The context according to the commissioner; This is from their "My Little Diseases" AU, where Twilight went mad from suddenly becoming an alicorn. She turned to science to keep herself busy, and she soon started making her own diseases and testing them on her friends - trying to make a cure before it killed them. Unfortunately, Twilight was unable to find a cure for this flesh-eating virus (inspired by the film "cabin fever") that she tested on Applejack, and chose to lock her friend away instead, leaving her to the mercy of the disease she had so meticulously crafted.
