Benign Early Repolarization (BER) can be a tricky diagnosis given the ST segment elevation and similarity to STEMI. In this video we go through a patient's 12 lead electrocardiogram (ECG) and talk about how to identify benign early depolarization, it's diagnostic criteria on the actual 12 lead ECG, and more!

3 doctors were shocked after seeing my st elevations. My h beats climbed to 120 after seeing their body language of stress. But in midst of my last testing tonight after midnight, I googled "st elevation without symptoms" and BER showed up. Fortunately this young forth doctor was smart enough to diagnose BER. Thank God. I imagined death today. I'm 31, male, totally healthy. No smoke no alchohol.

I had a borderline repolarization at first it went away then acouple wks I had really bad chest pain went to the hospital and got a EKG which said i have T wave inversion now evident in inferior leads and and a nonspecific T wav abnormalty noŵ evident in inferior leads. Im following up with my doctor but am still have axiety it means something bad.

i have this syndrome, luckily all the signs that indicate a possible increase in unexplained ideopathic v-fib are not present. i am very active and in my early 30's. in leads v4, v5, and v6 the J notch is clearly the fish hook pattern with the upward sloping t wave (downward sloping is higher risk). in leads 2, 3 and aVF i have slurred R waves on the down slope towards the bottom. I have had this for at least 7 years from the time i took my own EKG, but recently when i started back on Adderall and dropped off two copies of my 12 leads (1 taken on adderall, and one taken not on adderall) which showed no change from my norm. my psychologist is now requiring me to see a cardiologist before resuming my medication prescription refills. every thing i have read about this pretty much says a 12 lead with absent signs and symptoms and no medical concerns does not warrant further testing. sorry about the lengthy comment. i can't find anything saying adderall would be contraindicated with this abnormal ekg finding. and now it will probably take weeks, maybe even longer to get in to see a specialist.

@@chrispeters5194 Typically BER is more diffuse in the precordial leads rather than in a single lead!

I dont get the concavity part. I have looked at 10 STEMIs and 9 of them have the concave pattern. Only STEMIs I can find that are convex are the tombstone ones, as you mentioned, shark fin and lambda sign STEMIs. I guess I should focus on the 5 step check list, and not just focus on concavity.

Hey! We are in the same boat. Same age, male and also got some changes in the inferior leads yet they told me it was a normal pattern and not to worry about it. I also get chest pain every now and then, but I believe it mostly comes from anxiety. I'm not sure if you're going through some stressful moments in your life but that could be the reason also!

Thanks for the video. What does it mean if someone has BER; should he feel chest pain?
@WhiteboardMedicine
@WhiteboardMedicine 3 жыл бұрын
Thanks for checking out the video! Benign Early Repolarization (or BER) is just an electrical phenomenon seen on EKG that is benign and a normal variant. The patient will not have any symptoms related to it. It can look similar and can be mistaken for more severe pathologies like pericarditis or a heart attack. Let us know if that makes sense and/or what additional questions you may have!

@@WhiteboardMedicine 😊 thank you so much. I had massive chest pain at work last Thursday morning and was taken to hospital by ambulance. Part of the diagnosis is BER; I have a stress ECG this coming Wednesday. The pain hasn’t been explained yet. I have also recently been diagnosed with having GORD...it could have been that, however intense exercise seems to be a little problematic these days. Oh, and my gastro thinks I also have EOE. We’ll see; but strange when most of my life I’ve been healthy, apart from the last 2 years I’ve put on 8kg and recently returned to training. Unfortunately most of this year I’ve had extreme insomnia...5-5:30hrs sleep a night; the basis I think is GORD, but it’s all slowly being sorted out...male, 46, 178cm, 82kg...if any of those stats have any influence.
