Dear Dr. Prasad, Thank you so much for your videos. You said you're placing the filler deep on the bone. But what about patients with a volume deficit under eyes in the preseptal area (above the orbiral rim edge)? You are probably familiar with this picture after excessive fat removal in blepharoplasty, when the pretarsal roll becomes very prominent because of hollowness in preseptal part. Can this problem also be solved with filler in some way? The only successful examples of correcting this problem that I have come across were on the website of the late Dr. Meronk (now available only in the web archive) and in Raul Loeb's 1989 book "Aesthetic Surgery of the Eyelids" - in both cases solid pearl fat grafts implanted in the orbital fat (behind the septum). What do you think of this technique and why is it so hard to find doctors who use it (if it's still being used by someone)?

i wish i was closer. i think your explanation was helpful but also trusted you and would trust you to correct what someone did with filler. they removed most of it under my eye but they only do fat filler so i am getting another opinion in a couple of weeks

I love your videos! Was curious to know if autologous/donated cartilage could be used for infraorbital rim augmentation?? Ideally it should have a very low infection rate as compared to implants?

Hi @annaframeworktwo1288, Microcurrent devices have limited temperature settings, so I don't use them in my practice. I hope you understand that devices used at home are not professional grade, and the main problem with these devices is that they are used by people with no training in treatment, and are unaware of the time needed in between sessions for healing, and collagen synthesis. I love your videos Dr Prasad. Thanks for sharing the photos and your expertise. Question: does this filler placement work best on younger, thicker skin at the tear trough? If your over 50 or even 60 does the thinner skin around the eye area & tear trough make it more difficult to perform this technique? Thanks 🙂

Hi @TheMijas07, Lower eyelid skin is the thinnest skin in the body at 0.5mm thick, and it is also thin in young people. You can still have under eye filler treatment in your 50s and 60s, but like any procedure, the results do depend on the experience, and expertise of your doctor with filler in the under eye area. Many doctors don't perform filler treatment near the delicate eye area, so you do need to find a doctor experienced in administering filler, and other injectables in the lower eyelids. Hey doctor, Do you use custom infraorbital implants or standard implants? And how do you usually place them? Through transconjuctival approach or through lower eyelid incision? Which approach is basically better so that lower eyelid doesn’t retract?

Dr. Prasad- I am 4 weeks out from lower bleph/ fat repositioning. I had to have the surgery because filler was accidentally placed behind the orbital septum in the fat, so I developed a bag on that side. I still have a small bag and am wondering if that is swelling at this point or if it is possibly left over filler. Also, at what point is it acceptable to get under eye filler to camouflage the leftover bag?

Hi @mariannavarela4454, Sorry to hear about your problem. I would have to physically examine your eyelids to properly diagnose the issue, but fat repositioning does carry the risk of prolonged swelling. Hi doctor, i understand that fillers are the more reliable choice to enchance under eye support, but what if i wanted also get lower eyelid retraction repair for congenital lower eyelid bowing? Wouldn't the results of the operation disappear once the filler dissolved, since the eyelid would again be without support?

Hi @andreamarteddu985, Lower eyelid retraction repair, or correcting a congenital lack of lower eyelid support would normally involve surgical procedures to strengthen lower eyelid support. Filler would be used to treat hollowness behind the lower eyelids, not as a procedure to strengthen lower eyelid support, so the support should still be there if filler is dissolved. This isn’t related to the topic. I had a face and neck lift 5 weeks ago. The incisions around the ears are itchy and looking slightly red. I think I’m well past having to worry about infection? Should I be worrying?

Hi @muhammadtahabutt2072, Lasers can remove freckles, otherwise known as solar dyschromias. However, lasers can cause hyperpigmentation, or hypopigmentation in darker ethnic skin types, so I would have to examine your skin before recommending any laser treatment. Not all fat grafting is performed equally. Injection too superficially into the muscle or above, going into the wrong plane, placement above the orbital rim can lead to complications, featured in your photo examples. Fat grafting is simply a modality, to be used in conjunction with other surgical modalities, especially when addressing an area as difficult to treat as and less forgiving as the tear trough. Ideally these patients would have benefited from transconjunctival lower Blepharoplasty, fat transposition and fat grafting to use as a blending tool rather than as the sole solution.

Hi @k94tran, This does not change the behavior of grafted fat under thin lower eyelid skin, as grafted fat cannot hold a consistent shape once removed from its original blood supply. As an oculoplastic surgeon for over 25 years, I do perform transconjunctival blepharoplasty, as well as revision lower eyelid surgery to correct surgeries performed by other surgeons. Although I don't recommend or perform fat grafting in the lower eyelids, I often have to surgically remove fat grafts as the original surgeon can transfer the fat, but can't address it when complications occur. Hi @lisaaustin4561, I suggest you find a surgeon who is experienced with lower eyelid procedures, including surgery. Many doctors won't inject near the eye area because it's so delicate, so you need to start with a doctor experienced with the eyelids. I hope this helps you.

I don't really want to use fillers since it can move around/get displaced and I hear it can't always be dissolved plus there's the risk of becoming blind 😳 so I've been looking into fat grafting and implants hoping those would be permanent and then I could be done. But now I see the disadvantages of them. 😏 Someone have a bottle of the fountain of youth?

Hi @EMBL2011, Filler can be prevented from migrating with the right technique for placement to put them within anatomical structures, such as muscle, to hold it in place. The risk of blindness can be avoided with a doctor experienced in treating and operating within the eye area. 47% of blindness cases are caused by fat grafting, and only 28% by hyaluronic acid (according to "Soft-tissue Filler-associated Blindness: A Systematic Review of Case Reports and Case Series" published on pubmed), so it is definitely not the best decision to choose fat grafting in this regard.

@@OzodaSuyunova-bz1uz yes, it seems it is the best option at the moment until some predictable permanent solutions introduced in the future. redensity II worked very good for me, but different injectors have their own choice of filler. The most important in your situation is to find some oculoplastic/eyelid clinic or some place where they fill under eyes frequently, then the chances are higher that they know under eye anatomy and what they're doing. I know how hard it is after an unsuccessful blepharo, especially if a lot is removed. I wish you good luck in fixing your problem

It is important in lower eyelid surgery to not remove all fat, and leave a little behind to prevent a hollow appearance, and to have a nice contour in the lower eyes into the cheeks. Filler is likely the option for you, and it doesn't require additional surgery. I hope this clarifies things for you.

If it's only visible in the morning, then it's more likely an allergy, excessive salt intake, or sinus issues. Hi @MrsAnimesmangas, It's important to remember that the brand of filler doesn't determine results, but it's the doctor's expertise, technique, and experience that determine results. Think of filler as the material, or in the case of sculpting, it’s just the clay. The clay itself doesn’t determine the sculpture, it’s the sculptor, or the doctor that determines the final appearance. Hi @OzodaSuyunova-bz1uz, Skin grafts are usually a last resort procedure when there is a skin shortage because skin grafts will likely have a different color, and texture once removed from its original blood supply, and has to take to a new blood supply. Lower eyelid skin is also the thinnest in the body, so potential skin grafts areas don't really match with it. There are grafting materials I use for revision eyelid surgery, but I would have to examine your condition before recommending specific treatments for you. I hope this clarifies things for you.

Fillers work pretty well for your situation, but you need to choose experienced injector who inject under eyes frequently. Unfortunately permanent options are too risky and unpredictable at the moment, but fillers are quite predictable and stay longer under eyes than in other areas

Hi @OzodaSuyunova-bz1uz, The fat removed in lower eyelid surgery is permanently removed, and results can last decades, even permanently. However, it is possible for new orbital fat to prolapse, but if this happens, it would occur many years later. You can watch the video more closely for options to treat hollowness under the eyes. I've been thinking about to contact you for an evaluation since my surgeon said everything it's fine, but I know and people who know me that I don't look the same. I looked worst than before.

Doctor I am 18 year old indian girl, and i have terrible terrible terrible dark circles,the thing with my dark circles is- it's not only under my eyes, it's also in my eyelids, i mean my below eye and above eye area both are dark as hell , so from a far my eyes look 360 degree black ,like somebody has hit me in the eye..... I started having these since i was 12 years old,and they have been the same dark black since 6 years, i m not malnutritioned,nor sleep deprived, but i do think that it's genetic since my mum also has them ,which makes it even more worse , Also after observing different types of dark circles,i think i don't have puffiness or extra fat under my eyes, but i have hollow eyes and i do feel like the 2 main problems for my dark circles are hyperpigmentation all around my eyes and the deep hollowness under my eyes (may be due to face bone structure .) Just to give u a more appropriate image of my condition, from the above video 5:27 condition ,is most appropriate as me, although mine are way much darker,... My doubts: 1. I think even if I would do anything for my dark circles ,still most of the treatments are for under eyes but I also want to remove the darkness on my upper eye lid part , So can u please suggest something for both upper eye and under eye or separate treatments for both.. (I have heard glycolic peels might work for upper eye too , please tell whether it can work or not, or any other treatments/procedures) 2.For some reason ,I don't think topical creams or serums would work for my dark circles ,it might work for my upper eye lids, but my under eye area is hollow so I don't think that they would do any thing, although I am trying to apply vit c serum and a glycolic acid cream at night alternatively for few days on my both upper and lower eye parts ,and have been wearing sunscreen every morning.. So , should I continue this or they won't be effective?,or if there is any topical active ingredient for dark upper eye ,can u suggest some?

Hi @amiisonyondan244, Sorry to hear about your problem. I generally don't recommend treatment for dark circles for someone as young as you. Keep in mind your face is still growing and maturing, so there is still time for this to resolve itself. If you still have the problem when you are a little older, say in your early 20s when your face has stopped growing, you can consider treatment then. I think I'd wait till my 20s for any filler or prp , but till then, I'd try applying hyperpigmentation removing active ingredients on my eye area , and will swear by sunscreens ,and will see if I get rid of them naturally without any cosmetic procedures.. Hi@@amiisonyondan244, You're welcome. Aside from sunscreen, I suggest you consult your doctor about the products you intend to use to make sure they don't make the situation worse. There are many products, especially over-the-counter creams that may not do much, or may potentially harm your skin.

Is there any work or research being done on any kind of permanent filler? It feels like none of these options are great, given that fillers are temporary and an ongoing expense, implants don’t change with your face, and fat grafting is unreliable.

Hi @Steve.Maverick, There are permanent, and semi-permanent fillers made of PMMA (polymethyl-methacrylate). Since this material resists metabolization by the body, they are highly prone to complications like infection, foreign body rejection, and can break apart with time, reaching deeper tissues. Surgical removal of permanent, and semi-permanent fillers is extensive surgery, and not all of it may be removed. Hi@@Steve.Maverick, As I tried to explain in my previous comment, it is the resistance to metabolization that makes semi-permanent, and permanent fillers prone to complications. As featured in the video, silicone facial implants can last longer than filler, but they are not permanent since facial dimensions change with age, so they will eventually need replacement. However, many people find facial implants uncomfortable once they actually have them. But I’ve had filler for my deep under eye hollows, and I can honestly say with my certain anatomy I can’t get the desired volume I want with them and it leaves tiny little bumps on my skin from the filler injections. I’m considering tear trough implants.

Hi @blondy8021, Sorry to hear about your problem. The little bumps in the skin are likely from the way your treatment was performed with multiple pokes through skin, not due to the filler itself. I avoid bumps by minimizing the pokes through the skin. While I do perform tear trough implant surgery, a common concern for patients is that they don't find them comfortable. The appropriate viscosity of filler used in the under eyes does feel more natural than a silicone implant. I hope this clarifies things for you.

Pictures help, but they don't reveal details of internal anatomy. Also, I can't recommend procedures where someone else performs them as they will likely be done differently than I would do it. You would need to have a formal consultation before I recommend treatment for you.
