This is a routine case and there is a nice incision, a great capsulorhexis, and the cataract was removed efficiently. Now the capsular bag is full of viscoelastic and the IOL is inserted carefully. As the IOL unfolds, you notice some faint lines. What are these lines? What should you be concerned about? What is your next move?

At the 8-second in the video, just before the injection of the visco prior to IOL insertion, there is a brief moment where it appears that the posterior capsule is already ruptured. The edge of the rupture line, which later causes issues, seems to be visible. The injection of the visco moved this line to a location near the equator where it was not visible, but during IOL insertion, the anterior loop appears to have caught on this line, twisted, and come forward again. While it is difficult to make a definitive statement without viewing the video from the IA phase, this seems to be the case.

Do you feel comfortable doing twist and out in a case of PC rupture? I feel like the manipulation from twist and out may cause more pressure on the eye and ultimately draw vitreous forward more than cutting the lens and gently removing it

The issue here is that, the lens went in too fast and too deep in a filled ovd bag which popped the posterior capsule, excellent recovery Good job)

I would have reinserted the iol in the bag , checked for vitreous in the AC and decided to let it or exchange it.

Using the red reflex can be helpful in such a situation.

How many lenses (of various types) do surgeons have on hand? How did this surgeon just happen to have an appropriate lens readily available? (Note: I’m not a surgeon, just a potential patient and curious viewer.)
