Never? I took the Shinkansen in Shinagawa last year and the train was delayed for 30 minutes. Only in one platform.

How did the train driver not notice a person on the tracks let alone hitting him/her and even if it was an animal, why didnt he report? Hitting something the weight of a soccer ball at those high speeds is enough to shatter glass so a human will definitely do a bit of damage
