Dr. Campbell, you are truly a blessing to medical education. I am a retired physician who is thinking about going into teaching just to pass time. I came across your videos and all I can say is that your videos are superior to anything I have come across in books or online. I studied physiology in medical school in the late 1980s out of Guyton. We didn't have anything else back then. Guyton is a great textbook even today, but your videos add depth and breadth to what Guyton teaches. Plus, your use of language is efficient. I had a British professor in college and he was also like you with seemingly no wasted words or inept phrasing - nothing extraneous or imprecise. Like you, he had a wonderful command of the English language. I think it comes from a culture that respects and appreciates the beauty and nuance of language.

Great video! Just one question, will the transcapillary refill affect the intracellular volume of the cell? Thanks

Yes, it will. As water moves from the tissue space into the blood, water will also leave the intracellular compartment and enter the tissue fluid. This means the intracellular fluid has the capacity to act as a further fluid reserve. Therefore if the person does not have access to fluids, the fluid levels in the tissue spaces and in the cells will be depleted, but the volume of the blood can be maintained for a time. However, giving IV or even oral fluids will mean there is no need for this 'reserve' process.

Dr. John Campbell Thanks for answering but I thought you needed an osmotic gradient between the ICF and interstium for fluid to move? If you are losing isotonic blood, surely no concentration gradient is created and so no movement occurs?
