the art is made with objects that are oversized compared to their regular sprite size. so 'custom sprites' is enough to make it. otherwise I'm pretty sure such art is practically impossible to make, unless you want the painful experience of trying to overlap existing objects to imitate the specific geometry of the art you wanna make

​​​@@theofficialwoohoogamersh just 2 separate objects that sounds less complicated, and the way you think it works is less efficient because it adds another rule that only exists BECAUSE of how it was done, the most normal way to do it would be to have 2 separate objects and use the same sprite for both, because, when an object transforms into another object, it's permanement unless a rule is triggered that turns it back.
