Hello friends, today I'm bringing you a video overview of a very important condition called cervical myelopathy. This condition causes people to have balance and coordination issues as well as problems potentially with their hands. Frequently, people think they are "just getting older" or have been checked for vertigo without any answer. This is a progressive condition that can cause permanent disability but is a condition I treat frequently. Learn more about it in the video above.

Thank you so much for this explanation. I was diagnosed with cervical myelopathy and just had ACDF surgery. Makes sense why I’ve been dropping things the past few years, falling, off balance, and why so many actions I have to demand out loud for my body to eventually respond.

I had right single door C3-C5 Laminoplasty a month ago. I played Judo when I was in college. I’m 38 y/o, no history of trauma or accident, or no known injury during my playing years. 8 yrs prior I had been experiencing clicking sound in my neck and my right knee feels weaker going down the stairs. 4 mos prior surgery, I had numbness in the right hand, wobbling of right knee, and right leg weakness. I never experienced neck pain. I had an MRI which showed C3C4 Severe Disc Protusion and C5C6 Disc Arthrosis w/ Anterior Autofusion and right foraminal narrowing, Congenital Spinal Canal Narrowing, and Myelomalacia at C3C4. All symptoms went away after surgery, except for numbness but it isn’t as bad as before. My main concern is 4 wks post-op, I have been having left shoulder blade stiffness and spasms when walking and standing for at least 30 mins and sitting 90 degrees. I hope it gets better and doesn’t become chronic. I don’t know doctor if this is a normal progression or something bad waiting to become a chronic condition.

@@logan9920 I use pain meds, Pilates when neck pain worse, and walk regularly. I avoid sudden movements or impact activity. I have suffered poor tolerance to emotional stress on physical and neurological sypts since having condition so removing stress as much as possible vital for me

Awesome info here. I’m currently going through all these symptoms and they have gotten much worse over the past few months. I have had it on and off for over 11 years sometimes plateauing for a couple of years. Now the incidences are getting closer together with more symptoms. I’m waiting for the results of my MRI and the surgeon did discuss the likely hood of me having myelopathy. Thank you for this info. It has been very educational!

I never had neck pain, once he did fusion C3-C7 it destroyed my rib cage. Deformed it and worsened my hand and tortured my stomach

I am weak and in agony and I understand why now I had l4l5 decompression surgery September 2020 and my neck has got worse since everything you describe are the symptoms I have including vision deterioration I am frightened it’s gone to far and the thought of surgery but through you video I have a better understanding Thankyou so much for taking your time to help us all Sandra

Thank you soo,soo much. I was recently, finally, approved for an MRI to figure out my severe shoulder pain. My diagnosis was military neck, anteriorlisthesis of c2 c3, c3 c4, retrolisthesis of c5 on c6, convex left of lower cervical spine. Multilevel intervertebral disk space narrowing with chronic degenerative endplate changes most advanced at c5 c6, end plate edema at c5 c6 modic type 1 changes, facet arthritis on the right c2 c3. C2 c3 posterior disc uncovering, right sided uncovertebral joint hyper trophy with facet arthropathy. Right neural foraminal narrowing. C3c4 disc bulge central/right paracentral disc protrusion measuring 2mm in ap dimension.unconvertebral joint hypertrophy with facet arthropathy contact with impression/flattening along the ventral surface of the cord. Moderate spinal cord narrowing c3 c4. The thecal sac measures 8mm in ap dimension c3 c4 severe right neural foraminal stenosis with exiting nerve root impingement. C3c4 moderate left neural foraminal narrowing with contact/impression on exiting nerve root. On c4 c5 level- disc bulge uncovertebral joint hypertrophy with facet arthropathy. Contact with impression along the venal surface of the cord. Moderate spinal canal narrowing, the thecal sac measures 8-9mm in AP dimension On c5c6 disc bulge with endplate osseous ridging. Broad based central disc osteophyte complex.uncovertebral joint hypertrophy with facet arthropathy. Contact with flatteningalong the ventral surface of the cord. Moderate spinal canal stenosis. The thecal sac measures 7mm in AP dimension . Moderate bilateral foraminal stenosis with partial exiting nerve root impingement. Perineural cyst within the distal left neuroformen.c7 t1 normal disc contour facet arthropathy. All of this sounded like a lot to me but the surgeon said it wasn’t bad. That a 15 minute surgery would help the nerve pain. I asked what would happen if I didn’t have surgery and he said “nothing. You may continue to have the pain or you may get better. That’s all”. But your video sounds like he is wrong. So thank you. I’m going to seek another opinion.

Thank you so much for this very well designed and well informative video, I have been suffering from neck and shoulder pain along with clumsiness and uncoordinated lower limbs for several years, it led initially to a diagnosis of fibromyalgia until I started having shouting pain in my arms that prompted my family doctor to finally order a neck MRI that showed severe multilevel degenerative disease along with severe central stenosis at C6-C7 due to a herniated disc, despite all this findings and increasingly severe impairment due to the pain and lower limb weakness causing me difficulties walking my doctor is still hesitant to refer me to spine surgeon as my EMG was normal. Your explanation about staircase evolution is spot on with how my symptoms evolved, I go through some stabilization but as soon as I start feeling good enough and try to be more active my symptoms recur worse than before which was extremely confusing with the fibromyalgia diagnosis as they were considered to be fibromyalgia flares ups . What an amazing and insightful video. I am having many of those symptoms but it is not diagnosed. My trust has been compromised with my current provider for a variety of reasons.. I am seeking out another. Regardless, my MRI shows spurring through C2-C7 with moderate thecal sac effacement c5-c7 and severe foraminal narrowing. I have struggled with neck and back issues since I was a child due to a neck injury. My arm and hand tingles and goes numb and has been doing this for several months. This video has helped me understand the anatomy of and resulting functions of the spine. I feel like I can now make informed decisions. Thank you so much!

Hi, a guy from the UK here! 👏🏻 I’m 52 and recently felt a trapped nerve at the base of my neck, that was nearly a year ago but I haven’t fully recovered. I do get weakness of the right arm, and pins and needles throughout the arm. In fact, I can position my head/neck at a specific place to create the numbness and tingling. I want to go to the Doctors but I’ve never had much faith in our “ beloved NHS “ . I know for sure, I’ll never get an assessment as thorough as one of your videos! I had a two level anterior cervical discectomy and fusion in 2012. The highest level affected was C5/C6. I was 44 years old at the time. I had all the symptoms described, dropping things, fumble hands and feeling like I was walking on a boat at sea. Once one part of the spine goes out, it is likely to affect the rest. I try to keep the surrounding muscles strong but I was young for this, most people are past retirement.

In case it makes you feel better I'm having 2 levels at age 33 (and probably should have sorted this 2 years ago). I also have other bulging discs, and arthritis and it is apparently too bad for disc replacements now. So I may end up with lots of my spine fused. I like to watch scolosis patients recovery stories - they have it so much worse. There's a very young woman (maybe 19?) who had her entire spine fused after a life of pain. She took it like a champ despite so much pain. If she can do that....

Hi everyone , really liked this video, i realise everybody tuning into this channel has a medical issue , some worse than others but you all sound like fighters and that is reassuring for me . I am 55 year old male and not entirely sure how long I've had any condition but both my feet were broken at birth so had special shoes etc , over some years early teens i was sporty when my cartilage exploded in my knee aged 15 then 2 years later a big spur on same knee damaged nerves to have it removed , had back problems from 17 and just got on with it as you do , had 5 procedures on that knee over the years then surgery on both shoulders for decompression on left and same on right with a full tear rotator cuff thrown in for good measure , both shoulders over 5 years from age 45 , so i really cant say whats going on with all the spur growths , 2021 tendonitis right achilles which still bothers me always swollen and painful and developed binocular diplopia , as you can imagine i was getting rather pissed off , so pushed for MRI and neurologist confirmed spondylosis 2022 with cord compression , i was in shock and didnt ask enough or correct questions, he knocked me for six when he mentioned surgery , i said no and stood up , thanked him and left , havent spoken to him since but i am getting gradually worse and cant or wont bring myself to call him for an appointment , YES i am terrified and negative, i havent spoken to my family about this but now my son has back problems . Any advice please ?

After seeing several videos, it seems for surgical options, the new ultrasonic technology has the best chance of helping those who are diagnosed early. This is essentially using a “Dremel tool” bit of sorts, designed to be safe around soft tissue, to remove bone and calcification and make the spinal canal larger. This is best when diagnosed early and done early enough before loss of function. Considered better than fusion for most, since fusion creates more problems later on. And, I have also learned that there are some non surgical approaches that appear to slow the progression, at least in some patients. I am just stating this based on the many other literature and videos I have watched.

I am 55, diagnosed with early cervical spondylosis with nerve pinch at c6-c7 and c_spine MRI shows spinal stenosis. Hope to learn more about this subject and I am in regular medical consultations. @@argentinarodriguez4170 hi! Thank you for following up. My surgery was a success, I had a anterior Cervical discectomy and fusion. Two disc were replaced it took 3 hours because there was other work done. The only thing I still get are headaches once in awhile and neck pain but the whole reason for the surgery was to replace the disc and straight it so it won't hit my spinal cord. It was pretty close to it. But other than that I'm well thank you. God bless.

This is the best video I've seen explaining this; I was diagnosed in 2018 when my arms got really weak. It wasn't bad enough to warrant surgery. Today - I can hardly use my computer, I'm dropping things, and I use a cane sometimes to walk. I asked my doctor for a new MRI and she didn't agree and is sending me for nerve testing instead. What are your thoughts? She doesn't think it's surgical.

I think anytime you feel that symptoms are worsening especially if you were diagnosed previously with myelopathy, it is likely a good idea to get new imaging if it is over 1-2 years old.

I am in UK and a patient at the National Hospital for Neurosurgery and Neurology in London. I have CSM. My pain is down both arms on rising from bed daily. Yesterday I had my first steroid injection at C6. I have spinal cord compression at C/5 and foraminal stenosis at C6. My Neurosurgeon wants me to have another injection next month (Aug 2022) and I see him for consult regarding decompression surgery on Oct 4 2022. Today is the first day I have had no pain at all from the arms which feels great. My diagnosis was only six months ago and progression has been swift in the last few months (now July 2022). I am a retired nurse so I research everything! I know surgery can only stop progression of my disease process but it could give me more years of painfree life hopefully. I trust my Neurosurgeon completely. I found your video one of the best that I have looked at online and thanks for that. I wish more UK Neurosurgeons or Spinal Surgeons would make these videos! Thank you again.

best of luck to you in your journey. the statistics say about 2/3rds of patients will progress but they progress at different rates. that is why we tend to want to follow patients with csm closely to pick up on subtle changes in their exam/history.

One other treatment other than surgery that can be considered is Cox Flexion Distraction of the cervical spine. This of course with be co-managed with either a neurosurgeon or orthopedic spine surgeon.

My Dr's are telling me that I have Primary Lateral Sclerosis. But my MRI of my cervical spine reveals myelopathy, 2 times in the last year MRI's have shown myelopathy. Now while there is only mild stenosis of my spinal cord, there is a large portion of my spinal cord that showed 0 spinal fluid, on multiple levels of my spinal canal, I suffer from speech issues, balance issues, clumsiness with my hands difficulty with fine motor skill like buttoning shirt's. Now I've done research of my own independent of my Dr's, and have found that the symptoms of PLS, and myelopathy mirror each other almost exactly. I've also discovered that the lack of spinal fluid means that my spinal cord is not receiving the nutrients necessary to keep it healthy! However I can not convince my Dr's to agree with me. The diagnosis from the radiologist, who read the images said myelopathy yet my Dr's say no! Can someone please tell how to deal with this? I'm very much in a fight not only for my mobility but for my life. I fall repeatedly, have fallen down stairs and severely injured my foot, and ankle to the point that it needs surgery to repair it. It feels as if I'm being ignored, and taken advantage of because of my speech issue. I can still write, but it's very difficult to cover everything that needs to be said in a 30 minute appointment.

Thank you so much for sharing this information with me. I did not know how to read my MRI photos. For the past year I thought I just had arthritis in my hands, shoulders and neck. Because I was having pain and fumbling when doing daily activities. Next week is my surgery to correct the problem.

Doctor what type of surgery do you think I need and how long is the recovery? This is what it says on my report “There is a long-standing marked constitutional canal narrowing with further encroachment on the c3-4 canal and ( to a lesser extent ) c4-c5 canal by disc osteophyte bars. There is marked c3-4 cord compression and signal abnormality ( which is likely to be long standing) with additional incipient cord impingement at c4-5 and c5-6”. Thank you doctor for your dedication

hi dr i just want to ask you i had cervical spine surgerger last year nov 2021 c1 c7 my problem before surgery is only my left foot weakness no pain and like heavy feelings in my hips no other complains only difficulty of walking left leg operation done last nov 2021 then jan 2022 operation fo my L2L3 i stayed in the hospital untill april discharged april 62022 but unluckily i did not see any improvement i can walk but iam still using a walker i have numbness on both legs i feel like my brain is disconnected to my body . before i can not move my foot fingers . i need to imagine first to move my foot fingers my foot dorsnt follow what i want to do do like moving . but lately i can move it now i did my exercise always even in bef . how shoul i experience like this dr? i feel like i cannot carry my neck very heavy my legs so much numbness when i sit i feel like my neck is unstable . is this forever doctor ?

Nobody explained to me this step change. This helps me so so so so much to understand I am not crazy! I couldn't understand why I'd have one set of symptoms for a month, and then they'd go away and I'd get another set, and then the old ones would come back and it just keeps getting worse. I feel bad I waited so long :(

I waited 7 years. I’m 31 and had this myelopathy at 25. I regret for waiting this long. I’m going for surgery soon hopefully within this year.

​@@yes619I'm expiriensing the worst symptoms and I'm only 26 😢😢😢 GP has no idea and in hospital they outright refused to diagnose me because I'm having burning pain in between my shoulder blades whilst I'm walking. And tinnitus, vertigo, sycnope, shortness of breath from time to time, problems with vision, right eye sees less, numbness in head, right elbow, pins and needles sensation in back. I became very depressed and hopeless, even suicidal at times. I don't understand why it's so bad here in Ireland 😢😢😢 Will try to go to another hospital in a day to try to convince them to look it up.

@@ArmaghaniSpine I started dry fasting. Have been doing it for 1 year now, 3 days a week. I feel much relieved. Pins and needles have reduced, I can feel what I touch now a little bit better. I believe the body can heal itself if given a chance!

35 at last stages of cervical spondylitic myelopathy found out when I was 31 years old started trying to get help at 26 took 5 years to get my first MRI the worst are the migraines started back of your neck worked away to the front of your head back your eye they it's fucked up in the last for 72 plus hours you feel coming on your fever sweating cold 48 hour mark u start puking 72-hour Mark starts to die down it goes away and it always happens when I sleep the wrong way for too long and then the tingling numbness in the fingers and arm electric sharp stabbing pain from your neck find shoulder blade then tricep the burning feeling between shoulder blades and the back of the neck the electric jolts pain shooting from back of the neck all the way down to feet

Can muscle cramps such as hand cramps be a symptom of myelopathy? Or radiculopathy? I noticed holding my iPhone for an extended period of time or using a screwdriver cramping started on my affected side.

Thank you!! My honey was injured at work July 04, 2015. We have been fighting the insurance company that kept denying his surgery. 8 years later and he is not the same man. He can barely walk, he cannot hold a spoon, open a bottle of water. WE never really understood his diagnosis until watching your video. i have this from an auto wreck. got hit at a red light by a dumb B driver. as i sat in traffic. i have been under pain management care for 12 years. i was disabled by this. but fortunately my symptoms did not worsen a lot over these ears. until almost a year ago. i got hit again. now my hands go numb, pain down my legs. i never had. falling down. and memory, vision, where is this dr. i really feel he knows his business??? .

Not in video for unfortunately. PTS does mimic spine issues but it is something that gets better with time albeit slowly.

Hi Thank You for this video I saw a Surgen and he said do you have pain going down you’re arm? I said not yet but I have pinching in my neck and it gets worse when I turn my head to the left my MRI shows a bulging disc he said he didn’t recommend surgery instead get shots by pain management then they burn the nerve …After watching this video I feel like I should get surgery sooner than later. Like you said to stop symptoms from getting worse. Very frustrated. What do you think about this?

Had large extrusion at c6-c7, with severe canal stenosis and severe cord deformity. Edema and cord signal abnormality at and below level of extrusion. It was probably there for a while and symptoms gradually worsen. 5 months from complaint til first MRI, C6-C7 ACDF a month later, 9 months after 1st surgery, Anterior C6 Corpectomy with C5-C7 fusion. Ortho who did first surgery was considering a 2nd surgery from posterior, neurosurgeon wanted to do a 2nd anterior surgery. Neither surgery helped symptoms, plus have severe DDD at L5-S1 with large extrusion. It sucks because I can’t tell if symptoms are coming from the cervical myelopathy, L5-S1 extrusion, or the 2cm x 2cm x1cm cyst at the T12-L1. I’m only 40 and so many issues and told that symptoms will persist and hopefully surgery stopped the progression of the myelopathy

Hello sir ,my father is having tightness in lower back . In 2016 she had acdf C4-6 surgery due to compression of nerves .again in C4-6 he has myelomalacia but he dont have issue in neck . Is surgery is required .

My recent MRI says I have cord impingement at c5-c6.. it’s very obvious on the scan.. but my only symptoms are bad neck pain on the right side that goes behind my ear and above my ear.

I have moderate to severe canal stenosis and cord flattening at both C5-C6 and C6-C7 with posterior bone spurs and narrowing, and multi-level foraminal stenosis. My symptoms have been slowly worsening with numbness and tingling on my hands and feet, neck pain, unsteady hands, balance issues, and positive Hoffman. How severe does cord compression need to be before surgery is recommended to prevent serious damage from being done? How severe do symptoms need to get before surgery is recommended?

I have this problem, had the fusion and it didn't work, my Spinal canal is nearly cut off 3 through 7 , because of time off work, lost job and insurance, because of my original insurance wouldn't cover the a newer procedure. I have permanent numbness in my right pinky and palm, make sure you do your research on who does your surgery and get many opinions. I really don't know what to do now . I'm 48 years old

Is this MRI done laying on your back? On your side? And what about movement? MRI is a snapshot in time... Many abnormalities show up with movement
