Patients with AF who are above the age of 65, or have co-morbidities such as diabetes, high blood pressure, vascular disease or heart failure are at an increased risk of stroke. It is understood that these patients are at a higher risk of stagnation of blood within the heart; the stagnant blood can form clots within the heart and these clots can get dislodged, travel to the brain, and block the blood supply to areas of the brain resulting in stroke.
The prescription of anticoagulants, which act by reducing the ‘clottability’ of blood, to such patient has been shown to reduce the risk of strokes by about 60%. The problem with anticoagulants however is that they can significantly increase the risk of potentially life threatening bleeding and therefore the management of patients with Afib who are at a higher risk of bleeding poses a great clinical dilemma. Such patients include:
1) Patients with haematological disorders such as thrombocytopenia.
2) Patients with previous life -threatening (especially intracranial) bleeding
3) Patients who are at very high risk of falls
4) Patients who are very poorly compliant with anticoagulants.
If these patients are not prescribed an anticoagulant, they are exposed to a high risk of strokes and if they are prescribed an anticoagulant then they are exposed to a much greater risk of bleeding. It is for such patients, that the WATCHMAN device offers an elegant and very effective alternative solution.
Left atrial appendage occlusion
We have known for a long time that the most likely site for clot formation in AF is a beak shaped structure adjacent to the left atrium called the left atrial appendage. It would be a reasonable assumption that if the left atrial appendage could in some way be mechanically occluded or closed off then there would be no way for clot within the left atrial appendage to get dislodged and travel to the brain. It was this hypothesis that led to the development of a left atrial appendage occlusion device known as The WATCHMAN device.
WATCHMAN
The WATCHMAN device is a self-expandable nitinol cage which is covered by a layer of PTFE membrane and can be inserted into the left atrial appendage using a keyhole percutaneous technique. It is available in 5 different sizes.

Just so happen today I was reading about this device. I’m afib on xeralto 10 mg in and out of the hospital; my last stay the Doctor asked if I would consider ablation so I talked to cardiologist He wants to keep me on medication . HI Sanjay, I am an 81 years old male, I first suffered asymptomatic A Fib, and tachycardia (150 bpm) after a Covid infection, I had one cardio version which restored sinus rhythm, and eventually paroxysmal AF was diagnosed. Within a couple of months I was offered a cryo cardio ablation at the RSCH in Brighton since when I have had no further AF episodes. I tolerate anti-coagulants well, but after consulting Dr Google I was alarmed to learn of the bleed risks associated with these drugs, and decided that on balance there's not much to choose between a stroke, and a cerebral haemorrhage, so after consulting my GP I decided to cease taking them, against his advice. At my six monthly follow-up I discussed this decision with my Electrophysiologist, he offered to enter me into a randomised trial, and I was selected to have an LAAO using an Abbott Amulet a few weeks later, again performed at the RSCH Cardio Unit Cath Lab by Prof. David Hildick-Smith under GA with TOE. This operation was carried out 4 weeks ago, and my recovery since has been uneventful, with constant sinus rhythm. I am currently prescribed Clopidogrel for 6 weeks, combined with Aspirin for 3 months, then nothing. I mention this because I have found your lectures highly informative, and I feel both very lucky to have had such wonderful, timely care, but I also feel reassured that I have made the right decisions with respect to avoiding future dangerous drug regimes. Hi Dr. Gupta! I came across your videos not long ago and I have been watching them specially regarding AFIB. I just finished watching the video regarding Flecanide. I have been taking this drug since November 2014 and so far so good except for some reason two months ago I went to ER for high bpm (120) and flooder. Several tests were performed and they all were good. The Cardio prescribed Eliquis (I am not too happy to be taking it) and Metropolol which I told him I am not taking it because my heart rate runs from 50 to 60 bpm. Few years ago I was taking it and my bpm would go down to 42, 43. Well, I agree to take it if and when my heart bpm goes high again. I do carry it with me just in case my heart decides to race again. For better result the Cardio told me to chew on the tablet for faster results. Please keep up the excellent work you are doing here.

I will have the Watchman procedure done Dec.!st. I have had Afib. for many yrs. without a problem and without blood thinners. However I am getting older and my Dr. is concerned about stokes; I was concerned about a metal contraptions inserted into my heart. However I have watched a number of videos and feel more comfortable with the process. Thank you so much for the video.

Excellent video and very timely. I have Afib and am taking Warafrin. I am due to get my heart shocked to try to get it back I to rythm. I am curious your thoughts on using a device from Kardia to track your EKG. I am been using it daily just to keep tabs on my heart. Your thoughts?

Your video just popped up and was very informative. My mama is having the Watchman inserted on March 24 if her hemoglobin stays above 10. She is having another CT to check for blood clots that she may already have which then they can not proceed with the Watchman. As of today, March 6, her hemoglobin has gone down to 9. She was in the hospital during Christmas with internal bleeding and they could not find where the bleeding was coming from! Her hemoglobin was down to 5 at this point. Her doctor says she is in A-fib having mini strokes constantly. In layman words is there anything I should be concerned about? They have mentioned putting a pacemaker in if this doesn’t help her heart stay out of A-Fib. Is this the correct way to go about everything?

There is a new medication that when injected, removes inflammation in the brain and the patient goes back to normal, as they were before the stroke. Look it up, there are videos online that show it. I think the doctor is in Boca Raton, Florida!

My hubby is being recomnended for watchman flex We r nervous because he suffered a huge bleed last year nearly died on Elequis and he already has lung issues copd pseudominus and Mac He has to go completely under to have this done I’m worried about him getting bacterial pneumonia for example during these covid times he gets very I’ll w lungs often has to do percussion vest alb neb 7 percent saline and Tobramycin 2 wks on 2 wks off Your opinion???? We both are jabbed waiting on number 4

I hear that sometimes the watchman device has a sheath that can be problematic in passing it up to the heart and also going from the right to the left inside the heart through the wall. Is this a concern and how large is this sheath or device before expanded. How does one determine the correct size needed to block the appendage? Echo?

I found this really interesting as I find taking anticoagulants causes me such bad tummy pain and I have had three ulcers. It was suggested I have the watchman but sadly was told it is not done in York.

CRP video, please. Can you shed some light on it? IN the USA people are told they will "die in 3 months" if they don't go on meds with 10mg/L. Doctors in Spain are not bothered if a patient has allergies at the time of test or other inflammation but no risk of heart issues, cholesterol ok, etc. In Germany the same. So what is the real deal with CRP and Hear disease? Thank you.

My 87 year old husband with afib, had an aortic tear and life flighted to hospital…..April 19 he was taken off eliquis and December 20th watchman implant and once watchman in, he was put on plavix for a period of time and baby aspirin, and we were told that the plavix will be dropped in June and baby aspirin will be forever. This Watchman procedure was nothing like his open heart surgery years and years go. The surgeon that did the procedure did FIVE implants that day!

Could you answer me one question please ... im 17 yr old and i have pulsating sensation in my abdomen and when i presss that area i could feel pulsation quite strong, they are seeable as well a bit. Im neither thin neither fat, somethin in middle. Occasionally i get pvcs and pacs. Could this be caused by aneurysm? :( btw no history of this in family..

My problem is I have haemorrhoids that can bleed quite heavily and so I cannot use anti-coagulants (I would be regularly calling an ambulance) so this device could be useful. Thank you Dr. Gupta.

@@MrBizzynizzy oh tell me about it, I just had my second ecg because I was feeling ectopic beats and the ecg showed normal heart rhythm with sinus arrhythmia. They are not concerned about it at all. How was your ecg scan? Do you feel alot of ectopics?

@@Stacey_Hunter when I had my ECG I was having a PVC every other beat, I Honestly was expecting the worst news, My blood pressure was through the roof perhaps due to panic, after a few hours in the hospital and an IV drip I was sent home and told everything was fine. I continually had several 100 daily up until a week ago when suddenly things have seemed to almost stopped. Its nice to talk to people like you and watch these videos as most people dont understand the fear associated with ectopics.

it'sonitsway 1111 So sorry about your Dad. 😥 It’s my worry every day., even with an antidote because for me they’d have to send it from another hospital - from Minneapolis to Duluth, plus I have nearly an hours drive to the hospital. 🙄

AF Patient was on aspirin for years. Still had Major stroke. Now on Rivaroxaban at present. Kidney function low due to catheter issues. The new drugs v wafarin... what to do? Is it not a fact that problems arise from all meds given by GPs? They created more issues due to BP meds + statins +aspirin... resulted in new health issues, damage to heart due to inhibiting certain enzymes in body. GPs dish out more meds due to this. GPs fail to find source at first: chemical,physical and emotional state. Folk of a certain age over trust GPs and never question meds.GPs fail to warn health risks of certain meds. We do not see a reduction in health problems but an increase in these issues.

Hi, I am no medic, but my understanding is NO, AF is due to rogue electrical signals in the heart which confuse the receptor, causing irregular heart beats, sometimes fast sometimes slow. This can result in poor purging of the blood, leading to stagnation, and the formation of clots, particularly in the left Atrial Appendage. An Atrial Appendage Occluder closes the entrance to this pocket, and thus reduces the tendency for clots to form inside it.
