This is by far my most requested video for YEARS and I finally pulled it together to do it for you!!!! Today I'm going to talk all about OP babies - otherwise known as occiput posterior or sunny-side up baby's positioning or maybe you've heard of back labor? This is a huge concern for pregnant people and can cause a longer, harder labor - so I'm going to explain what OP is, how to know if your baby is OP, when it matters, how it can affect things, whether or not you should stress and then, I'm back in part 2 showing you different positions you can use to try and help your baby turn to OA or occiput anterior or back of the head towards the front of you- position, which can speed up labor and make it less painful for you! My daughter came out sunny side up - and I had a natural birth with no pain meds or epidural- so just fyi it’s possible!! Don’t freak out, your body is MADE FOR THIS! It can handle it!

​@@kamyepgatekeepa9732 lol then why they baby isn't made in a proffered position? Sunny side up position cause a longer & slower delivery along side a severe labof back pain. Your body aren't made to experince that. Anyway, congeats for your safe delivery.

My daughter was OP until 7 cm. My amazing nurse let me know and just had me do a few movements on the birth ball, which helped her move to OA 🙌

Hey Sarah, you're so awesome. I was watching your videos during labour and it felt like you were there with me. You were literally my labour doula. I laboured in the house for 10 hours and arrived in the hospital when I had dilated to 8cm Thank you so much. Anyone reading this just know that Sarah is the best!!

I’ve just watched this whole video whilst in extreme back pain waiting for the positions to help move baby to realise they aren’t even going to be in this video 😰 I could actually cry!!!

My first baby was born OP. We found out when I got to the hospital at 7cm. I was in labor for only 9 hours total. I was able to have an unmedicated birth as I hoped for! You can totally birth a baby even if they are sunny side up. Having two great support people to do counter pressure helped so much!

​@@LindamorenaI think she means that your nurse or doula use their hands to slow down the coming out of the baby by pressing slightly against its head and your vagina. My nurse/doula, which I met at the moment of birthing 😂 was great, she talked to me, guide me and actually helped me by do8ng this support. I did not tear and my baby was 4 kg and 36 cm head. She save my butt 😂😂😂😂

I birthed a sunny side up baby with my first. The pushing part was actually EASY compared to the back labor. I was actually only in active labor about 9 hours but it was very very painful. Trying to avoid that this time around!

This makes me feel better. I’m 41 weeks with an op baby and so nervous about labor. I’m usually pretty good with contractions but pushing is SO hard!

Last week I was searching for this type of video!!! Found out on Tuesday (exactly 38 weeks) my baby’s head is “very low” and I will likely have “pretty intense back labor” (quoting my dr) she told me positions for coping but I wanted more info!! THANK YOU!!!!

Yes I had a sunny side up delivery with baby #4 baby with my VBac it was in the back. I had never done a natural birth before, but he was born healthy. I had no idea what was happening. Thank you for educating us.- Becca

I put to bed on the 3rd of January 2nd time mom..i really had back labour(after deliverythe doctor said it was because the baby was in this posteriorposition )and yes it made my pushing longer.but your breathing techniques really helped me..i laboured at home and got to the hospital at 10cm Thanks for all you do..Good luck to all awaiting mommas..you gat this👍

I desperately needed to hear this! My first baby was OP, 4th degree episiotomy (with tearing) and vacuum. Second baby was a breeze but I have been worried about baby #3!

Yay, thanks for this! I wish this was made 6mo ago! I had a 9.1lb stargazer after 28hrs of labor and 4hrs of pushing. I had no clue until we started pushing. We had the name Orion picked out ahead of time so it was very fitting! It was actually an amazing labor experience and so empowering! The midwife had me moving all over the place as best I could, he continues to be a stubborn dude 😒 I watched all your videos before hand and felt so confident going in! Mental state was a big part of the labor. I kept saying Flex and Flow 🥰

Thank you sooo much!!! I'm 38 weeks now. Baby is OP. And my Gynae said:" this baby better turns or else you'll struggle." Which makes me feel I cant do it. But thank you! I wont stress about it and when labor starts Ill do all the moves to make sure Baby can move! Thank you!!!!!

You can do it, all three of my babies were OP . Concentrate on your breathing ! My labor was fast 5 hrs, 4 hrs. and 1 hr. All three were perfectly fine after delivery. 👼👼👼 three perfect little angels. Vickie 🙋🏼‍♀️

Just had a vaginal birth baby with MAJOR back labor last Sunday. I begged for the vacuum when she was crowning but they said no, had 2 small tears, but hey, I survived! Don't fear, guys. You're going to be okay. Just be ready to say "yes" to the epidural and get a strong birth partner to push on your back during contractions.

I had an OP baby that spun into transverse face presentation. Her heart rate dropped & we ended up having an unplanned/urgent c-section. I will add this video to my Watch Later list. I'm not quite ready emotionally yet. But this is a great topic. Thanks for sharing ♡

Are you me?! I gave birth last November and this was our scenario. It was so traumatic. I hope eventually you can reflect on the experience without pain. I'm working on that myself.

All 3 of mine were OP. My 1st was 27 hrs 2,5 hrs of pushing, using a vacuum. 2nd baby was diagnosed with Macrosomia and I had a emergacy c-section. (34 weeks) the next morning,. So I'm in the OR and my doc says " Ok, I 'm going to need to expand your incision a bit bc he is sunny side up." AND HE WAS HUGE! HIS SHOULDERS MEASURED 12 INCHES. HE WAS ONLY 9 LBS, I was cut from hip to hip. My 3rd was induced at 35 weeks also "sunny side up" and he was a v-back! So there is hope!

when I think about all the position changes the baby has to go through to maneuver out it seems like another reason why water birth is so great since the buoyancy in the water makes it easier for these last minute rotations.....I'm not too worried if my baby isn't in ideal position until the last minute....having a water birth which makes me feel more confident the baby will be able to easily make the turns they need to make to maneuver out... It's also nice how you are countering a lot of fearmongering about the baby's position in final weeks of pregnancy.....

I'd be interested to see a video on how labor and delivery affects the lumbar spine - how it moves, possible effects on the discs. I have an L5-S1 fusion and can't find much information other than "it should be fine!" I'd really like to not herniate an adjacent level.

My second was an OP baby born via VBAC. Took two freaking hours of pushing to get him out but he was also 9lbs 7oz and had his arms crossed over his chest and stayed OP though delivery😅 He's continued that trend for the last 2.5 years of doing things his way and being ~difficult~.

My first one was OP, long latent phase and 2hours of pushing with multiple position changes and doing double pushes but everything was fine at the end. OP is in the range of the NORM, do what you can but don’t obsess too much with it. 38 weeks with my second one at the moment and it’s OP again, just getting ready for whatever is going to happen 🙏

@@naimunnahar8330 Hi! My second labour was much quicker, thank you! Slow start, 14 hours of on and off contractions, when I thought I was around 5cm at the hospital they said I was only 3cm. But I kept walking back and forth and eventually got in a pool because contractions got very intense. I think my baby turned when I was in water because all of a sudden my contactions were every 3 minutes, I just about managed to get to my bed to start pushing. 30 minutes and baby was out. Fingers crossed for you, hope everything goes well 🤞🏻

Im getting contractions that are irregular in strength and frequency as well but last night they were about 5-7mins apart though same in strength/intensity so now im confused whether they are real contractions or not. I hope everything goes well for me too :) 38w5d today.

@@naimunnahar8330 If your contraction starts mildly and then grows in intensity, peaks and then subsides then that’s a real contraction. You can contract for days before you get into an active labour stage (5cm and more), hope this helps, good luck 🙏

My ob didn't tell me that my baby was sunny side up. So I had non medicated back labor for 22 hours, 6 of which I was trying to push this baby out. Unfortunately, my temperature started rising and I ended up having a c-section. But after such a painful experience I was so happy to get it done...

I had both of my boys in my early to mid 20’s, I had back labor for both of them but wow I didn’t know that was OP and how it might make it a little harder for babies to come out and on mommy. Fortunately, maybe the nurses got them to turn appropriately because I had smooth deliveries and pushed them both out in under 10 minutes. I’ve never dealt with front labor contractions, I think I’ll take back labor contractions, to me that’s just a pain I’m more familiar with when you get back pain. Lol Great educational video. Pregnant with my 3rd and they said the placenta is posterior. Does that mean anything?

Both my babies were OP when born, the back labor was insane, but ladies can 100% have vaginal births with OP babies. I have done it. Made it to 9 cm before I got an epidural with my first born, & no pain medication at all with my 2nd one. Again; the back labor is truly insane for sunny side up babies. But women should always be encouraged and never told that they can't have a vaginal birth just because the baby isn't facing down. Had 1 nurse tell me that women cannot have all natural deliveries by choice. I felt so empowered when my 2nd baby was born all-natural, sunny side-up, in a hospital: 18 hours start to finish. I love this ladies videos! She has the attitude more nurses need.

I had AWFUL back labor with my first 10 years ago. No epidural or pain meds, and as I'm preparing for this baby to come im researching back labor and find this. Ask my husband and find out my son came out sunny side up and I never even knew

If baby is in the optimal position (OA) at the ultrasound to determine their position, are there things you can do to ensure they stay in that position? Or once they get in that position do they typically stay in it for delivery?

Omgosh!!!!!! I love you for these videos!!! I have delivered 2 boys sunny side up with HORRID bad labor, and my daughter (she was between the 2 boys) was breech so I ended up with a csection. I’m 23 weeks pregnant with my 3rd boy and want to avoid OP again because I want my natural labor for ONCE 🤣 the pain in my back has been the only reason I’ve needed an epidural. A doctor once told me it could be my pelvic alignment. Is that true?

Polyhydramnious- I’ve had polyhydramnious in my last 2 births and been induced as a result as they predicted a “big baby” both births babies were not big (8lbs) and were health. It’s it essential to be induced with polyhydramnious?

I'd go ahead and start by watching her How to Engage the Baby video and also Bridget Tayler has some videos on different birth ball positions currently. Personally, I moved my hips side to side as well as in a circle and one of those did the trick. I hope that helps and congratulations! So excited for you to meet your LO soon!

My baby wasn’t OP before labor but turned OP during labor. Luckily he was born without intervention but it was definitely really painful and a much longer pushing process. But I’m pregnant with my second now. Does having one OP baby make you more likely to have another baby that’s OP?

Is it true that utilizing positions known to help spin babies can actually turn baby from OA to OP? Ive heard senior nurses say things like latent labor peanut ball use can in an epiduralized patient can turn baby into an asynclitic position

My second born was an OP (also VBAC). I ended up needing a episiotomy and vacuum delivery. My tailbone was super sore for a loooong time. My third (just 12 days old) was born OA but my tailbone is still very tender this time round. I think tailbone may have been fractured or even broken from baby #2 and birthing baby #3 has possibly upset the tailbone again 😩 all worth it but goodness I’d like to sit like a normal person 😂❤️

I had back labor and of course I want to try to prevent it this time around. Would you say that getting an epidural would help? I didn't get one with the first one. I didn't feel like my "hard pushing" was ever productive. The doctor did an episiotomy and used a suction. I am glad I didn't have a c-seciton. But I wonder if a epidural or not pushing on my back would have helped. I love your positive attitude. And yes, my doctor told me this week that my baby is OP and of course I became very concerned, and still fretting labor/birth more than I was....

If you have scoliosis, will your baby have more trouble moving down and out? Can you still get an epidural?

I honestly wish i had seen this 2 years ago when expecting my son 😭😭😭😭😭 Was in labor for 23hrs, the pain in my tail bone was brutal 😫😫😫😫😫😫 To make things worse i couldn't pee, so my bladder was probably obstructing the way 😭😭😭😭😭😭 Had told them at 3pm i couldn't pee, they only drained my pee at midnight after i told my husband they were trying to kill me 😂😂😂😂😂😂😂 (I know! In my defense I'm in Africa ok, a girl's gotta do what a girl's gotta do 😉). Long story short, baby came 4hrs later. I'm currently 37weeks still with a posterior baby 😭😭😭😭😭 i had no idea this was the cause of my loooooong labor last time so I'm grateful i saw this video. Found out last week that this girl is also breech 🤦🏾‍♀️ I have alot of work cut out for me now, thank you sooo much for sharing Sarah ❤&🕯

Does it mean anything that I get painful Braxton hick contrations? There are some that are rock hard and my back od painful and bladder feels like its ripping until I crawl to bathroom to relieve the presuure.

Wish my nurses from 40 years ago knew these things. All four of my 9+ lb babies were op from start to finish. Of course in those days we delivered on our backs which I believe is the worst possible position.

My second baby my first son was a sunny side up op baby the dr didn't tell me until after he was born. I had absolutely no tearing but a whole lot of back pain. They literally had to replace my epidural when I was fully dilated minutes away from pushing. He was my only baby I went into labor on my own, my other two I had to be induced.

Question: Is it true that the location of the placenta can increase the likelihood of an OP presentation? If the placental site is anterior how much more likely is an OP baby.

I had my baby last July and t in for an induction but ended up going into labor naturally and I was so scared to experience back, which I did. I had contractions every 30 to a minute and I also had double country when I would get them. Didn’t have a super long labor though, was only in labor for 11 hours and pushed for about 5 minutes. This pain was intense it’s was hard to breathe thank god for the epidural! Got it about an hour before I pushed.

I gave birth to one that was OP and at an angle (towards my right hip). He had other complications also, but it is definitely possible!! (Granted I was VERY glad my OB had small hands 🤣)

Oh my goshhhh mine is at an angle too!!! Towards my left hip and i kept asking if it was possible to have a vaginal birth if she doesn’t move by the time of birth 😭

@@ivanaperez92 From what I understand they will almost always move either before you go into or during labor. Mine was particularly stubborn and having heart decels also and the OB ended up literally reaching in and manually turning him and half pulling him out because it was either that or an emergency c-section (which I'd already signed paperwork for just in case). But that is a REALLY UNCOMMON thing!

My friend is 3 weeks ahead of me in pregnancy and was hospitalised because of strange pains. Her baby is OP and the doc in hospital wanted her to have a c-section sheduled in some weeks. She was 34 weeks at that point. Her firstborn was OP when she was born. Still baffels me, the baby has a lot of time to turn.

Both my boys were born in posterior position. The first one was the hardest one because I had no idea what a posterior baby was and I decided not to have any anesthesia during my delivery. It was 23 hours of labor and my mom in law later told me that she was afraid that they were gonna lose me because I got unconscious and my eyes were rolling back. The second delivery was way more pleasant because I opted for anesthesia and when my son started to push I was on my knees and the gravity really helped. It was way better and not so traumatic. And I learned that once you have 2 posterior baby the chances of having another pregnancy with a posterior baby is huge 😢

My 9.4 pounder was OP, and kicked on the way out! It’s been over a year, and I still can’t move correctly or sleep without pain, even with physical therapy 😂

2:37 I am 16 wks pregnant and have been having pain on the back of my pelvis already. 🤦🏻‍♀️ feels like tailbone broken & dr noted this during hsg, and open. These videos are my birthing classes

My son is op. I'm 37w2d, cervix is very ripened and seems like if baby wasn't in this position, I'd be in labor. I've been doing the mile circuit(starting at 36 weeks I did every other day), now my midwife said to do it daily and some spinning babies moves. He has moved a bit(he was so far right), but I am so exhausted. My body feels like it wants to kick start labor, but gets stuck.

Thank you Sarah! All of my labor was in my back, and it was definitely uncomfortable 😣. I'm excited to see what to do if the next baby is op in labor. Two questions: 1) Does the baby get into the pelvis before labor or is that all during labor? 2) Is all back labor due to an OP baby, or are there other factors making labor felt all in the back? (I never felt labor in my ab/stomach/uterus area)

There can be other reasons to feel pain in the back as well. Some people just feel it there. So it doesn’t for sure mean that the baby is OP.

Omg I just had some very painful contractions in my back and woke up out my sleep 2:47 am and now it’s 3:05 am now I am here I really hope he is coming !

Can you do a video on shoulder dystocia? I had it with my son last summer (due date induction, week long NICU stay and lots of other drama) and I’m trying to make sense of what I can do to avoid it next time but not getting explanations that make sense to me.

I asked this question so long ago when I was pregnant with my second. I already had her lol. My first was op and my labor was LONG. my second was ear up the whole time and eventually came out face up like the first because my body knew how to deliver a baby that way easier I guess🤷🏼‍♀️ the labor was a lot shorter tho so I didn’t mind

My last labor was so traumatic! My daughter came out facing my thigh. I was in labor over 28 hours! I was induced. My labor kept stalking out. They started and stopped pitocin, put in Foley bulbs, changed positions, among other things.

Can congenital lumbar lordosis negatively impact a woman’s ability to give birth without intervention? Does it interfere with baby’s ability to enter and exit the pelvis?

I’m 36 weeks and 2 days. 4 days ago at my appointment they said he is really low and rear facing and they couldn’t get his face in the ultrasound because of it. I’m dilated 2 centimeters also. This is my first child. Do you know an idea of what I can expect? Do you think I’ll go full term still? I’ve been having lots of discomfort and lower back pain.

Im in the same boat !! Altho my progression started at 37 weeks and im now 38 weeks 50% thinned and 2cm dilated....my best advice is its a waiting game. Just do whats best for u until signs of active labor show up and then time contractions. So rest lots, take baths, eat well, hydrate lots, stay in positive spaces and avoid stress and lots of yoga ball bouncing and circles ! All the best mama! We got this 💛🌻

Hmmm, I have an anterior placenta and I wonder if OP would be easier for the baby.. OA could be a little messy with the twisting and length of the umbilical cord... 🤔🤔

My first baby was OP. My water broke at 37 weeks and I wasn’t close to labor either. Back Labor was the absolute worst after I was induced. I ended up having her vaginally, but had a third degree tear all the way UP my vagina 😱 I did only push for 20 minutes though. Thanks, Pilates 🤣 I did end up pushing kind of on my side and my doctor was trying to turn her while I pushed. I have it all on video. It’s intense 🤣 Thankfully my second labor was perfect, all thanks to your videos haha 🥰

18 months ago I delivered my daughter sunny side up. She never turned, they tried everything. I pushed for over 4 hours and the vacuum was tried a few times and failed, ended up using forceps as a last resort. Had a 4th degree tear. I’m due in 3 weeks with a baby that’s measuring 2 weeks ahead and my doctor is suggesting a c section this time just to prevent going through that again. I’m only 4’11”, I don’t know if that matters or not. I really wanted to try a vaginal delivery though if baby boy is in the optimal position this time. Any recommendations?

This is the kind of labor I had!!! SO incredibly painful. But I was able to deliver vaginally without pain meds, thank the Lord. But 3.5 days of back labor and an hour of pushing with an OP baby… not fun!!

hi, I am having pains in my lower vagina and inner-upper thigh from yesterday and I also felt a tightness in my abdomen area but both the pains are not connected the tightness breaks but the pains in my lower vagina and inner-upper thigh are continuous what do I expect out of this?

I had similiar feelings in thighs and vagina when before I had my first... It started maybe 2 weeks or so before she was born and it was the most intense when walking. For me I think it was a sign baby had dropped and I was getting close.

@@ashb2404 this is me right now at 37 weeks. Getting out of bed is very hard, walking is much worse till I walk around for long minutes and the pain eases off gradually. Scheduled for my last scan in a couple of days but suspecting my baby is OP

I had back labor for 40+ hours.. my body stopped dilating at 9cm and I was told to get a c-section... This time around, I will move more, put myself in different positions! Do sooo much more to get baby to move, IF i have another OP baby.. it was really really awful..

Omg the same thing happened to me!! I wasn’t in labour for quite so long, only 14 hours, I pushed for 2 hours, and even up having a c section. My boy had so much pressure in his head it was cone shaped for the first few days.

Oh gosh same for me! I thought everything was going well. I labored unmedicated to 9cm but after that my back felt like it was being broken over and over with each contraction. After 3 hours I agreed to a c section 😓

So my son was sunny side up, and was taking to long to descend, and he was having dcells so we had an urgent csection. I was told I had way to small of a birth canal and should never deliver naturally.. is this really possible? Your video has me questioning what to do when it comes to my next pregnancy.

I had an OP baby, never turned after all kinds of techniques. Nothing worked, had to do a c section after 29 hours. 7 years later I'm pregnant again and I was hoping this video might show what to do to avoid OP position or what to do if it happens...

They didn't know my baby was sunny side up until she was coming out. It took me 10 hrs to go from 9cm to 10cm. I was in so much pain but no one believed me because it was my first baby. Once she came out the way she did. The nurses apologized to me for not believing me about my pain. They thought my epidural was stalling my labor when it was just the position. I'm glad I didn't know while I was in labor but it felt good to know after that my pain was validated 😂 I had to have an episiotomy due to the fact that she was stuck and not coming through, I have an unusually small pelvis and if she had come any later they told me I would've had to have a c-section.

Exactly same thing happened with me. My labor was traumatic to say the least and nobody believed me when I was saying that pain is becoming unbearable to the point where I started wishing death upon me 😭. My doctor had to use a vacuum to deliver my baby girl.

@@reiloveable same. Ive been telling my midwife that this cannot be normal but she just kept on saying that it is (with a smile on her face). It was my first birth but I knew something wasn´t right. I had no real breaks, constant pain with horrible pain during the contractions, never felt anything on the front, all in the back.

From one Sarah (with an ‘h’ obv) to another!! Can you talk about birthing the placenta? This terrifies me. How intense is the pain in comparison to the actual baby birth? How often does a completely natural placental birth occur? Though I’ll likely have to be induced due to having ICP, I’d prefer to have as unmedicated of a birth as possible. THANK YOU!

In less then 4 minutes it become obvious ‘clear that birthing on your back is the worst position. How can our pelvis open up if where putting pressure laying down, therefore it can’t open as much as it could which is going to result in a higher chance of tearing also!

My sister was delivered OP. Doc even tried to turn her. She just rolled back over lol. My moms labor and delivery was perfect we just got to see her face first 😊
