This patient had cataract surgery with the complication of a ruptured posterior capsule and retained lens material in the vitreous cavity. This is a complication that happens to all cataract surgeons and can be due to iatrogenic factors, patient anatomy and tissues, or both. The patient was referred to our guest surgeon who performs a pars plana vitrectomy and lensectomy. Then a three-piece IOL is implanted but the trailing haptic becomes disinserted during injection. What do you do now? How can you resolve the issue?
