I have mentioned but again as I say history will vary from case to case. You cannot ask everything to every patient.

Thank you for watching, whenever there is any chief complaint we always try to extract more from the patient, just to rule out or rule in different diseases..for example if someone complains of Chest pain then we will be asking about associated symptoms like palpitation, dyspnoea, sweating, swelling of legs etc to find out if the chest pain is bdcause of cardiac origin. I hope the explanation was of help

No basically it should be in patient's own words, we better avoid medical terminologies while describing patient's present illness.

It is important in some cases like in case of penile cancer which is seen rarely in muslims and jews because it is compulsory in their religion to perform circumcision
