Hello Doctor Liberman, I came here to say that I thank you for the informative videos. I'm a 27 y/o from Romania, and suffered for 10+ years from mild back back but in 15 of september 2022 hade a herniation that led to alot of pain. I came from a famly that don t trust doctors and the medical buissnesses. After watching you videos I took on the resposability and went to doctor. As of today 07 March 2023 I have a surgery with the removal of the hernia and I'm crying of joy cause i finally can t feel any pain. Thank you sir for the extra work you put in for people like us to be informed.

You made my day! Thank you so much for your comment. I am so happy that you are out of pain. Once the pain is gone most people get a window where they are unusually aware of how sweet life is, and what a great opportunity we are given with each new day. I hope you enjoy your second chance.

Also I have been fused from t8 to sacrum with one revision surgery a year later. Went back at 6 months still hurting and now he says I have pjk and he can't do anything about it because he said he was not capable of doing what needs to be done to fix it.. Great 😢!!!

I had trouble walking with shopping cart syndrome since 2019. In the last 4 months, I have had saddle anesthesia. Saw one doctor for pain management who suggested that injections would not help. Frequently lose feeling in my ankles when walking. Most recent 3T MRI (2 weeks ago) shows severe L3-4 spinal canal stenosis with thecal sack measuring 2 mm in diameter. I am 81 years old and swim 2000 YDS every day in about 50 minutes without pain. I have tried using an inversion table and hanging over a Roman bench for 10 minutes a day. Both actually help since I am now able to sleep on my back. I have no pain or saddle anesthesia when sitting. I have been told my condition is very serious and that I need surgery ASAP. The radiologist's report is all doom and gloom.

You were told right! Please hurry and get decompressive laminectomy before you lose more function. Make sure you find the right surgeon. Sooner is better.

I had the ablation done and took 70% of my pain away for almost a year. The first relief I had in a four year battle.

Did anything fix your pain closer to 100% or you still getting ablations? Whats your update now?

Thanks Doctor. I follow these videos because I'm at a loss. All the neurosurgeons and orthopedic surgeons I have seen don't want to operate on my lower back. My CT scan show, minor lumbar scoliosis concave towards the right side, L3/4 showing a minor disc bulge and facet osteoarthritis with mild foraminal narrowing, L4/5 showing reduction of disc height and a diffuse disc bulge. Facet osteoarthritic changes bilaterally with mild ligamentum flavum hypertrophy. This results in a mild canal and moderate bilateral foraminal stenosis. L5/S1 showing mild disc bulge and facet osteoarthritic change bilaterally. Mild to moderate narrowing of the foramen along both sides. CONCLUSION: Mild lumbar spondylosis and scoliosis with disc/facet degenerative changes, especially at the lower three lumbar levels. Is my condition that bad that they don't want to operate on me? I am going to have to consult your clinic.

Curious if he has pain bending forward or backward? Did you ask him that? That would tell if it is facet joint or not! If epidural helps then it probably is the disc at that L4/L5 level. DId he say it worked? Do you have a follow up with this patent? I bet he found relief if he had a microdiscectomy.

When I was 16 and diagnosed with a curved Spine and Wedged vertebrae, which was revealed later to be called Scheurmanns Disease. If they had braced that as a teen instead of ignoring it, it may not have deteriorated to this point which added in my mid 20's with Osteophytes; early 30's, Disc Bulging. All that ignored and add Cervical Bilateral Neural Foraminal Stenosis at multi levels in later 30's. Issue still ignored, and add Lumbar Spinal Stenosis, a compression fracture, and many other Disc Bulges, 40 years old. The 2 Neuro Surgeons and 2 Orthopedic rejects I seen just passed it off as nothing, due to being complete reject Surgeons , so it was passed off again. Tried loads of Pain management injections - the next one will be Radio Frequency Ablation, which is taking forever as Canada loves to breach the Canadian Health Act with us all and I'm suing my pathetic government for this. If the RFA works, so well and good for 10 months, but I can't see a surgery being delayed forever, even if no one is willing to touch it here in this reject Province of Newfoundland, Canada. It's unreal. I am consulting with Germany soon to see what they will say, as some other Canadians had great results with this clinic in particular. Canada is way behind the times when it comes to technology and my province (Newfoundland) is the province that time forgot. Stone Age Province. Anyone have any thoughts and in a similar boat while being failed by surgeons to help, and a failing government and no idea where to go to get actual options?

You definitely want a surgeon who specializes in deformity. I've seen good results from Dr. Rafath Baig here in Phoenix. You may want to contact him for an opinion about managing your deformity. He's excellent.

L5-S1 foraminal narrowing compresses the nerve root which gives you sciatica in the the glute, outer calf, foot etc...... no? I thought this was a very common diagnosis. Maybe you were referring to the L4-L5 foraminal narrowing and how that doesn't usually result in leg pain.

The L5 nerve root is in the l5/s1 foramen. That nerve root is sensory to the web space between the big toe and the one next to it, and helps you lift your toes when standing on your heel. You can actually test your l5 nerve root function at home using this info... stand on your heels and walk. If you can keep your toes in the air equally between your left and right foot, then your l5 motor is working. Scratch the web space on one foot and compare it to the other. If they are equal, then your l5 nerve roots are equal.

Thanks for the great advise! One question, would a direct visualized rhizotomy be preferable (permanent) to radio frequency ablation (only lasts up to ten months)?

Yes and NO. On the positive side DVR is permanent, but it's not covered by insurance and hard to find.

Wait do you do virtual consults? I have MRI and am looking for opinions. Had major sciatic for a couple weeks (was bed ridden) which went away but turned into weakness in my right foot and numbness on top of foot, and I can barely move my toes!! Have two doctors lined up here in LA but those are not for another 3-4 weeks.

I have a grade 1 Spondy with spinal stenosis at L5&6 (yes I have an extra L!). I've been to about 5 doctors. About half suggested a fusion and a laminectomy about half say I just need a laminectomy. I've already had a minimally invasive laminectomy at L3&4 that was very successful. But I still cannot walk more than two blocks without extreme pain. I've had steroid shots and an ablation. What do you suggest?

@@BestPracticeHealth right now, after visiting my Neurosurgeon last week, we have decided to get new MRIs in May and probably doing a TLIF in June. Meanwhile today I visited my podiatrist with curling toes, and he contributes that to the back issues! May need surgery down the road now with those toes on both feet! I don't know why my neurosurgeon didn't examine my feet? Would you?
