The song is around 200 years old and most of the original lyrics have been changed and/or forgotten. Given the age of the tune, it's more likely to do with moonshine or field slaves, than getting the clap.

It could be it you see back during the time of the 1880s in the southern state moonshine had become a popular known alcoholic beverage, but most controversial beverage happens to become known as cotton eye, Joe was the type of fermented moonshine that combined with whiskey, wine and other various alcoholic beverages, combined to the strange concussion of a drink that was rumors that once somebody drink it, they were a port gonna die from severe blindness or stroke, or a seizure, and there’s been reports of babies being born with deformities and developmental condition that’s why the drink has a caption says do not drink while you’re pregnant until the company that made the moonshines Tucker and bells beverages was closed in 1922. After reported cases of deformities and mortality rates increased, but in 1921 it was basically reference to the white, eerie foam that has a little hole on it that resembles that of an eye, and this foam has various array of toxins to it. It was first seen by barrels by investigators once they open it, they felt a strong, pungent smell. It was due to the amount of yeast and funguses that give that alcoholic flavoring, basically just overcome cognate, and just up a chemical reaction to the barrels, and having a very strong, pungent smell, and a foaming this give you the eerie look to an eye. Hence, the name, cotton eye, Joe, and a Joey is a southern, saying for a really bad fermented moonshine that goes AWOL it needs to be dropped out we should did in the sewer system

It's definitely about moonshiners who got infected with STDs.. I've heard this phrase used when I was living in Alabama for almost 8 years. Not a common phrase used outside of Alabama except for a handful of Southern states where it is mostly used by moonshiners!!

That’s the original rendition, the 1860s version could’ve been about anything but I think it and the 1994 version are just about a handsome dude who shown up to a town for some period of time and swooned most of the ladies
