I'm guessing mayhem points are given the more Pico shoots stuff around the house? I never saw a walkthrough where anybody got the maximum points.

Yeah, probably because you have a limited amount of bullets most people don't shoot the bodies of the kids or the windows during the Bear's first phase

Basically during the first fight you have to break all the windows while making sure the bear doesn't hits you, also if you take too long the bear will blow up your bushes so no more cover... And if you wasted too many bullets then you won't make it to the second fight
