well this is all fictional with doctors being able to tell the fetus' sex by checking the pregnant woman's pulse through a handkerchief which is even less realistic than no stds😅 in real life i think the stds problem was somewhat contained by sending girls to the harem while they were still young and had no prior sexual experience and grooming and disciplining them into obedience so they don't go around shagging guards and whatnot. if they are all healthy and their only partner is the emperor who is surrounded by doctors day and night and has access to all known medicines and treatments, it shouldn't be a constant problem
