Hello! This was flagged as spam so we're just seeing it now! Sorry! Yes- we have already filmed a couple of abnormal gas videos and how to fix them. We were all getting a little bored with the gas videos though- so we wanted to shift topics for a bit. We'll definitely get back to them though! Thank you for commenting :)

What formula do you use in determining the primary disorder in cases like mixed respiratory and metabolic acidosis/alkalosis? Thanks! 🥰

Hi Dr. Licup! So glad you're still here! The most important thing to realize is that the body will not over-compensate for an issue. So the first thing you look at is the pH. Is it acidotic or alkalotic? If it is acidotic - then the primary issue must be one causing acidosis- whether that be respiratory or metabolic. Does that make sense? Thanks that was super helpful and helped me understand compensation more. If the PH was in a normal range but the C02 was high and the bicarb was high, that there is compensated resp acidosis? Lets say the the PH was in a normal range and the C02 was high but the bicarb was low - is that compensated mixed? I'd love to see examples like that.

Hi Vickie! So glad you're still here :) So you look at the pH- and if it's low the primary disease is an acidosis. If the pH is high the primary disease is the alkalosis. In the case you described- the pH was normal and the infant had both respiratory acidosis and a high bicarb you'd have to go with the clinical picture. Is this a kid with long standing BPD or is it a kid with good lungs who's been on lasix for a long time? For your second exam[ple- if the C02 was high (causing a low pH) and the bicarb was low (also causing a low pH) then this would indeed be a mixed acidosis- but we would expect a low pH. We are planning to run a few videos on examples like this. (We just wanted a short break from gases! ha!) Thanks again!
