Are JAK inhibitors the future of treatment in alopecia areata ?
This is an overview of current knowledge about the efficacy of JAK inhibitors in alopecia areata.
The video includes information about tofacitinib, baricitinib and second generation JAK inhibitors.

I have Alopecia Universalis/Totalis, no faciel hair, eye brows, scalp hair, its all gone. I would like to say that I have some spots that have come back but not fully. I am worried of the side affects of these drugs that can increase infection. I had a full set of hair my whole life until 30 years old. I am 33 & a half now, I wonder if there is any hope for me...

With every drug it is necessary to discuss with the dermatologist the pros and cons. Definitely treatment should be tried, in my opinion

hey , I'm from south Afrika and 17 years old. Would the newly approved olumiant treatment have better results than the azathioprine I've been on for 7 months. The azathioprine is not showing much results and my dokter says its the same. After watching this video I see theres a variety. Is there a difference in your opinion?

There are no studies, which compare these two drugs, but azatioprine is usually not a first choice treatment for alopecia areata

There are no scientific data on this. On the basis of my general experiance with severe alopecia areata, I would decrease very slowly the dose (e.g. every second day etc) and react to signs of disease (re)activation either clinically or in trichoscopy

Thank you! Every drug may have a site effect. However, when treatment is monitored by a doctor the risk of a permanent site effect is very low.

@@SunnySharma-ve3jw I would be a moderate optimist. In alopecia universalis regrowth is only half of the success. The other half is not loosing the hair again. I believe that within the upcoming 5 years there is a chance for new very good drugs for alopecia areata. Also some drugs used in atopic dermatitis may be sometines effective in alopecia areata

what is the best treatment for alopecia right now? (i’ve done two rounds of steroid injections and my 3 biggest spots have recovered almost 80% but my other 3 smaller spots aren’t doing very well although there is some hair regrowth in there) my doc now has suspended with steroid injections i’m continuing with topical ones. thank you!!

This is a very complex question. My other video kzfaq.info/get/bejne/qKmlddJ8rJ-YeZs.html explains why. For details about adverse events please see the "Summary of Product Characteristics" for every drug. The most common adverse effect is an increased tendency to develop infections.

prof. Lidia Rudnicka Dr. I am from india. My daughter has started falling hair in patches. lot of hair she is loosing every day. Dermatologist Dr. Says she suffering from Alopecia areata. Dr has give TNIB 5 mg ( tofacinib) tablets almost a week now. My daughter is noticing more patches and more hair loss. Dr is TNIB medicine good for her hair growth. I hear from people this TNIB 5 mg for arthritis. Can I continue with that medicine

Do you have to take this your whole life or can you stop taking it once your hair regrows. What is the point in this cure if it needs 1 year to fully grow back but relapses after 8 weeks? And its 2022 now are there some serious side effects from xeljanz?

It requires maintenance therapy. How long - probably it differs significantly from patient to patient. Even in the case that there is a relapse, it is important to keep the hair follicles working, so that they don’t disappear because of inactivity (for those for whom a future chance for regrowth is important)

Hi madam, I had multiple patches of hair loss on head and on beard area. Since last 1 year I m taking tofacitinib tablet 5 mg twice a day. Before that also I was on azathioprine 100 mg for 3 months. Now recently I read somewhere that tofacitinib tablet can cause malignancies or heart diseases. Is it true? I m so worried now and in dillema whether to continue this tablet or not for alopacia. Please guide me madam.

All JAK inhibitors are currently off-label in alopecia areata. They may be available for other indications, but in every case, these are prescription drugs.

There are no data on this. Most probably there is no relationship between ASIA syndrome and alopecia areata.

@@profLidiaRudnicka Many patients affected by injections of “biopolimers ” in the breasts and buttocks who go to a specialized clinic in Bogota, Colombia, claim to have or have stained alopecia areata and other losses such as efluvium, it is an area little explored that would be important to investigate. Yes, tofacitinib was first investigated for arthritis and is registered for rheumatoid arthritis. It is being used in medicine off-label in also other diseases including alopecia areata. I have responded to your other comment here: kzfaq.info/get/bejne/qKmlddJ8rJ-YeZs.html

This is difficult to treat. JAK inhibitors give some hope. Children with alopecia totalis and atopic dermatitis may also respond with hair regrowth to treatment of atopic dermatitis
