This week’s video on rosacea was filmed in February but I think it’s probably even more apt right now, as we hunker down at home and skin is subjected to the ebbs and flow of stress hormones, sugar and dry, centrally heated air. It also just so happens to be Rosacea Awareness Month.
It’s a tedious mix of redness, spots and dry, uncomfortable skin which may flush up at any give moment due to a myriad of triggers. As someone with a definite ‘sprinkle’ of rosacea myself, I thought it would be helpful to revisit the strategy Dr Emma and I use in clinic, given that so many of you had questions on it at our recent Tuesday Check-up on Instagram.

Helpful hint during your workouts: cool wet microfiber sports towel. I drape mine around my neck, it keeps me cooler with less flushing.

I am suffering from rosacea for some months now. The part you talked about prevention was very devastating for me. If I would ask my friends to list things they think I enjoy the most they would literally go like “sauna, coffee, red wine and skiing”. Fml. But big thank you for educating me.

A video on skincare steps and what exfoliants, lotions, active ingredients etc we can use would be great as i still can't figure out how to build a routine for rosacea besides just cleansing, moisturising and sunscreen! I also use topical metronidazol but unfortunately that isn't being enough for dealing with texture irregularities and black heads!

Is the laser treatment a replacement for all antibiotics and topical treatments or just a a supplement? Are there different laser treatments and if so is there any specific treatment you would recommend or are they all the same? Perhaps types of lasers used?

Yes, so often the products for rosacea, acne or hyperpigmentation are targeted to oily skin.

Like how do I apply soolantra before moisturizer when moisturizer is supposed to go on a dry face but soolangra tells you to dry your face first :( am I supposed to just say goodbye to proper skincare?

I've just been diagnosed with Rosacea, 5 mins into seeing a great dermatologist. Three GP's and one dermatologist over 7 years had diagnosed my condition as Folliculitis and I've been on numerous antibiotics which caused all sorts of issues. I thought my symptoms were not related, but now I know I have rosacea, I can see they are related. Symptoms : Inner ear rash / irritation, inflamed hair follicles on my scalp, papules and pustules on the forehead, chest and scalp, flushing on the T zone and chest. I also developed a stye in my right eye on a trip to my company headquarters when we were having photo headshots done for our company website !. Chlorsig did not help and the irritation lasted a long time. I see now that it was Ocular rosacea. I've just had a breakout after an alcohol session, which involved Port wine.... I am now going to study and watch out for my triggers. Started Isotretinon 20mg capsules and Ivermectin 10mg cream. Love your channel.

Redness from mild seb derm and rosacea are very similar. It just takes trial and error.

Very nice job by the stylist on this video. You both look lovely and the outfits/hair are so complementary, flattering and sophisticated. Finacea 15% gel is finally available in the U.S. in generic so it no longer costs $300-$500 per tube.

I'm doing pretty well with managing rosacea. I use a gentle cleanser and (based on Dr. Sam's video on the skin barrier) follow up with an HA (hada labo). In the am, I use an OTC 15% AA and cerave mineral sunscreen with ceramides and niacinamide. sometimes I add burt bee's salicylic acid (2%?) every few days on the bumps which seems to keep them at bay. Other days I use TO vit c powder mixed with TO resveratrol -- just enough to cause a tiny bit of warming, not more or I'll have some irritation. At night I double cleanse, then again HA, AA and prescription retin-a, (highest strength, which I've been on pretty consistently for 30 years, so I am used to it and it causes no irritation for me). On top I use laroche posay cicabaum, which I love because it is very soothing. I still have some redness and am thinking of making some changes. As the doctors said, AA and niacinamide are the best things you can get for rosacea. But I am using OTC AA (15% max) and the niacinamide in the suncreen is probably not a high concentration. So when Dr. Sam says the niacinamide should be in an "appropriate concentration" I don't know if the amount in the cerave is enough.

Pleas post IG lives here too. I cannot keep up on all the social media now. I do not have rosacea but seriously sensitive skin. Would love a video about why some of us can never seem to acclimate to retinol. Have tried tretinoin .025, over the counter (yours) and adapalene (Laroche Posay and Differin). I tried very low does 1 x week for 12 weeks then 2 x week, and I still get irritation with 2 x week. Have used moisturizer as you suggest but don’t really see any results after more than a year. Still have acne, redness and signs of aging. I am 50, still have estrogen but low testosterone. Many C products irritate as well. How do I keep signs of aging at bay when I cannot use many of the actives?

Thank you for another infromative video! Could you consider doing a video on peptides? They are very popular on skincare shop shelves, but I don't think I've seen a single video from you on them, so curious if they're overhyped and not as potent as other active ingredients you often talk about (vit a, b, c and acids). Would be much appreciated to understand what they do for skin if anything and how effective are they in skincare / how to incorporate with other actives :)

Thank you for that charming video 🙂 from my experience (prerosacea and acne tarda mature skin) my skin enjoys azelaic acid (20% cream) and Rosacure Fast by Synchroline (wonderful for calming while being lightweight and super comfortable). When things go wrong - LPR Cicaplast Baume B5 (miraculous maker). Last but not least: my best cleanser for oily/dry/sensitive skin is Bioderma Sensibio Gel Moussant ( only one i find not to make any harm and has great cleaning abilities still). Mary Joyner I use „Acne-derm”. It’s Skinoren-like cream with 20% AZA. Unfortunately it’s available only in Poland, produced by lokal producer. I find it lighter than Skinoren cream and 2 to 3 times cheaper 😉 I’ve tried a few forms of AZA (Skinoren cream and gel, Hascolek - polish azelaic acid liposomal gel, based on dimeticone and azelaic acis peels based on propanediol and peg-400) and the 20% aza cream makes the less damage, and keeps my skin quite calm still. I do also use benzoil peroxide over my cetaphil moisturiser every other night on my cheen and forehead as well. BPO is the only thing that keeps my acne at bay. Azelaic acid by itself is too weak 😒 but it’s grate in fighting dullness and PIH.

Scheduled for my 5th Pulsed Dye Laser (Vbeam) appointment next month. It's the only thing that's helped me with the redness. Finacea helped with inflammation, but not the redness. Niacinamide makes me flush.

If you could answer my question I'd be grateful, if not its ok. I apologise for any mistakes, English is not my 1st language. Your videos on skincare enable me to learn something new about skin, so thank you for sharing your knowledge 🙂 Now I know something about my skin, previously my knowledge was quite low so I'm happy to learn. Secondly, I would like to ask what to do with very problematic skin? My skin as I noticed, is very sensitive and I also have acne and thus, is hard to treat. Lately, many cosmetics on my face make it burn and all creams I tested I can't use because of their irritation or my acne, and now I have almost no skincare. From time to time, I visit a derm, but it hasn't been really helpful. I visited a few and sometimes I get mixed information, and some do not even ask if my skin is for instance, sensitive and just prescribe whathever they feel like it. I think I don't have predisposition for rosacea, but my sensitive skin sometimes is very nerve wracking. I used duac gel few years ago and I tolerated it after some time, it even helped me clear all my pores from blackheads, but now I asssume I couldn't use it because of my sensitivity. I tried Qcera cream that contains 5% of as I remember azelaic acid and pirogronic acid ( I don't know how to translate it into English) but it burned very strong, so I didn't buy it. My derm told me I shouldn't use what irritates my skin. Moreover, I can't use most cosmetics because it affects my eyes. Conjunctivitis often. Usually I have to also have rather cheap skincare so its harder to find good quality, smooth cosmetics. I live in Poland so most cosmetics available in the UK or the US won't be available for me, but for sure there are interchangables. I also used La Roche Posay cream with 1% of salicylic acid but it burned too, and I stopped. I don't know if ultimately I can use any skincare 😉 and most importantly how to cure my problems with acne, sensitivity and take care of my face and body skin. When I used sunscreen in the summer on the beach my skin didn't tolerate it very well, body was more or less ok, but my face was even oilier and it came off, literally melted on my skin and into my eyes. I don't know how to take care of my skin because of the complexity of it and because cosmetics don't cooperate with my skin and as I noticed usually are too thick for my face. My GP prescribed me MetroGel, Metronidazole, to use once daily in the evening. It's definitely helped with the bumps and redness however I still have lots of broken capillaries and redness in some spots. How do you feel about MetroGel?

I’ve been struggling with rosacea all my adult years (almost 50 now) and I’ve tried everything. You mention that we should stay away from fragrance but then you mention Avene products which have fragrance! This is why we feel there is conflicting information out there. Citric acid is also a trigger for me and unfort Dr. Sam’s products contain this ingredient. I also stay far away with anything that has linalool and Limonene. Obvs everyone’s skin is different and you’ll need to learn what works for you so the best advice I was given was to simplify my routine as much as possible and add products slowly in order to isolate what works for you. Also, learn how to read the inci list and about the chemistry of ingredients. What works for me - Hada Labo Cream Cleanser, a lactic acid for exfoliation, Niacinamide with the Inky List Polyglutamic acid instead of hyaluronic acid, any mild retinol product but definitely not differin bc its way to harsh, a mild moisturizer without fragrance, and a mild vitamin c. I use a bit of self tanner to mitigate some of the redness and try to stay away from fragrance and alcohol in my foundation (I’ve been loving Mac Face+Body and Koh Gen Do Aqua) and blush products, too. Daniel Sandler makes an anti-redness primer that works pretty well. Giving up sugar has also helped clear my skin tremendously. Good luck with your journey and don’t give up!

I am 46, and I think I developed rosacea from 2 years of tretinoin use. During the day it is fine, but come nighttime and I am terrified to wash my face- this is my trigger...water. my cheeks below my eyes turn bright red for 3o minutes after a face wash. I am not washing my face in the morning- I go straight in with a vitamin c and sunscreen and it is fine then.

Ah that colour green looks absolutely gorgeous on you Dr. Sam! It really brings out your eyes! What are your thoughts on chemical exfoliants for those with early stage rosacea? And is sunscreen necessary even if we are indoors all day?

Hello! About the sunscreen, we should wear it even indoors, especially if we spend lots of time near large windows. We won't burn, but the aging UVA rays will still come through. I've been wearing SPF religiously every day for a year now, not only in summer, and I wish I'd started years ago.

Soolantra is great, I have tried to treat my skin without any medicated cream but does not help my type 2 rosacea, stress and other triggers still cause my skin to react sometimes badly. So I have decided to just use Soolantra even if it is needed fr continuos use. With Soolantra my skin still reacts if I eat wrong foods for instance but the skin calms down fast. And Soolantra is a better option than going on antibiotics which could disturb my body balance anyways. Considering a laser at some point also and I use vitamin c serum and hyaloronic acid serum in my daily skincare routine as well as high spf moisturiser. Dupe Ajayi did you have a terrible “purge”? I tried it for over a month and my skin looked like a 15 yr old girl with acne. I had to take an antibiotic it was so bad. I never used it again.

I want to know that, too!! Red/infrared light increases blood flow, so maybe not helpful for rosacea and post inflammatory erythema(PIE) caused by cystic acne…but some says their rosacea cured using those led masks, so I’m not sure😢

I’m not sure whether I have rosacea but my skin is super super sensitive and irritated at the moment. Everything is making it itch and sting, including all my Dr Sam’s products :( My main problem is around my eyes - they are dry and red and itchy. Every product makes them worse - any ideas on a moisturiser that would help? Even LRP toleriane eye cream makes them worse. Any advice gratefully received xx

I didn't hear you guys mention the Paula’s Choice 10% Azelaic Acid Booster in the video but it was listed, so you think its good or not? aha. I am hesitant to try the ordinary because lots of reviews say the consistency is too thick, but the price is nice.

I find that very dry, hot weather is the worst for my skin! Winter and high humidity are the best things for my vascularly-based Rosacea, it goes away in humid weather!

The gentle treatment of the skin is very helpful with my rosacea. I wonder about washing the face in the AM when I get up then exercising only to need to wash the face again after exercising. Is it best to delay the AM face washing for after exercise or was the face three times?

What about Tatchas rice exfoliant? They say it’s super gentle and non irritating. I’ve used it and I have rosacea not sure if it has effected it or not but I’ll stop using it for sure if it’s not good!

Phas such as gloconolactone and aha such as mandelic acid are good options, look for low % and always patch test first

Ella Y The Doctors advised to avoid physical exfoliation. Gentle AHA and BHA exfoliating works wonderfully but I find it helps to gently build up tolerance to these exfoliations if you have sensitive or rosacea prone skin.

what worked to get rid of the redness for me was Dermalax Rosacea cream, you buy it from the pharmacy and they say to apply small amounts on the redness, I do it before I sleep and the redness is gone in the morning. don't get me wrong, my skin isn't clear, its bumpy and has miss colours but at the end of the day its skin, we got blood n veins in our skin bc its not gonna look perfect, especially if ur very pale. but I do recommend the cream, also a cleansing brush works too but use it with face wash. :)

What if CeraVe Cream works for your day moisturizer, is that okay? Makeup does not run. Actually need to reapply moisturizer during the day. Is this okay, doc? (asking for a friend)

Dr Sam please add a skin barrier repair cream for very sensitive skin to your skincare line Im loving your Flawless Light, but sometimes when my skin becomes very irritated I cant even tolerate Flawless Light anymore, it stings and my skin becomes red and inflamed, I think maybe because of the hyaluronic acid. Please please do a barrier repair cream

Could you please reply for this question, I have hyperpigmentation ( maleasma) and was wondering if I should use a toner in my morning routine before Alpha aburtin and Azealuc acid 10% (both from Ordinary) or in my night routine with retinoid ( The Ordinart Retinoid 2% granactive ) and then finish it with nicanamide and moisturiser?? The toner is from ole henriksen dark spot toner Please do reply it would be great help Thank you

Thank you, this is so helpful. I am 34 and have had rosacea since my mid 20's. I didn't tolerate over the counter azelaic acid or the course of rosex (my first prescription) so I have just started a 2 month dose of lymecycline. I will return to my doctor and ask to try Soolantra- should I start this alongside the antibiotics or after? A video about actives use for people with rosacea (particularly vit c, retinol and any place for chemical exfoliants) would be wonderful and specific product recommendations if possible would be fab. You did mention niacinimide and vit c in sensible doses- what % would that be? Thank you both again and look forward to the next instalment!

For years, I have had constantly flushed cheeks despite having a minimal, moisturizing routine. Am I in a state of constant inflammation? If so, that sucks that I'm prematurely aging :(

I have a pustular lesion at the tip of my nose and was prescribed Doxycycline Antibiotic pills but they did not do anything for the 3 weeks i took them. So i decided to stop them fearing the damage to my stomach. But now i am wondering if it takes longer for them to work and if i should go back on them.

I use a daily salicylic acid wash with a tea tree oil base toner and a cetaphil branded facial moisteriser. That combination is the ONLY way I can keep it at bay. Especially the tea tree oil, it settles the redness right down. All this seems counter-intuitive to your advice.

7n regards to not using just a wipe of cleanser,for years I did this as recommended by a beautician, as I've sensitive sk7n and now prone to reddness on cheeks which was getting worse as I was getting small spots on cheeks to and looked like a rash when flared up.I began rinsing my face with plain lukewarm water at night after using a wipe off cleanser to remove spf.this has made a big difference with the red spots,so reddness dose not look as bad if I flush.I use a hyperalegenic toner after for my ph as I feel if I just pop on moisteriser after water ,skin feels irritated.I can't use face washes, I've tried all the gentle ones,just cause problems. I think the residue of cleanser and dirt I though I was wiping clean of was leaving a layer behind, maybe similar to facewipes effect.I've also learned mineral oil flares up my breakouts and reddness

re diet... foods that often trigger easily flushing are also tomatoes and fish which are high in amines, which for many rosaceans is not good

I find stress,emotions,indoor heating and hot drinks are my triggers...My biggest trigger though is 1 week before my period,my skin suddenly starts getting very inflamed,reactive,red,blotchy,itchy,sensitive, pimply and oily with large pores....my skin just looks tragic,then when my period arrives it starts to improve dramatically.....everyone is different but I've always found the week leading up my period creates such inflammation in my skin....

Hi, thank you for this video. Do you always get flushing with Rosacea? I’ve got what I think is perorial dermatitis but having red spots with tiny white heads come up randomly on my cheeks & forehead that can be itchy and never seem to go! No flushing or redness though? Can you advise please? Thank you x

Same issue! My GP originally thought it was eczema or atopic dermatitis of some sort, put me on a steroid topical and it blew up all over my face. I stopped the topical steroid and a nasal one I was on for allergies and the reaction is now contained to my eyes/tops of cheeks but with the current COVID-19 situation I wont be able to see a derm. Would love to hear their thoughts on it!

I haven't found good Mineral Sunscreen. Any recommendations? Actually, The Body Shop Vitamin E Lotion with spf 30 (chemical) I can use on my sensitive rosacea skin and it is great under makeup as well!

I've had rosacea since I was 33 and I'm now 51. It was worsening with every year until I resorted to neat Dettol. It burned my skin but it worked! It cleared the rosacea from my forehead, then chin, then cheeks and now I just have a little left on my nose. I started diluting the dettol with half water as I was worried it might make me ill. I did this every other day in order to give my body a chance to clear it out of my system. Now I do it every 5 days. Recently I've been using Hydrogen Peroxide to give my system a break from the dettol.

J T Vbeam is the gold standard. I had three IPLs and saw no reduction in my redness.

I really like CeraVe PM Facial Moisturizing Lotion, which contains Niacinamide, Hyaluronic Acid, and Ceramides. It is fragrance free and you can use it day or night. 👏 😊 P.S. I live in the United States and was just in Mexico where I picked up Finacea 15% Azelaic Acid Gel over-the-counter at a pharmacy. Unfortunately, it is only available by prescription in my country. I like layering it over CeraVe PM.

Me too. Thought it was vulgaris but it was rosacea. Really my friend don’t eat anything that is Spicy. I follow the rosacea diet and I am 90 to 95 percent clear. Good luck.
