I think she’s mostly recovered now, but she really liked the stream where someone was speaking for Marine when she was sick(was it Mio? Fubuki?) while playing Getting Over It, so I’m guessing she just decided to do it anyway for fun despite it not really being necessary anymore.

45p is sort of a joke name for her +18 audience. In Japanese 4 can be read as "shi" and 5 as "ko," which then makes the term "shiko," basically means jerking off. It's existed for a while but not really something she'd normally say on stream outside of bits like this
