Party was proceeding through a standard dungeon crawl, and encountered a room with loot. A brilliant sword of light, which startled startled them, and as they dropped it the blade effortlessly carved through the stonework, resting on the hilt. Feeling an intense desire to retain their limbs, they used mage hand to return the sword to its scabbard and place it delicately on the wall where they found it. They considered the trap solved and moved on.

A friend gave us the wand of Orcus, he laughs, two of us suicide trying to attune, and he didn't think we all would kill ourselves to laugh back at him. The third guy got a nat20 and attunned. Then he did a dungeon that needed to sacrifice the wand of prcus in a chest to rebalance the party, we disbanded that party a few sessions later due to super unbalance random fights, like, 6 lvl 6 with 10hp or less each and no more spell slots and 10+ chain demons with pack tactics appear from nowhere type of unbalance.

In one of my games i gave each member of the party 1/4 of a regenerating deck of things; A magical relic of regenerating cards you can pull from once a day that gives you a random item from the shop table. It worked pretty well for a gambling themed campaign, until two sessions later where the monotone halfling paladin accidentally seduced a faery dragon and got a one-time-use **supreme luck** scale i had meant for them to use in the boss encounter to beat the silly RP mechanic of the gamblar dragon who managed the region. The warlock got meta knowledge that combining the decks of things would turn it into deck of many things through a good communing roll with their patron, so they did so... Then used the paladins luck scale to draw, specifically, the card that lets them rewrite reality. For the next four sessions before i put the kabash on it they would rewrite every bad roll and event by reversing time to the previous round/day.

Player here, in character creation I ended up creating a character that was just wildly out of tune with how powerful the rest of the party was. Essentially, my character was able to give out overcharged battery packs that made everyone's weapons stronger, but due to the facts that we didn't track ammunition at all in the campaign that we were playing and that the dm has designed it to be easier because we were a group of younger players, that relatively small buff of two extra damage each time damage was dealt ended up taking over encounters. I noticed this, and as a result just stopped using them except when we knew we were going into specifically dangerous areas and instead leaned on other aspects of my character turning him from a mad scientist type character into a a fencer with a specialty in protecting his allies by forcing a couple enemies to target him instead.

I've had one or two occasions, but my favourites have to be the times I've handed out stuff with enchantments of Elemental Unbinding. This is a concept I borrowed from a DM/GM I played with in uni, who gave us (well, more specifically, me) the Sword of Elemental Earth Unbinding near the end of a campaign using 3.5 era OGL rules. What does Elemental Unbinding do? Well, anything that touches the blade of a weapon so enchanted has all the bonds of that classical element immediately dissipate (with a commonsense limitation regarding size: if the weapon was sized for a Medium creature, the effect will spread to encompass an entire target of that size or smaller, otherwise it's limited to stuff actually in contact with the blade). This meant I was landing 1 hit kills on everything below Large, and effortlessly lopping off the limbs of bigger things (and carving through what turned out to be load-bearing support columns of the dungeon we were exploring, below a lake bed. Whoops.), as pretty much everything solid partakes of Earth- one chop and any human (or orc, elf, dwarf, gnome, etc.) was suddenly reduced to a fluid mass of the same volume. Of course, I could never sheathe the damn thing (it'd just disintegrate the scabbard, followed by me), and if I ever dropped it/put it down it'd disrupt a hole straight down below the blade and fall to the centre of the planet, so the OP nature of that one generally solves itself pretty quickly. What of the other classical elements? Well, the Dagger of Elemental Air Unbinding is kept in an airtight scabbard, because when drawn it will destroy all gases that reach the blade, at a rate that will reduce the breathable air pressure noticeably. Not a good idea to unsheathe this one in an enclosed space if your party has lungs, and definitely worth keeping the suffocation rules to hand. The Pike of Elemental Water Unbinding: stick it in liquid, and it'll instantly turn to gas (of course, if that liquid is, say, the blood in someone's body, they may well explode from the sudden difference in pressure). Responsible for major droughts and desertification, just through contact with the moisture in the air. In fact, the least overpowered one is possibly the Staff of Elemental Fire Unbinding. This will put out any fire it touches (killing any fire elementals or fire genasi), and inflict the Paralysed and Incapacitated conditions on anything else as it snuffs out the spark of life, stopping the heart. If they aren't revived sharpish (or they are hit with Lightning damage), then it's curtains. (oversized creatures may make a CON save, taking a level of Exhaustion instead if they pass). Definitely not a good idea to try and wield with bare hands. How did I 'fix' it when my party finds one of these? I don't bother. You see, I don't do "encounter balance" according to the challenge rating (not least because the CR system in 5e is horribly flawed and inaccurate). I run my campaign worlds according to "does this make sense?", and by running through the logical consequences. So far, I think the longest anyone has had one of these before deciding that literally world-ending power is not something they want and getting rid of it (or trying to, at least) has been 3 sessions.

My bro gave the barbarian a op greatsword in a 3.5 campaign he ran years ago. It did 2d6 base damage and did an extra 1d6 of Sonic, Electric, Fire, and Cold damage as well (for a total of 6d6). The barbarian got it around 4th level, and at 6th level I pointed out that as a wizard I could do 6d6 once a day with fireball or lightning, but the barbarian could do 6d6 twice a round (more if his cleave/great cleave feats triggered giving him extra attacks) with his sword. It wasn't too long after that where we had an encounter with the BBEG who demanded we give him the sword. Of course we refused so he cursed the sword (without touching it or giving the wielder any type of save) to permanently drain a level every day when wielded (and it accumulated negative levels every day it didn't feed). If the level drain killed the wielder they'd immediately raise as a powerful undead under the BBEGs control and go to serve him with the sword. Every day it went without feeding it also got colder (the chill of the grave type of thing). So we couldn't use the damn thing any more, but we couldn't get rid of it because whoever touched it would then carry the damn thing off to the BBEG. So we ended up wrapping it up really, really well and stuck it into a bag of holding that we used as a freezer.

I played a druid/rogue support character once who's primary purpose was sneaky rougue stuff and supplementing the cleric's heals when nessicary. It was a high level campaign and over the course of several sessions my GM randomly rolled up the Staff of the Woodlands and an amulet of Proof Against Detection. Combine these two items with Expertise in stealth and I could essencially become invisible At-Will and couldn't be tracked by magical or non-magical means. Although it really only came up on occasion, this combo broke one particular sequence in which all but myself and our barbarian got captured by a hostile group. Barbarian went and found a safehouse for us to meet up at, and I quite litterally just walked into the fortress where the party was taken, made sure they were ok in their cells, went and found their gear, then returned it to them, and casually walked out of the fortress without anyone noticing I was there. I even snuck into the party's cell and re-cast Pass Without Trace to help them with their escape. It was kind of a nutty session.

Our dm gave us a lightning attack that charged with distance traveled and had no cap. We were looking to test it out but managed to not find any of the planned fights for 3 sessions straight so the player that got it decided to just blow up a nearby area. Unfortunately, it had charged up so much it blew up the water area we were currently in (evaporating all of the water), the ice area we came from, as well as an arena and cemetery we had been to before that. The dm ended up destroying the source of the attack at his earliest chance in-game to prevent future mishaps. 😅

Not so much given out, but at around level 10 the party of 3 was sent on a quest to infiltrate Vecna's archive of all secrets to find some hint of where to find a religious relic. The way it works is that each person that pays the toll is allowed to leave with one secret. The Wizard found the hint to the relic (Dispater has it), the Cleric picked up a Talisman of Torment to deal with fiends, and the Fighter was instructed by a mysterious voice to find some sort of oil known as Meersalm that he knows is relevant to his deity. Being a tanky boy, however, the Fighter thought he could handle whatever consequences a second item would deal, so he took a Belt of Storm Giant's Strength and used his UA Indomitable to suffer a total of only 5 levels of exhaustion instead of 6. He had to be dragged out by the Cleric and nearly drowned when it turned out the way out was a portal into the surrounding swamp. Since then, bounded accuracy has been a joke for him, compounded by a Tempest Cleric and War Wizard that respectively are able to destroy everything and succeed on nearly every saving thrown known to man. That isn't to say I've done anything to take it back, or there haven't been consequences. Fighter made the choice to take the risk to spite THE Evil Guy, and it paid off, but to get there the party had to ask for help from the local Vampire to solve a puzzle they would have next to no way of solving properly. Because she knows about the Occluded Empire, she knows she can just get the Sword of Kas because it's Vecna's creation, a sword that famously has several beneficial and detrimental effects of the DM's discretion, in this case the classic "holy water near you spoils" and "immunity to one damage type (Radiant)" are extra fitting for a creation of Vecna. So now, on top of whatever Vecnite cultists will be sent at inopportune moments to exact revenge on the Fighter for his greed, they are directly responsible for a Vampire that can go out in broad daylight and can't have its Regeneration turned off. Such is news that the local Church of Light, the sworn enemy of Vecna since his time in the mortal realm, I S N O T H A P P Y W I T H ! Considering the party is all about peaceful solutions and not throwing their weight around, strong-arming them with the roleplay threat of being a disappointment to the Good People that helped them is far more effective than any broken statblock.

Not a DM, but it actually happened to me. I was playing a campaign and my character, a warrior prince, found an extremely powerful sword called Oblivion(I'm sure you can guess why it was called that) from a mysterious man that was supposed to deliver it somewhere, presumably to the demon king we would eventually be challenging. But immediately, the DM noticed that it only took a few attacks from me to kill a powerful enemy that was meant to take up a good chunk of the session time. So he made it so that Oblivion was so heavy, that my character had to take at a turn to actually lift the thing and another turn after the attack to catch his breath after using it. On top of that, my character could only unsheathe the sword if it deemed our enemies worthy enough of its power. So, in short, it was a ridiculously heavy sword that gained sentience and an ego bigger than the continent we were traveling across. But it felt so satisfying to kill the demon king with the sword that was supposed to be his.

Another one just popped into my mind and decided to share it quick, though to be fair I never had a chance to fix it. I was running a published Eberron adventure that had an early boss of the dungeon as a pair of young black dragons that had a necklace of fireballs in their hoard. Turns out the final boss of that dungeon (but not the whole adventure) was a half-dragon mummy (who happens to be vulnerable to fire)... The player who had the necklace didn't even wait to see what was coming out of the tomb, but just chucked the whole necklace in as soon as the lid opened and nearly fried the fighter who happened to be Exactly outside the radius. I guess they were still annoyed about the vampire from the previous adventure who gave them so much trouble. ^_^

I don't know how op it will be further down the line since we're only level 3 but our DM gave our ranger a dagger that when attacking with advantage deals and additional d6 of damage after our first boss fight. Now this dagger is the most effective weapon our party has by like... a lot thanks to a simple combo: favorite Foe+ Zephyr Strike + dread ambusher. This means that in the first round of combat he can deal 1d4 + 1d4 favorie foe 1d6 + 1d8 slashing + 1d8 force +3 plus the regular attack that deals 1d4+ 1d4 favorite foe

In my first game I was given a Vorpal Longsword at level 2. Looked it up and everywhere online said that Legendary Items should never be given out that early. I begged the dm to swap it for a +1 or even a Flame-tongue if he still wanted an OP weapon but he said it was fine. Scratch to 1 month later they hard nerf it by taking away its +3 and giving it 2 charges which after used will shatter the blade. I would’ve been completely ok with that had they talked to me about it beforehand but the change was mid-session. We were interrogating a slaver and he was not seeing the error of his was so for flavor I said I ended the conversation by decapitating him. The dm used that as the opportunity to tell me about the changes and how that was one of my charges used up.

Yes!! I gave my friends a “wish” stone on their birthdays where they could use it to get an item or have advantage all session long. (That’s already an OP item) but the big one was something called The Tax Blade. My buddy (a dark elf rouge) wanted it, it has 10 slots meant for gold coins. While out of battle you could slot in 10 gold and during battle the blade melts them down and give +10 to attack, damage, and did extra fire damage on top of it… lets just say he would end up killing my boss in one swing using all the charges and rolling sneak attack! 😂

Shortened story: Not an item, but I screwed up character creation for my players, had a minor dyslexic moment and this was the first time GMing any game, so we ended up with a orc demigod, we had to use a d20 plus a d12 just to give him a CHANCE to fail, it was that bad. Mind in this system you were supposed to use 3 d6s, the dice could not count high enough for any encounter, so that d12 I took from someone else because I didn't have a dice set at the time got nicknamed: the Khane Dice. But still one of my favorite characters to date and would not change a thing, especially since we rolled with whole demigod thing at maximum speed, just I had to adjust the encounters so normal people could survive but also so a demigod couldn't breeze through it either.

For me, it's quite simple. Most campaigns are not ones that are going to be run across multiple parties with different players. It's the story of those specific characters at that specific time. And, because there's also likely no other spectators to critique the narrative of the players as an outsider, it is perfectly okay to start powerscaling things. Make the challenges appropriate to the party's strength rather than forcing the party to conform to your challenge level. It's the "Yes, and" approach to DMing in regards to difficulty scaling.

This is basically the story of my oldest brother's DnD campaign in a nutshell. By the time the party was lvl 10, they were in possession of a genie lamp, about half a dozen Pokemon Master Balls (homebrewed by my brother, who doesn't understand how Poke Balls work) and the Buster Sword from Final Fantasy (which of course was in the barbarian's hands). He eventually had to throw a CR 23 monster at em when they were just lvl 12 so he could genuinely challenge them... and they clapped that thing the first time around too because my brother didn't learn his lesson and gave the fighter an item that could reflect projectile spells back at the caster when said CR 23 was a primarily spellcasting opponent. ...emphasis on "first time around"; the party was overpowered but not very smart: the CR 23 he threw at them was a lich and they didn't destroy the phylactery despite knowing exactly where it was and what to do with it, so the lich was able to come back and take em down (but not out) one by one a few days later.

I gave my party a homebrew "Wand of Fate-Twisting" as a reward for defeating a flesh tree. It allows you to roll a d20 before doing a check or a save, and the result of the roll is the new DC. Hopefully lowering it. It only has 1 charge per day, so I didn't think much of it, especially since it also has the risk of making things way harder. This was before they tried to fish up a Juvenile Kraken that was guarding an artifact, and used the wand to change its contestant strength roll of 24 to 14. When the enemy has 7+ to strength, rolling EVEN A NAT 20 for the wand is likely going to lower the DC. I have yet to come up with a way to nerf this, but I'm considering having someone steal it.

as a DM, i can offer advice to players: if your DM is experienced, be VERY wary of extra-powerful magic items. Those are usually either cursed or important plot items. when i first started running 2e, i used the tables, and rolled treasure before each lair/dungeon, some of the things rolled were OP, but 'hey, it's what the rules say' ... those campaigns got both uncontrollable and kinda boring pretty quick. i learned pretty fast to limit some of the more excessive things, but as a 'fix', i've found that having an out-of-game chat with the player(s) who have the OP items, to explain that the items are breaking game balance, and you'd like to work something into the story to replace the items with more suitable ones, then adding some variation of: [original owner returns/local noble arrives] to request the item in exchange for [item/gold/favour]

Yes. Yes, I have. They accidentally got a tank and a towed artillery piece. I gave them each a blank army requisition form, thinking they'd use it to pick a nice magical item that'd go well with their character. They rock up to the army depo and since we're playing a steam punk setting, I start describing the artillery and tanks standing out in the yard. Yea, you know where this is going... So my fix for that was traffic cops since the whole campaign was mostly focused around one city. Well my players decided to run for office in their downtime (which fit very well with the theme of the game so I supported that) and after a very hilarious side quest, their first act in office was to amend the city's traffic laws to allow elected officials to drive tanks for "personal protection". I didn't have it in me to say no to that. So now my players drive shopping in a tank

Intentionally, I home brewed a boss and every time I use copies of the player’s character sheets and ran a battle by myself to make sure it’s fair, I realized this boss would destroy the party, but I didn’t want to nerf it because I wanted the players to see its power. That’s where I gave them op magic items, Barbarian basically gets Vergil’s sword, Fighter gets boomerang Buster sword made of a gem dragon’s body, warlock gets an ice spear that can make spiky walls and pillars of ice, Ranger gets a flaming bow with doubled range, and the blood hunter receives a scythe rifle combo infused with a holy flame. They annihilated the dungeon, and then the warlock, who was a fiend warlock, casted fireball, hit the boss and killed the fighter, the barbarian tried to jump off a building to surprise the boss and lost half his health, later dying to a lightning bolt spell, the ranger decided to try and Hotwire a car to ram down the boss and died because the boss wasn’t distracted enough, the Blood Hunter was actually intelligent but died because the warlock tripped him trying to escape! Then the warlock didn’t even get away because he ran down an alleyway and wanted to climb a fence! I gave the party powerful magic items so they could stand a chance against the boss and they still died due to their sheer stupidity!

Are table rules that if you get a Nat 20, you can roll again and if you get another Nat 20 then you instant kill it! But it works both ways. So if the DM rolls two Nat 20 you hit 0 HP. Sprinkle failed the save and died.

So, for magic items specifically, there's a monster in older editions with an ability meant to nerf whatever OP item you gave out. It's called the disenchanter. It literally nullifies a magical property of the item, forever.

My DM accidentally gave for a very small amount of gold a ball that could 1 of a hundred things listed all you had to do was roll a d100. So they were either really good or really bad. Me and the Bard bought the entire stock of 200. 100 for me 100 for them. We enter combat I toss two and the goblin we were fighting turns into a black whole you can just throw out and let it consume a small room. Dm goes oh crap…you were only supposed to have one of those. So we enter another town find a questionable shop keep who goes to me and the Bard I’ll buy how ever many we had left at this point for really large sum of cash. I go sure but I’m keeping one not to use but because I now have a black whole and I think this will look nice with it. We went from being poor to being able to buy almost anything with a bouncy ball.

DM realised the magic staff our sorcerer picked up was super OP. So the condition was, if you try to use the staff to cast higher than your level, you risk breaking it. The higher the cast the higher the risk.

So yesterday I hosted my first ever game and I had a secret condition. Where if they had rolled a nat 20 on this hidden item (bag of holding) it would come with a legendary harpoon and low and behold he got a nat 20…can’t wait to see where this goes.

Not a DM, but the player that was given the powerful item. Was playing a barbarian fighter tank combo. DM gave me an item that would double my Strength modifier on attack rolls. Needless to say, I always hit, and even with a -5 for great weapon master still was netting +4 on my attack rolls. Also had Hill Giants strength as well. It was stolen from me, but knowing how strong it was, I didn't bother to get it back.

My, cute innocent 15 year old satyr bard, got a decanther of endless water, that i got ALOT of good and funn use out of, then i got the chance to uppgrade obe of my magical items. Thats what i chose to uppgrade, i could use it as a laser sword once per loong rest. I tried it out on 1 of 2 red wizzards on the first turn of combat, on a airship we where traveling on, and ended him in that one turn. The other red wizzard made a Hole in the airship and jumped down to -> try

I wouldn't say overpowered but they used it in a way I was not (but really should have) expected. I gave them a Bag of Tricks and I gave them that because the Dungeon Dudes recommended it as a good low level item (and it is). But here's where things got messy. I didn't exactly account for it in my encounters and the only time I've seen it used is one animal at a time. Well the fighter took the bag and used action surge tossing out what became a Giant Badger and a regular Badger. I also gave another player a robe of useful items and he threw out the patch with two Mastiffs. So what was a moderate encounter turned into a massacre due to action economy. Again it wasn't overpowered per se but it did result in some combat bloating. No fix needed in fact I was kinda proud they're all newer players and that was the first time they planned and coordinated an ambush like that. Plus we will now always remember the fighter and his now dubbed "Badger Legion".

if you give out equipment that can level up by itself, thats gonna be exploited without mercy. cap the levels or abilities of conscious weapons, servant constructs, animal familiars, and overpowered unique mage staffs. or limit their usage and durability. or make it so that the overpowered equipment starts being rebellious.

my current party are way OP because i gave them OP homebrew items, thankfully they are dumb when it comes to combat and all i need to do is throw more than one thing at them and the fights become fair again, sure i mess with the HP of some things but i've always ran combat on the 'they die when they need to' rules, ofc named mobs and bosses are going to get 1-2 rounds to do their thing, otherwise its 'surround and pound' every time which is boring

So I made a powerful item.. but it was cursed. The item was a dagger that delt triple the damage that it delt but also damaged the user. Granted this was after the change when the dagger did 6× the damage it delt to the user as the player got a crit on an attack and delt 54 damage to themselves which would have insta killed their character if not for the fact I was doing a homebrew death thing. To be fair. The person was trying to kill any all sus npcs so I think a bit of a scare was needed. But even I'm not that cruel.

*Cracks Knuckles and Cracks Open a Cold One* My Homebrew Campaign has some doozies. • Toilet Brush of Agony & Despair: A mace like weapon that deals 2d12 bludgeoning damage + 1d8 acid damage on a melee attack or can do a Fling Attack of 2d8 acid in a 30 ft cone requiring Dex saves. Unsheathing it requires hostile creatures to make a DC 17 Wis Save or flee from combat. Downside: It's attunement doesn't end until death, nobody else can use it, and nobody will buy or upgrade it. (If you think this is overexaggerated, run at someone with a wet, dirty toilet brush and see their reaction) • The Edgelord's Edgesword: A blade made up of many other blades for easy proficiency that does 6d4 Piercing or Slashing damage. Downside: Any 1s rolled in the damage are how much damage you take as well (Counter to this is Great Weapon Fighting Style and/or being a raging barbarian for resistance) • Glove of Open Hand Strikes: A glove that forces your hand into an open hand position and only allows you to do Unarmed Strikes with that hand, preventing the use of two handed weapons and/or shields/weapons. Upside: The crit multiplier is x100 and upon critting shouts in an etherial voice: "BITCH SLAP!" • Blade of Painlessness: A dagger that upon being used on a willing creature drops their hit points to 0 and numbs the nerves so they don't feel pain while they die. Downside: It makes an inappropriate sound upon their death. (Put down a dying pet to relieve their pain as they go, but why do we hear a party blower?) • Tablecloth of Counterspell: Upon laying the cloth onto a surface, it produces the spell last used on it once per day. Just wrap it over a spell scroll and whatever material components it may need, then it'll consume it and be able to use it. It can range from an instant Heroes' Feast to a 9th level Fireball centered on itself.

Problem the target is still marked and i cant mark another target vor 7 Days or on the next day it the target dyes. TLDR got a cool magic item and oure figther made it just a logbow for 7 Days

Outside of homebrew nonsense, there really isn't much that's too OP. Maybe a Helm of Brilliance too early on, but you can still tweak your encounters around that. What usually happens is the GM just gets too adversarial. It's fine too be, if that's what the group wants. But the point of the game, usually, is for the players to feel awesome doing awesome stuff.

That said, I DID once give a dragon slayer sword to a party I definitely should not have. After they had slain two entire clans of dragons, I had the council of Wyrms, in its entirety, descend on their home city. The king of dragons ever so politely let them know they were no longer allowed to slay dragons on their world. He demanded the party give him the dragon slayer sword (gave the player who gave it up a nice consolation prize in return) and report directly to HIM about any future complaints with dragons rather than taking action them selves. Yep, that party was banned from killing dragons on their home world. So... they literally went right to their world's spell jammer port and headed off world to fight dragons elsewhere. You gotta do what you gotta do.
