Correct me if I'm wrong, but I think this is the order in which induction of general anaesthesia is done (also correct me if I made a mistake elaborating what each thing is for): 1. Midazolam, to calm the patient and to produce analgesia; 2. Preoxygenation, to supply all the organs with plenty of oxygen; 3. Fentanyl, to further increase analgesia; 4. Propofol, to put the patient to sleep; 5. Endotracheal intubation, to connect the patient to the breathing machine and to administer an inhalation anaesthetic that will keep them under general anaesthesia for the entirety of the surgical procedure. What I don't understand is what the role of the saline is here and why is it given multiple times?

I'm not an anaesthetist, so no comment about the saline, but if they are get an ET tube, they'll generally get a neuromuscular block first (but probably not for a LMA).

The saline is used as a flush to ensure the full dose of medication is in the vein and not resting in the connector!

the saline flushes the medication to ensure it's timely delivery. the missed sequence of your comment is that the patient must be ventilated after administration of propofol since the patient will be apneic. After confirmation of ventilation, the patient is paralyzed with a depolarizing or nondepolarizing paralytic agent and then intubated for general endotracheal anesthesia.

Can I ask what the “relaxing” medication used prior to the propofol induction was? A benzo or a low dose of ketamine, I assume?

The first one was Midazolame, as it was written on the syringe. I couldn't see the second one, might've been ketamine 🤔

The first drug, Midazolam, is a benzodiazepine. I believe that was the only pre med given in this video. The second medication, the induction, was propofol. The third drug (red label), I think was rocuronium (muscle relaxant).

Is intubation standard with anesthesia?

I'm not an anaesthetist, but generally here in NZ it is standard where a neuromuscular block is used, or there is heightened risk of aspiration.

So induction starts from when preoxygenation and the first anesthetic medication (in this case propofol) is given till the muscle relaxant is given right ? and then the intubation begins right ?

Typically, yep...make sure the patient is adequately sedated (check eyelid reflex/corneal reflex) before proceeding to give muscle relaxants (Succinylcholine) and then intubate :)

This video is horrible. Please verbalize what you’re doing as you’re doing it. For example what medication you’re pushing and how long to push the med over.
