Air France B777 performing flight from JFK to Paris was on short final when the airplane failed to respond to pilots' commands descending below glide path.
Flight crew managed to go around and position for another approach.

This is why we love pilots. Hundreds of hours easy flighttime and out of sudden everything turns really bad. Pilots be like: Focus, analyse situation, run checklist, right decisions, everybody is safe.

Blancolirio already covered this, and he is current on type. He strongly suspects that interference on the localiser caused the turn, but the pilots' failed to disconnect the autopilot in response and fought it instead, from his own knowledge of the warnings going off in the background. But he cautioned that it will not be until the BEA has decoded and analysed the CVR and FDR that they will know for sure. They do have both, and apparently, both the localiser and glideslope data as captured by the aircraft are logged on the FDR, along with all control inputs. He confirmed translations with his wife, who is a fluent French speaker.

@@phillee2814 I'm curious why the autopilot isn't programmed to automatically d/c when manual control inputs exceed a level that would be considered intentional rather than spurious. I mean, I realize that would be asking the engineers to idiot proof the system. And engineers know that whenever you idiot proof a system, a better idiot is generated as a result, but I digress.

Stress was so high they depressed the radio switch unknowingly. That means they were grabbing and fighting the flight controls, whilst being low on final. That is madness.

Not madness, it's panic. Not something that's a good look from a professional flight crew. Interesting to see if the aircraft wasn't responding because of something the flight crew overlooked.

Nobody going to mention how quickly the ATC reacted and kept HW in front of them at 1500 feet so AF011 could climb freely in its go around? I know this is their job but that was still a great reaction.

As an ATCO myself I was most impressed with the call atc made at 1:52. He gave all the information the pilots needed, in a very calm way, and in one sentence removed so much stress from the situation. I found the difference between the adrenalin in the voice of the pilot amazing. Before he was very stressed out, after that call you could just hear the pilot relax a little

Yup, bro heard the involuntary comm from 011, and immediatly understood that something went very wrong with them, and immediatly took action to prevent any further collisions, that's a great ATC right there

They were also initially fighting against the automation, which makes the controls considerably "heavy". Once the automation was disconnected, they were able to quickly regain full control. The autopilot was re-engaged once they were stable again. Kudos to the pilots for their quick recovery -- they definitely prevented a major catastrophe!!

The new rapport suggest the pilots were both flying (steering) the plane. Even so bad the clutch between the two yokes was disengaged. Also they both pushed the TOGA switch what brings the plane in a maximum power climb. Both are likely to be fired.

So the initial report has come out saying that the aeroplane was operating perfectly normally but it appears that both pilots were giving control inputs, thus fighting against each other to hand fly the plane, not the PF against the automation.

Wow, that's a super scary situation and absolutely incredible how a professional flight crew can make such a basic mistake. I hope they get to the bottom of how that was ever possible.

That's surprisingly not so uncommon, remember that these guys are under a lot of stress, and a lot of time on very weird schedules, coupled with sometime jet lag... It makes for some very dumb mistakes that can have very dramatic outcomes. Not saying that's excuse what they did, they DID screw up. But at least they realized it quickly enough. ALL pilots makes dumb mistakes, even pros.

Absolutely terrifying hearing the pilot struggling physically with alarms blaring. Then brief silence when ATC asks "AF011?" hoping they respond. They must figure out why none of commanded inputs were responding. Epic save by the pilot!

@@kevinhacken9801 A year late, but the report showed the pilots incorrectly diagnosed an issue (there was none) and then were giving opposite inputs for ~50 seconds causing all the issues.

This is the first time I believe I've heard a commercial airline pilot panic on the comms. Just hearing his voice, heavy breathing and the alarms going off sent chills down my back. Happy and relieved to hear they got control back of the aircraft and were able to get on the ground safely. Would love to know what caused the issue they experienced

have you seen the Air Astana Embraer flight after maintainance in Lisbon? They nearly crashed due to not being able to steer the aircraft. Pilots asked for a site to ditch/crash the plane several times... must not have been a great feeling for the pilots...

I agree that it's not panic, it's just the brain not processing parts of the incoming information that are not critical to the attempts to regain control of the aircraft. If you have a firm grasp of the control yoke and are applying significant force to it because the aircraft did something you didn't expect and you're working to correct for it, the specific placement of your fingers or what sounds you're making with your mouth and/or voice are not something you're going to be thinking about in that moment.

yes, panic. I think they were startled, as the plane followed the glide scope that was messed up, and forgot the autopilot was on. my guess. turned it off flew away

@@jamescollier3 Startled? yes. Shaken? Probably! Paniced? Absolutely not. Startle is a reflex you have no control over, being shaken is a reaction to being startled, but most pilots that have been trained to handle nearly any kind of emergency situation have also trained themselves to not panic under stress, it's just something you have to do as part of being a pilot. Because if you panic when piloting an aircraft, you not only lose control of yourself, you lose control of the aircraft. These pilots most certainly did NOT lose control of the aircraft for long enough for anything bad to happen, therefore they did not panic. Were they stressed? Afraid? Probably! But again that doesn't mean they panicked.

I was on this flight and it was scary, super grateful for pilot’s professionalism and crew that kept calm. It was also complete fog so we had no idea how close we were to the ground.

Oh yes, we felt it - it felt as if it was accelerating very fast with the sound and then falling - three or four times , some passengers screamed..was scary! Felt super lucky to land safely after all

After watching Blancolirio's video on this, it could be that an airport vehicle or other aircraft briefly obstructed the localizer signal, which caused the autopilot to veer off course. The pilots then didn't turn off the autopilot quickly enough

french prelim investigation finds that FO was putting inputs into aircraft that the captain was not told about, so the captain called for go around, both fo and captain made simultaneous opposite inputs, even when climbing. It is human error

I have been very confused until now why controllers switched from talking to 011 to HW. Now I realized they are two separate aircraft. Previous reports were that 011 was instructed not to go below 1500, but they were struggling with the plane and went down further. Now I realize that it was a different aircraft that was told to level off to stay away from the distressed aircraft, very interesting

I find it interesting that one of the pilots eventually called and told the controller why they didn't answer before; I don't think being an instruction flight matters. Keeping the airplane from crashing matters. Aviate, navigate, communicate. In that order. "Sorry control, but you're last, I'm trying not to crash."

You're confusing the AFR011 (the Boeing 777 that was having issues) with the AFR73HW that was having the instruction flight.

- Wrong approach selected in the FMC (common) - Autopilot left engaged for far too long in the approach (habitual) - Trying to overcome the autopilot without disconnecting it (common) - Hasty and embarrassed go-around causes gear to be retracted before flaps (a quarter of all go-arounds) which triggers the config warning audible on the radio - Rapid return to the airport because the pilot knows there’s nothing wrong with the aeroplane. If he genuinely thought there was a defect there would have been a lengthy hold.

Regarding the wrong runway in the FMS, interesting that the asked for the other runway during the go-around. I heard 2 alarms, a two-tone and a "cosmic" tone (config warning?), any idea what they relate to? Also isn't the AP suppose to disconnect if you move the yoke?

These are bold claims to make. I’m pretty sure that operating the controls disconnects the AP. Either way, I can see that your suggestion could be correct and that gross situational awareness, combined with startle and not following SOP (leading to more startle), may have caused this. The 777 is far more reliable and has such redundancy that this situation shouldn’t happen.

@@atubebuff The first alarm is the master. Its almost certainly due to the gear lever being moved to the up position whilst the flaps were fully extended. The master wont silence until the flaps have retracted to at least 20deg. The second tone sounds like the AP disconnect. As for will the AP disconnect if you move the controls... nope. It will actually try and fight you. Hence why the PF was out of breath.

Seems stressful indeed, loss of control so close to the ground... accident was few seconds away. looking forward to know what happened. Would have been another crew , might have been really bad news.

The 777X during test flights the FAA discovered an event where the airplane experienced an uncommanded pitch down event just like this.

It's "très longue *vente* arrière" to be precise, not just "vent". "Vent arrière" would mean tailwind while "vente arrière" would mean downwind.

@@KyrilPG lol, no. I'm French... vente arrière doesn't exist. it's "vent arrière" but when you read it or say it , you do the 'link' with the 't' that is usually silent., so it just sounds like 'vente', but it's vent arrière and translation is downwind (from a circuit pattern in this case)

@@iaf22 Merci, je suis français aussi et on prononce bien vent(e) arrière en l'accordant au féminin et en marquant plus la liaison comme s'il y avait un e. Sinon c'est un vent arrière... C'est un héritage de la marine et comme pour beaucoup d'autres termes le e marqué est un abus de langage certes mais on dit "une vent(e) arrière" quand on parle de downwind. Toute la formule est d'ailleurs un abus de langage mais j'en ai vu pas mal qui écrivaient avec un e pour différencier la manœuvre. J'en ai même vu écrits dans des documentations, probablement pour éviter toute confusion. J'ai simplifié pour nos amis anglophones mais apparemment c'est raté 🤣

Juan Brown of the Blancolirio channel, himself being a B777 pilot did a video on this incident. He identified the alarm sounds in the background as nr 1: master warning and a bit later nr 2 the AP disengage warning. He thinks that de pilots initially fought the AP. When the AP is on, the controls are very havy. Anyway what exactly caused this B777 to suddenly veer of course he could not tell either.

The most disturbing thing about this was hearing a panicked pilot; a species I had hitherto thought of as imperturbable, even in situations like this. If the pilot sounds afraid, be *very* afraid.

This is a scary-sounding incident on the surface, but honestly the more details I hear about it the more it sounds like it was relatively minor and actually calls for some additional pilot training more than anything else. This video is a far-better translation and gives us far more information than I’ve seen yet anywhere else. I’m not an expert on aviation, much less an investigator privy to any details, just a fan who has followed the industry for 30+ years so take my analysis with a grain of salt. From what I can see here this is the sequence of events; AF011 is on final to 26L, flying on autopilot along the glide-slope using ILS as AF73HW is taking off from the same runway. For reasons unknown (Speculation; a combination of bad weather and the presence of AF73HW between AF011 and the glide-slope antenna led to interference in the glide-slope signal), AF011 deviated to the left of the glide slope. The pilot-flying on AF011 observes the deviation and instinctively attempts to hand-fly the aircraft back into the glide-slope, however they fail to disconnect the autopilot and wind up fighting against it. As the PF realizes the aircraft is not responding to his inputs he panics and accidentally keys the transmitter, letting us and ATC hear that they’re struggling with the aircraft and various alarms are going off. The tower hears that AF011 is in distress and instructs AF73HW to stop their climb, amidst this we hear AF011’s autopilot disconnect tone transmitted (Whether the crew recognized that they were fighting the autopilot and turned it off, or the plane itself disconnected it due to control inputs or a stall warning will have to be discussed by investigators). Shortly thereafter the crew on AF011 informs the tower that they’re going around and had experienced a flight control problem, but they’re in control once again. The airplane continues around and lands without further problem. I could be completely wrong; As I said; I’m not an expert, just a casual observer. In any case, thank you for this VAS as it’s the clearest view of what happened yet.

That seems like the general consensus among the people who have experience. Juan Browne suggests that the master caution warning is related to raising the gear while the flaps are at the wrong setting. Which makes some sense with the overall situation.

To be honest, you will notice when you are trying to make inputs while the autopilot is engaged. When you feel a lot of resistance in the controls, the first thing that comes to my mind is to check if I, for whatever reason, failed to disconnect the autopilot. I don't fly airliners (yet) though so what do I know :) . It's still a weird situation, mainly because they were fighting the controls but eventually got them back rather quickly and landed without any problem. If it were a hydraulic failure, most of the time they would publish that info quite soon in the media. Maybe they had a stabilizer runaway, but that doesn't really explain the movement to the left. Could also have been some computer f*ck ups. It's just a weird situation.

@@milantenbroek1214 the problem in these kinds of situations is that they are generally caused by complacency, and complacency breeds stupid decisions and thoughts. Just like how we all KNOW how to recover from a stall, until you're actually put in that emergency situation (not simulated), it's very difficult to see how you will actually behave. In a similar manner, I'm sure looking back at this tape, the PF is thinking "how the hell did I not realize the autopilot was still connected" but in the moment your brain is not thinking rationally and it's probably thinking about the 100 other (extremely rare) reasons the controls could be fighting him and not the common cause which should not be true (because normally the AP should already be disconnected, so "obviously" that must be true). One thing's for sure, though: he'll never make this mistake again.

My take on that is that there could also be issues with the ergonomics of the controls. Pilots will do mistakes - we need to acknowledge that fact and try to design the controls and alarms to instinctively guide the pilot so that the situation does not escalate. More training, sure, but also maybe the controls can be improved, or some automation can be changed. This is why I am very happy to know that the BEA (French NTSB) will do a full investigation which will very likely lead to multiple recommendations and hopefully not just "pilot training"

@@uski Sorry, not meaning to sound like I’m dumping all the blame on the pilots. Incidents like this require an alignment of any number of multiple factors and the pilots’ response will ultimately likely only be a part of the story. But from the early articles I read you’d swear we found ourselves a new 737MAX scandal and every bit of new information that comes out about what happened makes the issue sound less and less dire, more and more like a narrow set of unusual circumstances meeting up with pilots who managed to momentarily get a little behind the aircraft, and that’s actually quite relieving when weighed against the MAX issues.

AP is supposed to disconnect if you give the yoke enough input force but it seems like it did not despite giving it all the muscle he had, I would consider that a minor to moderate mechanical or software related failure but in that instant the pilot forgot to do one of the most basic things which is check to see if the autopilot is still active or not but I guess in a panic he was only focused at the yoke.

Wow! I would like an investigation to analyze what happened and read the report. I’m guessing ground equipment signal interference as there is not enough info in the video. Approach and landing speeds and acft configuration make this segment of flight extremely dangerous. Glad the pilots did so well. Phew! Ret Captain

If you listen, the ap disconnect tone sounds after their initial struggle. They were fighting controls because the ap was still engaged. 🤦‍♂️

Turns out it is because of poor crew resourse management. The pilots were putting in conflicting inputs, one of them should've said "my controls" to prevent the incident

A little late disconnecting the AP? The deviation of ILS approach may be been due to a localiser antenna anomaly. Potentially the pilots could have been fighting with the AP on final prior to the TOGA (causing the plane 'not responding')... Just speculation... Will be interesting to see the final report on this.. thanks 👍🏽

The AP will immediately disconnect if you put in enough steering into the yoke. Also, it's the very first thing pilots are customed to do when something is wrong. So my best guess would be they actually couldn't get the AP to disconnect for some reason. I'm sure they didn't "forget" as you can hear them breathing strongly - which means they were using the yoke.

at approach i believe the alarm at 1:08 must be the low speed warning, the only other alert for this is the crew alertness monitor(i think?). 1:25 is clearly the autopilot disconnect

I don't remember a low speed warning even exists, I think this warning is about a wrong landing configuration (no gear but flaps 30), but maybe that's also dependant of speed/altitude..

I was on an Air France flight AF990 (Boeing 777 -300) leaving CDG for Jhb on 8th arriving in the morning on the 9th, at 8:37 just before touching the ground the plane suddenly went up, there was silence for almost 30 Seconds then Air Hostess said Pilot will explain, there was further delay, later the pilot said they could not land due a situation and it was quite normal and will be landing in 7-10 Minutes again. The flight flew around for a while, only to land @ 9:02. Clearly the details were hidden from passengers After googling I saw the April 22 incident? Is there perhaps a problem with the 777, Air France??

Actually, this is one of the few "events" that requires correct steps be done by memory. However, many of the items of info have already been set in the physical computers/instruments. Plus, the process SHOULD have been briefed/reviewed for the human computers BEFORE every approach. The only "surprise" is that the need for the procedure actually happened. In general: add power, flaps (as needed), check climbing, gear up, check/engage missed approach altitude. Navigate as needed. Communicate when able. Fill out paperwork on the ground. 😁

All those concerned here speak the same language, the other pilots who do not speak French are not concerned, and when they must be, the tower or pilots speaks to them in English. Whether you're English or not, it's so English to want to impose things on others that they don't need.

My take would be an inadvertent wrong programming of the autopilot, e.g setup for 26R instead of 26L. Approaching the runway, pilots notice the wrong alignment with the runway and try to manually override the AP without switching it off. This explains the physical struggle of the pilot. They then decide to go around, but raise the gear first without raising the flaps, causing the configuration alarm. As soon as the aircraft was configured correctly, everything went back to normal.

Both pilots should check the FMS set up before the approach, so this should be unlikely. Not impossible though, and human error is always the first point of blame for these incidents.

You don't have to physically struggle to oversteer the autopilot. As soon as you give some input into the yoke, the AP disconnects by itself.

I doubt that they touched the gear or flap lever in such a situation. I would guess the alarms started because they suddenly went to low or slow with the wrong configuration - however the configuration wasn't meant for this low and slow.

@@jemand8462 - I can’t recall or find the required force to disengage the AP via control column input. It will clearly exceed the normal forces applied to the control column from holding it during critical phases or even nudging the column when exiting or returning to the seat during flight. I also haven’t seen any bulletins from Boeing, so the initial response to the situation isn’t like that for 5G.

I am originally from France and have live in the US for 28 years. There are good and bad things on both sides. The French ATC and pilots should only be speaking English especially at a large international airport like CDG. The Turkish pilot we hear has likely no idea about what is happening since it is all in French (no situation awareness). I also agree that there is too much chatter w/o real information. There is also a very special way aviation people talk (including flight attendants) which sounds annoying at time (it is like a singing that only them use). But at the end they saved everyone and will likely only fly Airbus in the future (last part is a joke!).

Fly by wire means things can go wrong with the AP disconnected since the yoke and rudder pedals are not directly connected to control surfaces. Redundant systems are designed to prevent this, but a perfect storm of problems could easily affect safety of flight which is very likely what happened here.

@@prorobo - the information strongly shows that the pilots were panicking because they couldn’t control the aircraft whilst the autopilot was still engaged. Major pilot stuff up. Nothing wrong with the automation, apparently. The automation has 3 autopilots and redundancy in all systems.

@@prorobo - I’m familiar with the 777 systems and, like with the electrics and hydraulics, there are back ups and redundancy. The flight controls do have direct connection to one pair of spoilers for roll control. Boeing have so much confidence in the capability of the systems that these two spoilers are all that is required for the 30 seconds or so it takes for the automation and FBW to restart. I’m not sure about he suggestion that non-US companies like their pilots to use automation to excess. You’re suggesting that American pilots are the best and everyone else is, to some degree or other, is worse. These aircraft are docile and so pretty easy to fly. I think pilot ability is more to do with understanding the automation than just hand flying skills. This incident suggests the pilots didn’t understand what was going on. The Asiana 777 crash showed us the pilots didn’t understand the automation. These are training issues no amount of hand flying skills will help. I’d also suggest, from this US centric channel, that there are more than enough problems in US aviation.

GS or LOC interruption caused the AFDS to enter the flight stabilisation mode. Pilot has panicked and tried to fly the aircraft without first disconnecting the AP. The go-around was incorrectly flown with the gear going up before flaps, resulting in a config warning.

I figured it was the Pilot..because I have not seen anything from Boeing or Others about any issues on the Landing of Triple 7..it is like in Dec.2021 when Emirates Boeing 777 was taking off from Dubai and a small checklist item was not taken care of…Like Autopilot Altitude which was Still set at 0…Just barely missing homes at the end of the runway!

The 737 , 757 , 767 and 747 400 use conventional controls with cables and PCUs. 777 and 787 are fly by wire.

Would be better if everyone used English on frequency, to maintain good situational awareness (SA) for all. Confusion and lack of SA due to multiple languages being used has caused accidents in the past. However, good job from the crew and the tower controller. Glad they landed safely.

ICAO official languages are the UN official languages and refers only to documents and publications. This is one of the more busy airports in Europe and crews are from all around the world as policy eng should be the only language to maximize situational awareness of everyone.

For ifr commercial flight however you can only use the 6 icao languages and each one of them has a standard phraseology You cant talk german for example or any other language that are not icao for ifr commercial flights. Real world atc is not like flight simulator where everyone talks in english, everywhere, although everyone who is involved in commercial operations can talk in english as required ,which is different.

@@VASAviation i know, but it was a very long flight, u know. Maybe they forgot to turn off the AP and wondered why their inputs didn't follow directions, so that they increased the input's strength, which let the AP turn off

@@christophbucker904 That doesn't make sense - if the AP had been left on (but not in glideslope mode or descent mode) then it should have been preventing them from descending - instead it seems like it was making them descend too quickly.
