I've been practicing in the ER and urgent care for 25 years. Axis is easy...... Lead I and lead II are all you need. Imagine the QRS as two thumbs. One thumb in lead I and the other in lead II. If thumb I and thumb II both point UP then that's great... hey man, two thumbs up so this is normal. If lead I is thumb down and lead II is thumb up, then they are pointing RIGHT at each other, so this is RIGHT axis deviation. If lead I is pointing thumb up and lead II is pointing thumb down, then they are pointing way from each other or they LEFT each other. That's the quick and easy way on the fly in the ER when you gotta be fast and on your toes.

what if the direction of the perpendicular lead is not positive? does the lead run along the same (its own axis)?

Sir if the direction in the perpendicular lead is negative then what will be the axis? Suppose the equiphasic lead is leadIII and perpendicular to it is avR. But direction in avR is negative. Then what will ne the axis????

Nice video...really whatever is explain is clear cut....now what if lead 3has equiphasic wave...and lead 1or lead avf has equiphasic waves.....can u explain these two examples in more.....means we can have all six possibilities in hand then.....plz reply...thanx ..ll..may b my question will not b right...as not very deep in cardiology concept....but even if u say this...I can learn that my querty is baseless.....n surely I have assimilated what is given within video...n it is all for me to solve...axis related questions in entrance exam..ll(u have already explained same thing with avl and lead 2,will it b same scenerio with....combinations of remaing two sets of perpendicularlimbs..)

How r you saying equiphasic complex ? You said it’s perpendicular wave but you’re taking perpendicular wave in 2nd step so how to determine 1st step?

I have a doubt in 1st exampel you checked the equiphasic wave in limb leads and in the second one you checked the equiphasic wave in augmented lead So which one should be checked Please clarify me

Example 1 is also normal axis ecg no. Think logically since equiphasic lead is seen in lead 2 in example one , the lead perpendicular to lead 2 is avl . And avl shows positive deflection so the direction is exactly towards avl which is -30 degree. From avl to +90 degree that is avf is normal axis no. So how can it be left axis deviation.

In order to call it positive or negative you have to look at it in relation to the hearts vector. At minute 1:18 the big blue arrow is the hearts vector (physiologically always in that position). So the vector at -30 degrees is positive because in relation to the hearts vector it goes in the same direction. If it would go the opposite direction for example how AVR is, then it would be negative.

There is a mistake at 4:30. It should be lead I and lead II, not lead III.

Thats mnemonic. Some text says lead II and some lead III, but correct method is what I discussed first.
