were to include “routes through Class G airspace only when requested by the pilot,” that assigned altitudes on established airways must be “at or above the MEA for the route segment being flown,” and that where MEAs have not been established, aircraft are to be assigned altitudes “at or above the minimum altitude for IFR operations.”

Thanks Dan, I don't see how others do it on a routine basis as it has to take a little out of you each time because you can't help getting connected to the family and the souls lost. We had to walk away and step back so many times while filming this and a few times, we didn't want to finish, but it was a story we felt like needed to be told.

We just had another crash in Michigan 2 weeks ago somewhat similar to pilot and his family flying into IMC with possible ice..... it didn’t end well. I’m an IFR pilot, without ice capability, I have to learn from these examples of ADM’s.

This is so sad and heartbreaking. Ron / Janis I think you did a fantastic job of wading through the report and finding all the things that struck you as odd from the vantage point of a newer ow hour pilot and calling them out. May the family rest in peace.

Yea, there was a lot of information and we tried to stay with the most germane without getting too deep into the details and not seeing the forest for the proverbial trees. So avoidable . . .

Thanks for sharing, and for the respect with which you approached & filmed this. From a controller and CFI who is actively developing a training course for controllers, you have my full support 1000% that the most important word in a pilot's toolbox is "unable." Please don't ever let ATC pressure a pilot.

Very sad video but thanks to this channel for bringing a few things to light to help pilots learn! As a current ATC, 32 years, and also a pilot, I read about terrain issues with ATC almost everyday. It's very important for pilots to understand you are fully responsible for your safety when it comes to terrain and weather!! NEVER assume ATC will keep you clear of ALL terrain and/or weather, you MUST take this responsibility on yourself. ATC is great at AC separation but struggles sometimes with terrain and weather. NEVER accept a clearance that violates any chart altitudes, don't assume the controller knows better! ATC is not god as his wife said so don't look at them that way. We want you to get home safely also but our consequences aren't quite the same as yours!

Great comments and points for all pilots to take to heart. Appreciate you commenting from an ATC point of view as well. We couldn't figure out why he accepted that last routing that took him over the widest mountainous terrain, he was pretty much lined up and ready to go, then a reroute, I guess at that point he was just ready to leave. Way too many Red Flags on this one! Good final point regarding consequences! Janis made that comment as a general statement that pilots tend to think of ATC as the ultimate authority, probably not the best way to say it, but if you didn't know this already, we don't script our videos they are pretty much raw, warts and all-HA! Again appreciate a 32 year ATC point of view as well as a pilot, you have the luxury too see and know both sides of the proverbial fence.

Love the content. Couple teeny things you can easily do that will transform the experience for the viewer. 1. Use body mics - it eliminates the boom echo which is actually louder than your speaking voices, which brings me to 2) avoiding “Bob Ross Syndrome” - particularly when doing the voiceover sections, connect your breath to your voice using your intercostal muscles and keep it supported throughout. That will keep resonance in your voice which right now keeps fading into an ASMR unvoiced percussive whisper (which is fabulous for those with insomnia, but a little too soothing for everyone else zzzzzzz), and 3) 86 the music - you don’t need it and it only makes it harder to hear. This is great stuff you’re doing - keep it up!

Thanks, others have said the same thing and we were on the fence using music, we thought it would help drown out the echo we had post edit due to hardwood floors and no carpeting in the house to dampen it. We will figure out something better for the next one... Appreciate the constructive criticism.

@@apga1998 I agree it took us several weeks to finish this and several times we didn’t think we could as we became somewhat emotionally attached to the story. We thought the music would reflect the somberness of the moment, I guess we were wrong. These NTSB reports tend to be full of details that are hard to navigate through and put them in a timeline that is understandable. That way other pilots can learn and make more informative go/no-go decisions in the future.

@@Parr4theCourse well, the complexity and detail of these NTSB reports is necessary to exculpate vendors and protect insurers from aggressive tort lawyers. The pilots are intensely interested in the decision -making process, as that is what they can and must control. Most of these NTSB reports should begin with the story of Icarus and Daedalus. Great job of covering this tragedy with respectful tones and aplomb. I don't know if I could do nearly as well as you folks did and I'm sure I would have to stop many times too. But the best take away is perhaps someone just you or even you yourself may think think twice about this situation and determine to not fall into the old "get home itis" trap or the "I think I can make it" syndrome. It's unfortunate that I have known of 3 different pilots in my time who fell victim to ignoring those red flags. Like Janis said so correctly, we never know what it's like unless we were there at the time and place of the situation. Nothing is to be gained by placing blame on anyone, but for me any little grouping of problems would begin to raise my red flags. When too many things seem to be against you it's time to re-evaluate just how important is it to get home right now? Some people may take issue with this statement but wintertime flying in Moutains of high altitude and rough terrain in a single engine aircraft can be very risky and often is pushing the limits of you and your aircraft to the very edge. As pilot in command we can't just think of ourselves but must also always consider the lives of passengers if any are with us. You are to be commended for your coverage of this accident Ron & Janis. You both have what it takes to convey the message well and with the proper tones.

Thanks Preston, these are so hard to listen to, much less to video. This is one where they're were so many "Red-Flags", The re-route was the final and possibly most significant one, as prior to that, I believe they would be alive today, not that I or others would have made a "Go-Decision" prior to the reroute, but those are based on your personal limitations. Get-there-itis is not a moniker, but a real issues all pilots must deal with.

As you know I fly this area regularly. There is no way I would ever have attempted this, not going to happen. There are about 2 -3 months out of the year that you have a shot at not icing up. Even then if an emergency arises you have zero options. When you can see there is limited options but in IFR its a death sentence. Come out and I’ll be happy to take you to KJAC.

I still think had he flown his original two planned routes, he would be alive today, not that he should have with a non-FIKI plane!

@@Parr4theCourse I wouldn't even think of it if he had received his planned routes. There are still zero outs and he still didn't have a FIKI capable aircraft. I am strictly a VFR pilot if the temp is even close to freezing. Fall and spring can be especially challenging. When I did my end of course (pre check ride) it was cloudy at about 6000'. My examiner was a charter pilot for the same company that I did my training with. One of their charter pilots reported that there wasn't any icing so we decided to proceed with a couple approaches. The first approach was fine without any ice. When we were heading to our initial approach fix for the second approach we started building ice rapidly. This was in March. From that experience I learned that conditions can change rapidly. Some of the worst icing we can see is in the spring and summer when the temps are just right. A trip with these conditions will always be a no from me without a FIKI equipped aircraft.

You most likely correct being from this area. I gave "some" benefit of the doubt to a pilot that had previous experience flying there and that there was a possibility he would have not perished had he flown his "planned" routes, not that he should have, especially with a non-FIKI aircraft, but that the outcome "may" have been different. I appreciate your insight to this tragic event. I only picked up icing once, during Instrument training and it was so minor, we simply descended and it sublimated as fast as it appeared. But still made an impression on me that I don't want anywhere near it in a non-FIKI aircraft. Up in your neck of the woods, your going to love that Malibu when you get it, opens a few more doors! Appreciate the local knowledge, puts things in perspective!

One of the more significant limitations he faced was simply being normally aspirated (incl. on the oxygen). I flew a 20K/252 for a couple of years (for my company) around the western eleven states. I learned a lot of lessons with some of the crazy missions given to me over that time in that machine (which was being flown because it was very cost-effective for small numbers of people / small amounts of cargo being transported), but it was a fine, well-equipped airplane and could get me in and out of situations in the Sierras, Cascades, Rockies, etc., that I would never have attempted in a 20J with "iffy" clearances relative to altitude limitations, weather variables, oxygen, and the inability to be in the flight levels when needed. I remember an inadvertent encounter with a mountain wave in the mid-teens one time IFR with about a thousand fpm downdraft, and just advising ATC that I was slowing to max climb speed, needed a block altitude, and might have to have an emergency escape route if it worsened. Fortunately, we were able to hold our own in the block over the next ten minutes or so and were out of it. The 20J is a fine airplane but, especially in mountain-related conditions in the west, needs to have even more conservative margins applied.

Thank you for doing this. This one hits home because I’ve spent a lot of time in this area. You briefly mentioned this as some of the “most remote areas in the lower 48.” I can tell you from both long time flight operations and backpacking/hunting in this area that it is. Once he got in to trouble he had absolutely nowhere to go. These mountains (Windriver Range) are some of the most rugged and beautiful, yet unforgiving landscapes you will ever see. I’ve been turned around in this area backpacking on the ground, let alone in IFR conditions with a plane that is rapidly losing performance, he didn’t have a chance once the events unfolded to a certain point. Again, thank you and my condolences to the family and all involved. Tragedy doesn’t even begin to explain. And the thing that makes it even worse if possible......and we say it time and time again when something like this happens..... completely avoidable.

@@Parr4theCourse Ya it is an entirely different deal here in the mountains of the West. I now fly corporate and charter and live in Utah so I experience it almost daily. Sure, it’s all business/corporate jet aircraft now (the occasional Pilatus or King Air) so it is a little different because of aircraft performance, but we are still very mindful of it if we were to have an issue in a critical phase. However, in my training days, that dictated a lot of our route options (I actually did my second solo cross country to Jackson) as well as emergency and alternates. I speak with many pilots as I travel around and one of the main things that comes up is density altitudes in the summer. We have our personal King Air hangared at an airport that sits at an elevation of 4500’. In the summer it is not uncommon to have density altitudes from 7-9000’. In fact that was one of the main factors that contributed to a death of four friends in a Bonanza a few years back. My point is, it’s not just mountain obscuration and icing that we deal with around here. JAC and the surrounding area is a perfect example of all of these issues....icing, mountain obscuration, high terrain that go hand in hand with steep and complicated approaches, runway snow and density altitude to name a few and are not to be taken lightly. These very well could all be “red flags” to many private pilots whether instrument rated or not and most certainly in light pistons that are not fiki equipped (which very few are for good reason). You did a excellent job with Dan’s assignment! I jumped over to your site after watching you on Bryan Turners live stream. Very tragic story that hit me pretty good! I’m about a 75 hr pilot, haven’t flown in 15 years. Bought my first airplane, completed a flight review in the last month and started getting some time in the seat. I am close to the same age as the pilot in your review and live with 2 hrs from him. I took 2 of my kids for their first ride in the new plane on Christmas Day against my gut feeling of waiting until the weekend. I made a few mistakes and had a close call with a cross wind and icy runway. I’m still a little rattled by it and upset with myself knowing I made a few bad decisions with my kids lives in the balance. After watching this I had a confessing discussion with my wife about the flight and decisions. I will never again ignore the red flags and gut feeling! “Every plane with ice is an experimental aircraft flown by a test pilot,” says my 90 year old CFI who was a commercial pilot for 40 years in WA Srate earlier in life and who is deadly fearful of ice.

Absolutely, I had "traces" very light traces of icing once when ATC asked me to climb into a layer for a hold (yea areal hold) due to an inbound medical flight and that scared me enough that I asked for lower and was about to declare an emergency, I got lower and it sublimated almost immediately, but that has been etched into my memory of things I don't want to repeat!

Thanks, I was on the fence for background music, we kept it as low as we could to help dampen some of the echo as we did this in our foyer and we don’t have any carpeting to help damp the sound either.... We will do better next time with some lapel mics....

This was a very thoughtful & respectful report. You may only have a few hundred hours of fly time but you have certainly assimilated a lot of important minutia. It is also apparent that you both listen well. I can see how hard this was for you guys. And I can also see why Gryder stays so "fired up". This was truly a tragedy and I pray for the family.

These are real lives you are discussing here and it is sad that this flight had the outcome it had. You did well to get through this. When there's a major accident it seems like it's rarely just one thing. There often seems to be a cumulative series of things that go wrong before the event happens. Teasing out the details is important to understanding not just what went wrong but what can be fixed. I know this was hard to do but you may have improved future decisions made by someone else. That's a pretty big deal.

In a Dan Gryder video recently he had a list of items. Any two items would scrub the flight for most GA airplanes. Those were: NIGHT IFR ICING TERRAIN In this scenario there were 3 of the 4 conditions.

Excellent job. Thank you so much for digging into this accident and reviewing the details that you learned. I have a few suggestions. First, since you are new at this I wish you would have introduced yourselves and given a little background. Are you both pilots? What and where do you fly? I think a brief bio in the intro provides context and helps your viewers connect with you. Second, I like that you told about the pilot and his passengers, but I want to know more about the pilot and his experience and ratings? Was he current with instrument proficiency? How long had he had his instrument rating? What kind of navigation equipment did the plane have? Also, great intel and discussion on the oxygen issue, a very important piece of evidence that would have influenced the pilot’s decisions. Discussing a Mooney 201’s performance capabilities with charts showing its altitude capability would have been helpful. I real liked how you brought the red flags up in your discussions and I LOVE the format with the two of you discussing the issues. Finally, your point about telling ATC unable was well made. This guy got out into a situation that led him into a high terrain in an off airway route. I fly my Turbo 210 in the mountains every month and rarely on an airway with published MEAs, and I am often cleared between two waypoints below the published grid MORA. I wish there were a way to depict those altitudes. If the Mooney pilot had seen the 16,000’ requirement coming up along his route, he might have requested (insisted on) a different route. Just a few days ago I was cleared between to waypoints at 14,000’, but when I asked to deviate for weather they required me to climb to 15,000’, even though that wasn’t published anywhere on a chart. Finally, I loved the close with the famous Isaiah verse, very nice touch. Keep up the good work and do another one. PS, I’m a very senior airline captain, and a GA pilot for over 48 years.

We are new to videoing accident reviews, we typically produce videos about our trips. Most who have been following us know our background, but I got my PPL @ 60, IFR @ 61 and at 62 flew from Wichita to the Bahamans and we did a series on it that a lot of pilots used as their itinerary. Pilot was current and was going to take a checkride for his commercial when he got back home. I like your idea of performance charts, etc. its a balancing act between too much detail and not enough, Ive learned that a lot of our followers are not even pilots. We have done two more since this one and are currently working on the WSU football team crash in 1970, we hiked to the crash site a couple of weeks ago . . .Congrats on 48 years of flying, I'm only at 4 years but having a blast! thanks for watching and great comments!

This was a great accident review, your delivery was flawless, I thought. I also noticed several comments disliking the background music. I must disagree, I had no problem with it and thought it added appropriate somberness to your narrative.

We did have an echo that the music helped to dampen somewhat as we don't have carpeted floors. We took "most" comments as constructive criticism. Excellent video! Aviation tragedies are mostly the sum of small errors. I’m following for sure. And those people complaining about the music and the audio are most likely non-pilots. If small stuff like this throws their focus off that quick then they don’t belong on a plane’s cockpit. Dan Gryder, Scott Perdue, Paul Bertorelli, Aviation 101, Blanco Lirio and now Parr 4 the Course on my list of go to aviation channels.

We don’t do a lot of accident reviews, but will do a few occasionally that we connect with, they do take a lot out of you as you as you can image. We are ready to get back in the air and shot some more “fun” stuff. Very sad! The biggest red flag in my opinion is using a J model mooney to fly winter IFR in the mountains. A 20J lacks the climb rate and service ceiling to deal with anything but the best conditions. I would say a turbo charged piston with Deice capability would be minimum, some say turboprop or jet. It's unfortunate because it means the average guy gets priced out of winter flying except for extreme VFR but it's what you have to deal with when you live in the mountains. If you aren't from the mountain areas please don't assume because you do fine in the eastern US that you should be fine in CO, WY, UT, ID etc. You will need to be able to get to the flight levels fast and stay up there. If your plane can't do that then wait for VFR to fly. I wish that commercial flight had worked out, it was the only right choice in this situation.

I think your assessment is spot on! The whole incident had so many red-flags through out, I attempted to present it as you get past/over one, then another, then another, etc. I “personally” would not be attempting this flight in a non-FIKI aircraft unless CAVU. Thank you for your comments!

Personally the lesson I took away from this is that it isn't enough to make the right decision once but over and over until you are sitting back at home again safe and sound. This pilot did the right thing, but it didn't work out and eventually he was tempted into something that was a bad idea. Each if us has their own breaking point. Given those conditions it would be a hard no, but what if it were a little better, what if it was a little better than that and so on. Eventually you are tempted. What is going to stop me from taking the bait? That's the question to answer. Thanks again!

Hello from Weston Fl! First I would like to respectfully say that you make a very nice couple flying! With regard to Dan’s request to ask you to review an accident, I could not agree more with him, first of all because it is precisely low time pilots who could benefit the most from his TITANIC efforts to get this information responsibly to us so that we can avoid falling into the same trap and a reminder to older more experienced pilots to fight “Complacency” !! It takes a lot of discipline to Never get behind the command of an airplane, no matter the size or weight, and pretend nothing is going to happen! If you do mot feel well, relaxed, focused and with the right attitude to fly... DO NOT! Don’t tomorrow ! Do not leave for the airport or walk away from it! I have do it! It works and it pays! Today, due to health reasons I may never fly again! Im in the hands of God! But I can assure you that recognicing when you ate not in the best shape and attitude to fly will save you and as importantly yhoa innocen passenger along with you for the ride!

And I agree, if something doesn't "feel" right don't fly, we had to make that decision when Janis' mom was in critical condition in the hospital and we did a video about our decision, it was hard but the right ADM.

Great video. If you need the music, please decrease the volume so that it's not distracting. Also...camera angles are changing too quickly. Small stuff, really. Just distracting from the message, which is outstanding!

Thanks, but I was surprised at the difference in volume, it wasn't until several made the same comment that I listened to it on KZfaq, it wasn't "terrible" but noticeably louder than when I edited it. You've hooked me in, I've subscribed 🙂. Janis is so blessed to have him at the great age of 90. Unfortunately I was never able to have the same experiences as my dad passed when I was just 9 and I'm 69 now.

Thanks Andre, we don't mind constructive criticism, as long as its tasteful. We realize the written word without inflection can be difficult to comprehend and if it was intended to be harsh or as a means for us to improve our channel. Sometimes comments typed on a mobile device doesn't lend itself to explaining oneself accurately, or at least I find it difficult to respond coherently typing with opposing thumbs - HA! We APPRICATE the sub . . . Janis lost her mom a few years ago and her dad understandably is taking it hard, so we try to fill the void as well as her brothers. We managed to get a flight in with her mom/dad a few short months before she passed away, A clip of that was played during her moms funeral services, we are so grateful we have that memory! So sorry for the loss of your dad at such a young age. I hope/pray that the Lord has richly bless you beyond measure with great vivid memories of the short time you had together.

Flying over the mountains in clouds with icing conditions in a Mooney without icing protection was suicide. Even with the turbo Mooney with TKS it would give me pause. The Rockies are not just a ridge of mountains they are mountains for as far as the eye can see. And even if there is no forecast for ice once you get over mountains there will be micro-climates that can create ice. When relatively moist air is lifted by the mountains that air will be cold and just waiting for that wing to disturb it and create ice. In those condition there is no "out" and you are relying of luck. You are betting your life on that luck.

@@billthomas635 Nope, others have said the same and some liked it, we were on the fence, we didn’t think it was distracting as we kept it as soft as we could mostly to mask an echo we were getting due to no sound dampening in our house, all hardwood floors. During editing it was softly in the background, when uploaded to KZfaq it appears to have been slightly louder for some reason, not sure why, my duly noted wasn’t a dismissal of your comment if you took it that way, wasn’t my intent. We still may use music but as you said not while we were talking or fade it out... we’re still new to this and learning. We also know we can’t please everybody, so I hope you saw past the music and saw/gained something positive from the accident review.... Thanks for the feedback....

First attempt at one of these, this one was complex, wasn't sure how much to leave in and how much to leave out and we did leave a lot out. We have a baseline now to improve upon, hopefully you were able to see past these issues and learn from the incident.

Sorry we don’t normally sit and talk and only had one body mic and wasn’t sure it would pick us up equally. I guess we rolled the dice and came up short, as we are pretty new to this and trying to get better within our budget of course. I thought the music was soft and appropriate, didn’t think it was too loud. We will pay more attention to the volume in the future. I didn’t know dissolves were improper, but we are not pro film makers either... Thanks for the constructive criticism, hopefully you found something positive. We are a work in progress.... @@Parr4theCourse If you only have one body mike you can make it work for two (yes, one clipped to one of you is not good but it would have been much better than the distant recording device mike; any microphone that’s closer to your mouths is better than that, although a built-in camera or phone mike can work if you’re as close as possible in a room with good/dead acoustics like a carpeted bedroom with drapes). Use the boundary effect for the microphone. Put a bare table of any size as close as possible in front of you. Lay the tiny microphone capsule on the table on its side with the end facing you. You could tape it down to secure/hold it, either forming double-stick or simply a small strip over its side. You want the mike placed equal distance to you two (or closer to one If either of you don’t speak equally loud) and you want the mike a minimum of about a foot from the table edge. The same method will work for a hand mike but a miniature clip mike is better because you want the element as close as possible to the surface. To prevent any contact vibrations use a tiny piece of thin double-stick foam (3 M makes tiny squares that I use for that) or just forming a double-stick with most any kind of tape will do.

Good deal thanks for the tips, we will start incorporating them as opportunity arise, if we are slow to implement it’s just because we are slightly overwhelmed with learning new editing software, extra cameras/angles/digital audio in the plane , etc.... I mean we started with just one cell phone and no audio during our first flights. I see film/audio is your passion, so you must have a critical eye/ear, we’re just not there yet as we want to do the best we can within our abilities and still be fun!

@Bruce John Shourt Dan Millican yea I’ve seen it and that’s why we bought a lapel mic, but again wasn’t sure how the mic would pick up for two of us. The camera wasn’t that far away and we thought the sound quality was fine. In a noisy environment or if we were doing a walk-a-round or turning our heads to point at something behind us we would have used it. So I agree with you on that one. The music comment was a bit harsh, but duly noted, as it was a way to drown out a little background noise during dead space. Some like soft music background and some don’t. We can’t please everybody. I’m still working on the best way to use transitions/cross dissolves, I get “experts” say don’t or limited use, when I view the video it just looks too choppy without them so that’s a work in progress. We appreciate the honest feedback, but hope that you saw beyond how lacking we are and got something out of the video anyway....

This flight was doomed when he left. Weather was perfect for icing. High altitudes and one engine aircraft in mountains and crap weather. ATC cant know your experience and mods on a plane. You better have situational awareness on a vector. I am a high time GA with a Baron and would never had left the airport. I know because I made a stupid decision and only hand of god and being an avid reader of flight safety materials saved me. My auto pilot couldnt take the chop and was stalling my plane. Ice kills in less than 60 seconds. I never criticize the man. I wasnt there. Take a moment for Mom. I cant even imagine.

It was one of our first reviews, it’s a fine line between too much details and not enough. We don’t do many of them as they are difficult to do without getting wrapped up in the story….

Noted, will make softer or not at all on next one, it wasn't that loud during editing it got louder for some reason after uploading to KZfaq, we didn't think it was too loud, after it was ponied out to us, just louder than we had wanted it to be.

Yea, I've been beat up on it already, it didn't sound that loud when I up loaded it to KZfaq, for some reason the volume wasn't what it was during editing....

Too bad he didn't look at his boys , think he's dad and responsible for his sons , and decide nah, bad vibe, not doin this . There's no shame in changing your mind , aborting . Those are smart decisions. Error on the side of caution . Very sad. Often the case .

I don’t want to be overly critical (😁) but the awful audio part of this presentation pretty much ruined it for me. horrendous “music” was a deal breaker.

Yea, I responded to pervious comments on why it was louder for some reason after uploading to KZfaq than it was during editing, it was a lot softer, I'm still learning and hopefully will get better.

@@pj61114 Thanks, uncharted topic for us and difficult to do as we tend to get somewhat immersed in the story and those effected, so we don’t do a lot of them….
