a CBCT scan will often times be optional at a lot of dental offices. However, I have an example of why you need to make sure you have one, especially before a coronectomy.

From someone who has worked in a periodontist office and in general dentistry for over 18 years I knew before the Dr even mentioned that the previous Dr probably did not have a CBCT. Run don't walk if your periodontist or oral surgeon does not have access to this. This Dr and video is sound and excellent advice. My advice for patients just pay for the CBCT. I know a lot of patients see the $300 plus price tag and think it is not worth it especially if the insurance doesn't cover it. Trust me it IS worth every single penny and it provides your Dr a wealth of information.

I was just recommended a coronectomy and found your video. I think my situation is a little different I have a small mouth, with long roots, super close to the nerve, and my number 32 wisdom tooth roots are crossing with my number 31 molar. I asked a lot of questions and he seemed to really know what he was talking about, he said if he was me he would absolutely do the coronectomy. Hopefully the bone will grow over it or worst case the leftover roots will migrate back to the top away from the nerve and be extracted later.

I got a coronectomy a little less than a week ago (30/F.) Had regular X-rays and a CBCT scan that confirmed my oral surgeons suspicions that my lower wisdom teeth were interacting with (resting/pushing down on) the nerves and dangerously close to the jaw bone. Some deep boi’s! He was worried about nerve damage and jaw fracture if we pulled them. I think we made the best choice for me right now and hoping for a speedy, uneventful recovery! So far no pain, only mild discomfort a couple days.

I had a coronectomy 2 days ago, and I've had no pain, and was super easy with iv sedation. I'm hoping healing will continue to continue well. Back to do my other side in a few weeks.

@@northtexasdentalsurgery Would also like a 3rd opinion. Went for a cleaning and was told that had impacted wisdom teeth /no x-ray ,scan was performed. Went to monarch dental had panoramic x-ray was told that everything is normal .Any feedback would be appreciated!

Had a consultation with an oral surgeon this morning and was told I would need to have a coronectomy, and or if I did a full tooth/ root extraction I could have numbness for 4/6 even 9 mths... He didn't do any kind of 3d scan or any scans at all... Not sure what I'm going to do, but I thank you for this great information😊 11:50am

I just had my top 2 removed today and was told that my roots were too close to my nerves on the left side (i didn’t bother to ask about the right side because it wasn’t recommended for removal but I should have and will when I go back for my follow up in a week). My dentist did mention Coronectomy being a possible option but couldn’t say what procedure would be best until I get a CT scan. She then referred me to a specialist to get it all figured out. In hindsight I remember when I had my left Molar extracted a few years ago the left side of my chin was numb for a few hours…I thought it was the anesthetics but I do remember kind of feeling uneasy about it since I hadn’t experienced that with other extractions on the lower right side but since I never heard of nerve damage risk until now I had no reason to freak out. So from what I gather my then Dentist failed to inform me of how close the tooth was to the nerve and just stormed the hell in there lol. I can find humor in it today but the reality is his disregard could’ve been costly for me, thank God it wasn’t. Also all the times I’ve been to the dentist (since 17 until now) never was I ever told that removing my wisdom teeth early could minimize risk in the future. They all said “do they bother you? If not leave em alone”. They said that about all 4 even though the tops were nowhere near any nerves. Well years later and 2 successfully removed here I am with all of you scared and learning from all those who came before us and those currently in the same place as us as well as all the Dr./Dentist that took the time to give us the info we may not get from an overworked surgeon during office hours. As I lay here with my head uncomfortably elevated with sutures in my mouth craving food I can’t eat with a slight headache, I’m thankful that we are all here getting the valuable information we need to make informed decisions moving forward. I’m praying that we all are blessed with caring surgeons with anointed hands. Lastly if you’ve recovered or not please share your experiences as well as updates negative or positive. We would love to know if you are still in recovery or has recovered. When we post we become apart of a community so please stay the corse because you’re story is extremely valuable to us and those that will join this community days to years later. God bless you all and I will update you guys so check back periodically. For now I have to wait until I get CT scans done. ❤️

Hi Dr. I got three of mine removed, my two bottom ones are impacted. Doctor removed the right one, was 30-40 minute surgery, hurt like hell. He says the left one is laying right on the nerve but isn’t affecting any other tooth near it and I can leave it. I want it out idk he says to have second opinion because he says the process can really damage the nerve. I don’t want to lose sensation on that side but also don’t want it to keep growing. Idk what to do, I’m conflicted, can you help?

So I’m 23 and hadn’t been to the dentist in a while and asked about my wisdom teeth. They weren’t causing any pain but the dentist I met did an X-ray and recommended I get them removed. I was told this was a potential risk of happening from the first oral surgeon I talked to in KS. He showed me a 3D X-ray of my root grown out around my nerve and how that can risk nerve damage happening. I was moving to TX so I had to wait to get my surgery done. Well I finally found an oral surgeon and he showed me the base X-ray and mentioned that my root was pretty close to nerve if not literally touching it. He said he wouldn’t really know what his decision was with it until he got in there and started removing it, saying if it was too close to the nerve, he was going to leave part of my root in. I’m 3 days post op and I have no clue what he decided to do. I just have major facial and neck swelling and the pain is radiating down to my chest. Like my collarbones are very tender and hot and I’m having some chest pain and pressure. Not even sure what’s normal and what’s not. Just needed to share this because I am miserable. The pain killers I have aren’t great and I still can’t sleep for more than 2 hours straight.

@@northtexasdentalsurgery it took a little longer than I anticipated but I’m all healed up now! I’d say it took a whole month for me to be able to open my mouth fully again with exercises and warm compress. I had an infection in my mouth but it cleared up completely after a second round of prescribed antibiotics. All I was left with was knots and tightness in my jaw. (My husband didn’t remember them advising me to massage my jaw after the procedure so it doesn’t lock up, it also wasn’t on the paper they gave me)

@@northtexasdentalsurgery yes, the infection was the reason I was still very swollen on my left side, but the chest pains were unexpected. Ibuprofen was the only helpful medicine, but I was miserable for 2 weeks. I was also prescribed Tramadol as the opioid pain reliever but it did not help. I was sweating bullets on my post op appt. My oral surgeon informed me that if the infection didn’t clear on it’s own or got worse, they would cut it open and drain it and that was my worse nightmare… MORE surgery. Fortunately, I have not had to see him since 😅 Thank you for having this comment section open to share and read others’ experiences!

The case in this video and my unremoved-yet teeth are really similar in nerve-tooth positions. The 3d scan relation/distance seems similar. But extra: I have a narrow/small jaw that even my own teeth can’t fit well... Even though the first doctor/surgeon I saw scared me with ‘close tooth-nerve position, risk of permanent numbness’; other couple of doctors said like ‘despite the fact that they are close and the risk exists for every operation, it can be removed rather than leaving there for worse future scenarios’ So I decided to get them removed in the place/doctor that I got the best/advanced vibe. They said the left bottom is more risky/harder than the right bottom. Operation is in 1 month. To be honest, I’m scared a lot of having permanent nerve damage... Wish me luck! Edit: I updated my process in the reply part below.

I’m here with updates! To sum up; two lower sides are got removed without dumbness/permanent nerve damage. I got removed the most risky left-lower teeth, the surgeon said it was as easiest as they could expect; in one piece, quick, clear. She said sometimes even the tooth is not even a bit close to the nerve, the patient may have nerve damage. And sometimes, even though the tooth and nerve are in touch, nothing may occur. Thank God that mine was quick, easy -for a risky teeth, with a 1 hour operation-. The first week after the operation is the most important! I was so careful but it was still painful in every sense ( a lot of pain, no food, no sleep... ) But after the sutures were gone, everything started being as it was before, day by day... so be patient. After 2 weeks; I got the other side, right-lower wisdom teeth removed. It was risky too but not in touch with nerve, just so close (0.4 mm). But this one was impacted 45*(degree), had a position like hiding under the other teeth’s crown(upper part) and had a weird root shape. The operation was hard, not gonna lie, i felt like i’m gonna faint for a moment; it took 1.5-2 hours; many of the first removal trials were unsuccessful, my surgeon’s professor came and looked and suggested removing it partially in order to not damage the nerve. The surgeon said she took it off in 3 pieces... I saw the tooth afterwards since i wanted to see; the roots were 1 piece, the upper part(crown) was the other 2nd piece, but i don’t know the other piece... couldn’t look in that detail and long. I remember hearing crackling sound during the removals of the pieces. After the operation, numbness in my tongue passed in 2 hours but numbness in my lips passed in 5 hours and I freaked/cried a lot in those 3 hours. Now, I’m on the 4rd day after the operation. The first days were awful since the removal was very traumatic. But I'm getting better every day. She said the last-one-like removals might cause pain after 1 month. So, I should be on the alert for pain, swelling and etc even after 1 month😔 I hope no pieces are left inside and I will recover swiftly, healthily. And I wish the same and even the better and better scenarios for you guys. 🙏

I recommend people also do their own research regarding coronectomy vs full removal. There isn’t literature that proves coronectomies are harmful or have worse outcomes as full removals. It’s is a legitimate treatment option. Particularly when done by a trained hands that leaves immobile roots behind. Looking at the cbct scan I agree the tooth should have been removed fully and the risk of parasethesia was not high. and it is odd the other surgeon never took a scan. But don’t freak out if you’ve had a coronectomy before. Even in this case more than likely her surgery will heal just fine and be uneventful.

Had a coronectomy 3 days ago in Mexico, it still hurts, But its getting better... The CBCT scan was 75.00 and the extraction was 200.00. So at least the pain is contained only in my mouth.. Its getting better tho.

Hi Dr Choi, I've seen three surgeons for consults and although my bottom two teeth are horizontally impacted, all three said a CBCT scan is optional/not needed. I am 25F. I have the option to get a CBCT but do you think I should get one, given the radiation risks and the fact that all three surgeons said that it is not needed? Thanks!

My ortho had a 3D scan and my root tip are in because they are too deep inside the bone and it would of been more jaw damage trying to get it out. I had to get a bone graft on one side because they broke piece of my jaw bone trying to get it out

hi doctor is a coronectomy an option if I don't want the full tooth extracted.. it's not impacted and fully erupted but I've been getting cavities and was advised to just pull. which I don't want.

Awesome case and nice review. I had a case today that I referred to OS. Did a CBCT- needs a coronectomy. Thanks for presenting this case. Did the pain resolve with narcotics/ time or did you have to go back to extract 2nds with the root remnants?? Would love to know what happened to the pt.

My bottom right wisdom tooth are heavily impacted. Growing horizontally, on a nerve, both are actually but the right one has been giving me issues lately. (I’m 29 btw) I saw an OS 3 days ago and he recommended this procedure and though I’ve read it’s a safer alternative to an extraction, I’m still nervous. Could you shed some light on the post op recovery and any risk factors? I work in IT and the thought of loosing sensation in my mouth is nerve wrecking. Thanks

I just had a coronectomy done on Thursday and now am dealing with excruciating pain from an infection in my lower jaw. I wish I would have done more research before I had this treatment done but it's too late for that. I never received a 3D X-Ray and now just have to wait in hopes the antibiotics I am taking/drainage of the infection I had done this morning are enough to treat this infection without having to have surgery again. Any advice???

Hi Dr. Choi, I was just told yesterday by my general dentist that I need to extract all 4 wisdom teeth due to minor infection, I have a consultation on the 22nd with the surgeon. The receptionist told me I should get bone graft on all 4, since I currently have braces and will eventually get veneers and crowns on the 4 front teeth. Each bone graft is 550, I have spent so much money on my teeth it’s ridiculous. My question is, do I really need all 4 bone graft?

Well this makes me nervous. Mine just told me that he might have to do a coronectomy but that is more of a game time decision (meaning he won't know until he's there removing the tooth already) he said to forget about the 3d scan (even though I had gone to another surgeon 3 yrs prior to remove a different tooth and they recommended that) and that it wasn't necessary. Now i am out of my allowed scans for the next 3 yrs according to my insurance ( they only allow either one x-ray or 3d scan every 3 yrs) and i don't know what to so

Lots of conflicting opinions ! Went to a Dentist with out him even doing an xray was told that he could remove the. Impacted wisdom teeth . Went to a different place that has scans they said no need to have an extraction!

I got a 3d scan and the nerve runs high up between the two roots they never even offered a coronectomy and I don’t know what the risk is I’m 35 and wish I knew what to do about it

Hi Dr,very informative video. I don’t know much on dental surgery, I have been trying to read up on on it since I was recommended to have a coronectomy for a horizontally impacted wisdom tooth that is too close to the nerve. One thing that I am unable to lookup and I was hoping you can help me is that the surgeon wants to “pack” the roots with a medication (I don’t recall the name) and then have me return to the office to “repack” in 3 days then 3 days for a packing removal. Have you heard of this? I tried to research on my own but have not been able to find anything. Thanks in advance

i personally dont pack the site with anything do it but doesnt mean that it wouldnt be a valid treatment. because i dont know what the "packing" is its hard for me to comment on it.

Dr. Choi, I am a 62 yr old woman. I have all my teeth so far, I have a slab fractured lower left wisdom tooth, I want my dentist to put a crown on it but she said that 90% of crowned wisdom teeth end up in a few years getting infected and a root canal will need to be done and the tooth will need to be pulled anyways, so I might as well just pull the tooth and save myself some money. I would like to know what your thoughts are on this subject? Crown or no crown?

My wisdom teeth are impacted horizontally and my surgeon is recommending a coronectomy . After reading something online, it said that it’s not recommended for teeth positioned horizontally. Do you agree? Should I get another opinion? Also, they are bothering me. I’m 33 and he says that it’s best to do it now but I’m apprehensive.

I am 32 and did it. He left the roots in. i did have an infection after the surgery thoi as the roots were not removed and i guess they were infected.

My lower bottom right tooth which keeps getting Periconitis. I’m 29 now the tooth grew in only 1 year ago and my surgeon says it’s my choice what to do but it looks quite risky on the CT. I don’t know what to do as am worried it could get worse in future

I had a consultation today with my oral surgeon who advised me I was one of those rare cases where my root or my wisdom tooth is under my nerve and the way my tooth grew it grew around the nerve, so automatically my best option for this impacted lower right wisdom tooth is the coronectomy or do a regular extraction and endure paresthesia. I cried my eyes out and asked if there’s a chance I could endure the root wanting to grow under the nerve then it has toe be extracted. I’m worried because he said there’s a small cavity on the crown of the tooth but doesn’t believe it has connected past the nerve line. This is the tooth that is bothering me the post and I’m just unsure if the cornectomy will go wrong and I have to possibly get nerve repair surgery to get sensation back in my mouth. Any tips help or advice?

Omg 3 days ago I went to a "wisdom specialist" she cut me open and left me like that ,she couldn t take the wisdom tooth out bent roots also like this with the nerve problem but she never told me that , yesterday I was to a surgeon took the tooth out in 5 pieces... Very hard surgery it was horrible...and he told me about all this..... Medical sistem in Romania 🇷🇴 crap.. i stayed with that wound in my mouth 2 days....now I suffer it is so painful and i m so swollen ,cries all night.... Really hurts... can't wait to heal...good luck!!! 🤍🤍

Is it safe to get a coronectomy on both wisdom teeth the same day? Oral surgeon wanted me to remove all 4 but honestly its out of my budget and top ones dont really bother me. Any recommendations?

Any update on how long it took for the pain to go away or if you had any infections? I can't believe how painful this is and I'm pregnant so I'm not taking any pain meds.

I had sound in the ears and after removing my wisdom tooth , the sound still did not stop

Coronected my right side wisdom tooth yesterday and left side a month earlier. My left side is closing the gap and my GP told me it’s doing well. After watching this video, I’m worried about the years to come if the remaining would erupt 😂😂😂

Toridol. Tisk tisk. The worse pain medicine. Tore my stomach into a million pieces and d did nothing for me.

How long does a wisdom teeth site takes to close? i took 3 wisdom teeth out 3 weeks ago and the site isn't close as yet
Nerve Damage?  Cant talk properly after wisdom teeth removal?
