After a restoration I was curious what my dentist had to do to accomplish the repair. From a patients perspective, I appreciate the artistry involved with this type of procedure. Thank you for the video! I just learned class 2 prep and restoration at my school and I’m amazed at how little composite excess there is at the margin. Do you have any tips regarding minimizing the excess at the margin or using #12 blade?

I have two tips for that. First, make sure you have gingival clearance with your preparation so that you are able to seat the bottom of your matrix below the gingival margin. Second, spend the time making sure your matrix and wedge are placed exceptionally well. This will minimize the need for interproximal finishing later!

I'm curious why you did not use a slow speed round burr on the dentin. Was the lesion not deep enough to fully penetrate the enamel and effect the dentin?

The slow speed round bur does not cut well on these lab simulation teeth, hence why it was omitted.

Hello Dr. Ryan, I keep getting voids in my proximal box. Do you recommend doing a shell/marginal ridge first to convert class 2 into class 1 or just pack the composite incrementally with the shell? Thank you!

Great question. I always built up the marginal ridge one increment at a time first to convert a Class II into Class I. Some people also use the “sandwich” technique where they fill the base of the box with flowable, and then build up the rest with packable.

My school keeps making us do Fiji liner LC which is only to be applied to dentine surfaces... i have so much problem to keep enamel walls clean esp at the contact point. This one uses adhesive/unfilled resin which i was also asked to use or flowable resin, but it is to fill up the margins. Am I able to not use liner? Because i know RMGIC is needed to protect the dentine because CR may cause some inflammatory directly. Also RMGIC has fluoride release which help remin affected dentine. I’m just desperate to find a way to do better for my next clinic session... if anyone has some tips please lmk thank you..

Well a lot of dental schools do teach to put a liner of GIC. This is not so wrong after all , but you need to think about your case. If you have a deep caries it is a good idea to put some liner ( Ca(OH)2 ,) and than continue with the rest of the restoration. If you find it hard to keep the enamel clean you can first put your GIC/liner and than renew your margins with burr, after that you begin your adhesive protocol and finish the restoration.

Proximal retention grooves provide secondary retention to help resist proximal displacement of the restorative material. To be honest, most clinicians today will not place them in a Class II composite preparation as long as they can achieve a good marginal seal and a strong adhesive bond.

Retention grooves in a class II composite are not necessary. Composite restorations are very technique sensitive, relying on proper bonding protocol only for retention. If a composite filling is being held in place due to retention grooves and there is poor bonding,, especially at the margins, that filling is going to easily get recurrent decay. Nice video, though!

What is the purpose of retention grooves, dove tail, etc. When using composite dentistry? Mechanical retention isn't needed for composite and surely isn't valued over conservative tooth preparation since we are dealing with adhesive dentistry i.e. natural tooth structure is to be retained as much as possible and we should only be getting rid of the carious lesions to preserve sound tooth structure

Interesting question! I would disagree with the premise that "mechanical retention isn't needed for composite." There is a wealth of data showing that while the adhesive bond to enamel is incredibly strong, the adhesive bond to dentin is not nearly as predictable. Therefore, there is some value in having mechanical retention via convergent preparation walls and secondary retention features where possible. A conservative preparation should always be the priority, but making a groove that is a fraction of a millimeter deep is not going to violate this principle. Retention grooves may not significantly improve shear bond strength, but it has been shown to improve resistance to displacement among other mechanical properties.

Patients often can't afford the good stuff in developing countries. Dentists are pressed to use traditional Amalgam.

Sectional Matrix System. Never used it yet. More expensive than regular Tofflemire.

@@ypherrist2792 We use them in our clinic, very nice for MODs and recreating contact in large spaces where you have to burnish out pretty far. In most normal circumstances a tofflemire or omni-matrix is just as good, though.
