Wait a minute. How would epinephrine help other than possibly making the person have a pulse? If you give someone with pulseless V tach epinephrine, isn't all it is going to do is what adrenaline does in the body just to a higher degree? So in other words it would speed up their heart, slow down their breathing, cause GI ischemia, etc. just like adrenaline(the body's form of epinephrine) would? In a tachycardia, speeding up the heart and slowing down breathing and causing ischemia is the last thing you want since the heart is already sped up. So wouldn't it be better to give them something to slow down the heart first and then if it gets too bradycardic administer epinephrine instead of administering epinephrine first after a shock doesn't work?

That's a whole "bag of worms" you've opened (bad ACLS pun, sorry). The use of epinephrine in cardiac arrest is based on a dog study from a long while back. The thought is that it increases blood pressure thereby pushing more blood into the coronary circulation, feeding the heart, and thus allowing the heart to do better. That said, there was a recent study out of Japan showing that epinephrine may actually lead to WORSE outcomes. In cardiac arrest, there's no blood pressure. But I understand your thinking.

great video! in my first year of paramedic science and this video helped me alot. they havent spoken about the IV/IO, Adrenaline and amiodorane yet but they have spoken about the 5H and 5Ts.
