Are cancer cells still contained in the lymph node if only 5mm of cancer and no extracapsular extension present?

Greetings Dr Tasha. Just wanted to know your thoughts and insights on my sister's metastatic bc condition. My sister will turn 60 on 1st June 2024 and she was diagnosed with stage 4 aggressive her2+ bc in Mar 2021. She responded well to chemo but recently showed symptoms such as stiffness on her right hand and slurring in Jan 2024. Dr did a brain scan and revealed numerous tumors in her brain. We were not aware that her chemo didnt target the brain..also dr didn't perform any brain scans unless she showed symptoms (which wud hv been too late.) urgently..doc took action to did 5 sessions of whole brain radiation in a week..as well as radiation to her breast (as she stopped chemo for 2 mths since Jan). Both oncologist and radiologist recommeded hospice care...predicted she had less than 3 mths to live. She is now bed bound since then but is showing signs of improvement such as her alertness to her surroundings..her speech, appetite very good and physical outlook such as eyes,skin,teeth are healthy (even the dr was suprised) except she cant sit by herslf...at this time we focus on giving her nutritional supplements and healthy food in liquid form. Do you think its possible for her to build her strength first before giving her a new chemo ie. trastuzumab deruxtecan (that can enter the brain) which prob take ~1yr max. Really appreciate your reply esp from experts like yourself and others. Tqsm❤❤❤

Hello. I am really sorry to hear about your sister. I am afraid I can’t comment on individual cases. What I can say though is that chemotherapy is very challenging to the body. And patients need to be in the best possible condition to be able to tolerate it. The oncologist will be able to assess this. I hope your sister continues to stay healthy and well 🙏🏽

I would like to share my wifes experience. Not advice just information for your consideration. My wife has/had stage 4 HER2+ met breast cancer (breast/lungs/bones/lymph/liver/brain). While various chemo's did hold the line for a while they did not stop the advance. Her oncologist put her on an immune checkpoint inhibitor (enhertu - herceptin/chemo) which attempts to stimulate the immune system to attack cancer - which if it works it is usually very effective and long lasting. However this ICI (and most ICI's) only shows a full resolution rate in the low single digits when given standalone. Per our research (PubMed/NIH) over months we uncovered various adjuncts that act synergistically with ICI's that can greatly increase the odds of immune involvement with the cancer. In my wifes case, after combining a fast mimicking diet (Dr Longo), a plant based diet, attention to gut health/bacteria (fermented foods) and very high dose melatonin (much science around all these adjuncts and their synergy) with the immune checkpoint inhibitor she has had a full regression of the cancer for the first time in 10 years per CT/PET and now two negative Signatera genetic tests which has a high level of sensitivity to any remaining cancer activity/circulating cells. Her oncologist is floored by the results as are we. We have come to believe it is only the immune system that has the sophistication to fully reverse advanced cancer and much evidence points to this way of thinking. We have also come to believe that a much more core issue related to cancer is not the cancer cell itself but the failure of the immune system/gut bacteria which are THE systems that normally keep errant cancer cells in check in a "healthy" person. It is when the immune system/gut bacteria become dysregulated that cancer can progress and eventually get the upper hand. Having cancer is not much different than having AIDS which is a failure of the immune system and not surprisingly most who die as a consequence of AIDS die of cancer. It may be that the focus of science over the last 60+ years on cancer cells and toxic agents to kill them may have been the wrong focus. Worse still, most of these conventional drugs further impair the immune system/gut bacteria and pull the patient further from a full recovery long term. Regards, Mike

I am TNBC stage 2 cancer survivor who completed my neo adjuvant chemo in july and had lumpectomy and achieved PCR and had 18 rounds of radiation. What is the possibility of my reoccurance

It is difficult to predict recurrence. However, having PCR is very encouraging.

I have a stage 4 her2 tripple positive breast cancer and it is metastatic spread to lungs liver and bones and currently in my 4th cycle chemotherapy we ask my doctor on her plans after the 6th cycles of chemo and she said I do not need to go on surgery because stage 4 is incurable does her plans ok? my prognosis is good the tumor size in my breast have shrunk and continue shrinking can you explain why her plan for me is not to go for surgery? more power on your channel and thank u in advance...

Hello. I am afraid I can’t comment on individual cases. However, what I can say is stage 4 cancer is mainly treated with chemotherapy. Sometimes, surgery is performed for isolated spread (metastasis), but this is assessed individually. I hope this is helpful and I wish you continued good health 🙏🏽

Cancer do not spread to distant organs via the lymphatic or blood vessels. Those cells are completely different from the original cancer. We have cells that are damaged from toxins exposure roaming in our body. Because of changes in the cell membrane, these cell are outliving their lifespan and do not die. Even thought these cells are the cells that form the tumor, they are not called cancer cells when they are still roaming. So even when you have a tumor, you still have other damaged cells that have not joined that tumor. So if you get another tissue damage from oxidative stress, then the damaged wound healing cells that are still raoming around in your blood will go to the new damaged tissue site and attempt to heal the tissue. This is why cancer cells take up glutamine. Because they are trying to heal the body. However, if inflammation is too much, or toxins is at the site, then the healing process becomes uncontrollable. The cells multiply because of inflammation and cannot die as long as there is inflammation. The inflammation have to stop long enough for the healing process to complete. Also the immune system is suppress in cancer, at least the part that deals with these pesty cells. To stop cancer from spreading, control inflammation, oxidative stress, and stress level.

@@meahdahlgren6537 this is about mechanisms. People are not different. Cancer do not spread the ways they talk about. You have cancerous cells in your body looking for a wound to heal which can turn into a tumor if the healing process is not done properly.

@@meahdahlgren6537 a cancer cell do not break off from a tumor to form another tumor. This leaves open too many questions like why that new location, what’s wrong with your lymphatic system because it supposed to be filtering out toxins and bad cells. Cancer is a wound that’s not healing properly because of inflammation like a keloid. Every cancer is different because of location but the mechanisms are the same.
