There is no charge, you just have to pay for tickets that you get from post office next to mofa, just in 50 to 60 rupees. No additional fees for mofa, its free. For time, you get the documents on same day but you have to wait there until evening. I also hear that there are not receiving documents directly, they are receiving by courier service (TCS ). Not sure you have to double check.

When you go there for visa, once they receive your documents. They will also inform you when you should come.

Yes, once you enter there , then they will start counting how long you can stay. Its just visa validity not stay validity.
