Educational video describing the Boutonniere deformity of the fingers. Also describing conditions of Swan Neck and Mallet finger deformities.
Deformity is characterize by PIP flexion and DIP extension. Injury to the extensor tendon or central slip usually prevents the finger from active extension.
Anatomy
What are the differences between Boutonneire and other deformities of the finger?
Mallet finger
Rupture or avulsion of the terminal extensor tendon.
Treatment:
•Immobilization of the DIP joint in extension. Keep the PIP joint free.
•Surgery by pinning if subluxation of the joint or avulsion fragment is more than 50%.
Swan neck
Deformity and casues
•Mallet (DIP)
•Volar plate laxity or injury (PIP)
•Subluxation (MP)
Boutonniere
Extenrsor tendon injury. Three components:
1-Central slip rupture
2-Traingular ligament attenuation
3-Lateral band volar migration
Stages to Boutonniere:
1-Injury to the central slip will lead to lack of extension of the PIP
2-Triangular ligament, lateral band separation and volar migration will casue flexion force on the PIP and extension force on the DIP
3-Retinacular ligaments contracture
4-PIP and DIP capsular contracture
Clinical examination
•The extensor tendon of the finger splits into the lateral bands.
•The lateral band then come together and insert into the base of the distal phalanx.
•The central slip insets into the base of the middle phalanx. If the central slip becomes ruptured, the lateral bands will slip down to the volar position.
Elson’s test
•Used to determine possible tear of the central slip before the deformity is evident.
•Patient is asked to curl the affected finger around the edge of a table (the PIP is bent 90 degrees over the edge of the table).
•The examiner then places their finger over the middle phalanx.
•The patient is then asked to extend the finger.
•If the central slip is intact, the examiner will be able to feel the tension of the finger being extended.
•With a ruptured central slip, the examiner will not feel tension, as the patient will be unable to extend the PIP joint.
Treatment
Acute Boutonneire
•Static splint of PIP for 6 weeks
•Used for acute injuries less than 4 weeks
•DIP and MP joints should remain free.
Acute open repair of boutonniere: open injury requires surgical repair
Chronic boutonniere: more than 2 months after injury
Reconstruction of the extensor mechanism. Use splint before surgical release. Full passive range of motion of the PIP and DIP is needed before surgery.
Bad prognosis if:
•Patient is more than 45 years of age
•Associated fracture
•Fixed PIP contracture
•Prior surgery

I am going to take an exam for occupational therapy at Keiser University. I've been having Boutonniere Deformity on my right hand little finger for more than a decade... However, it's not too severe: there is no arthritis, and I can make a complete fist. I consulted a very experienced hand surgeon yesterday and I'd like to share his input on this: 1. Larry Bird had this problem on the same finger long time ago, and he still has it. We don't need to discuss why he didn't fix it in the first place, but I think there must be a reason he is not fixing it now. 2. This is one of the most difficult problems for Orthopedists; a lot of procedures, long treating and recovery period, it's not guaranteed that your finger will be better after the surgery. This is the sad part, there is a risk that you can't make a fist or you completely lose mobility on the finger after the surgery. According to aaos, "Surgery can reduce pain and improve functioning, but it may not be able to fully correct the condition and make the finger look normal. If the boutonniere deformity remains untreated for more than 3 weeks, it becomes much more difficult to treat." So get it fixed as early as possible within 3 weeks. But if it's too late, like my situation, you need to think a lot and do some research before you make a decision.

I've been having swan neck pointer fingers for as long as I can remember and I was too embarrassed/shy to tell my doctor about it when I was little! My mom knew about it but she never brought it up to the doctor's attention either. I brought it up to one doctor now that I'm 22 and she had no idea and thought it was genetics but I told her my mom and dad don't have this. However my sister has boutonniere on her pinkies and I think also swan neck. I am using a ring split on one finger and I hope it will eventually help my finger be normal. although I know this would've been more effective had I started treating this issue sooner, I can't go back in time and surgery seems way to agressive! It seems tempting though bc you would think you would get a "perfectly straight finger" after surgery but that doesn't outweigh the risks/complications + scars! (Personally I think scars would be worse bc then people would notice your hands for sure and you would have to explain why you got surgery) vs with no surgery nobody can tell I have swan neck deformity unless I am extending my finger

i had an injury at my pinkie playing baseball, the finger bend to the opposite side. it passed almost an year already but the finger never went back as the same. I dont know why and I am pretty sad about it. I made X-ray twice and went to the doctor. My bone is completely straight but the ligament was sprained, and I really dont understand why I have a big pinkie finger now. For sure it is better and smaller than last year but it is still very visible. Do you know what it is?

thank you so much for this. It allowed me to cut down the splint I got from urgent care because i have the boutineere type in my left pinky so now i freed up my dip and mip joint and i can type this comment.

Six months since my injury and I have to say I am not happy with my healing. Despite wearing the splint for 13 weeks and another month after at night only...I have much difficulty moving my finger and I can't straighten the tip. It's permanently deformed. Difficult to play my guitar.

I fractured my pinky near the knuckle and surgeons never reset it. It’s in a hyper deformed position and malrotated. Happened a year ago and hard to make bar chords. Been debating on getting surgery man but so scared it’ll make it worse with all the scar tissue and stiffness cause I have great mobility in it now

OMG I fixed my finger myself o.o no idea it could get so severe... OMG... glad I inmobilized everything and it just grew back.

Feli González I got cut on my middle finger above the last joint and the tip keeps falling down it doesn’t straighten this happen 3 months ago I was in a splint for 2 months and I did therapy I gotta a lil of mobility but my tip of my finger falls down is not straight should o re splint it again ?

I have a pblm since my birth...my left hand little finger is bending.....they could not afford the expenses so I have to bear that pblm all along with.and now I'm 19 and this is so awkward when people around me r looking at my hand... and make pranks.... is this a way.....if it is plz help mee...iwanna be like others... I wanna simply shake hands easily.....

What are the chances of a full recovery from a Boutonniere Deformity? I'm somone that has the deformity in my right pinky finger. It's been about 12 months since surgery and 6+ months since my last physio. Unfortuntely the finger has returned to its position it was prior to surgery. Just looking for any help at all. I guess surgery again would be an option but I'm not entirely convinced that it's the only way to fix it. My left pinky finger (non injured one) has an odd PIP where it can bend both ways, my right pinky use to be able to do the same thing before the injury occured.

I have a terrible boutonniere deformity on my right ring finger from a surgery i had a few years ago. Its becoming a nuisance and quite painful at times. I don't have insurance so surgery is not an option at the moment. Any suggestions to loosen these tendons up?

I meet an accident before one year my little finger pip joint broken. After surgery my pip joint not bending. It only in straight position is ther possible to get my finger back normal

Hi rajesh kumar, y bcoz your finger not bending means ? your finger nerve cutted. u told u had a surgery, I dont know how the doctors missed to join the nerve? after 1 year I don't know it's possible to bend it back. ur cutted nerve slowly go inside in ur palm bcoz iam also had the same problem on 2005 April, but after one and half month I made a surgery after that my middle finger bended but it's not that much flexible like previous , now iam having swan neck deformity, iam not feeling my natural gripness

It's been 8 months since my boutonniere deformity on my pinky. I was on splints for about 2-3 months and now my deformity is coming back.(Right now it's not as bad as before. My finger was badly bent that I couldn't even straighten it). I can straighten my finger now, but the problem is my joints are not straight.(My DIP joint is bent down, while my PIP joint is pointed up). And the other issue is when I relax my pinky,, my PIP joint is bent to a 90 degree angle, but my DIP joint can't bend/relax like it's supposed to unless I force it to. And it's same when I purposely curl my pinky. I have to force it to bend. Is surgery the only solution? Will splinting it now still work?? Please reply...

Oh and my finger is still swollen and the huge callus is still on my PIP joint. I also forgot to mention that a tiny piece of my middle phalanx. The injury is very close to the PIP joint. I'm not sure if the fracture has healed yet.) Also when I do finger exercises, my PIP joint hurts.

I can't straighten my pinky at all, i think you have to get surgery, but i dont want to get it, oh and i dont even know when or how my finger got like this, i think maybe when i was like 9, 4 years ago🤷‍♀️

My mother had the boutoniere deformity. Her little finger on both hands was from birth. I suppose it was a genetic deformity. Both fingers were exactly bent the same. She never had pain from it and she learned to play the piano really good. Mostly gospel music. It didn't bother her. She lived to be 80. 1916-1996.

So how to treatment this deformity and don’t use a scalpel , l still don’t cearly understand 😢😢.the reason l think is my English aren’t very good . 😢 l got boutonnière deformity last two years,beause of an car accident what let my finger hurt 😢. The doctor said don’t care about this tiny deformity just l can’t play paino but l still hope my little finger can work .anyway Thanks u share this knowledge for who get boutonnière deformity.maybe l watch this short 100 times will know how to do for my finger

It's been 6 months for me, I can play piano, guitar very easily, but struggling with violin because of the full bend. I keep my finger in a straight splint all night, everynight. It will hurt for a while and stiff in the morning. If you move it in water it will really help. Then practice piano, take ibuprofen 2x a day. You will fix it 80 to 90% of usage. Better than cutting into it. Hope this helps! But healing ones self isn't for everyone, I'm not a doctor but I did the research! My husky did this to my finger!🐾

Is there actually anyone here that has gone through surgery and can tell their experience? I've had this deformity since last year (playing volleyball) in my fifth finger and since they could not recognize it at the hospital i didn't take care of it properly and now it's been like this for long.

Andrea Mandracchia I got this injury about 2-3 weeks ago from fighting I splinted it a hour or so later and didn't get medical attention until 5 days later and didn't get to see a hand specialist until 3 days ago and I was told to keep my finger splinted for 6 weeks by the specialist and said not to bend it at all and it will heal up fine. my advice is to get medical attention asap after the injury occur.

Boutonnière Deformities. My advice to anyone is to endure all physiotherapist pain if offered. Do your exercises & wear your finger splints even when they have to be forced on over the swelling. I wish I had this info at hand at the time & I wouldn't have my amputation to look forward to. Chase dates up! Just coz it's the NHS & there busy, trust me u don't want to end up in my position due to health carers INCOMPETENCE & Orthopaedic Surgeons are not gods, they make mistakes too. Google your surgeon & I think you might be surprised when there known for drink driving & not surgery. Good luck 🍀
