Dr. Charles is truly amazing! He diagnosed the woman so easily. His knowledge of medicine and psychology makes him the best doctor on the show, in my opinion.

​​@@drakecarter1780 House has a whole team of people who help him diagnose patients, some of whom double as foot soldiers to break into his patients' homes on a dime. Dr Charles is not only working as one man with perhaps a doctor to help, but he does it without risking being sued for medical malpractice, property damage or assault every week. Dr Cuddy has to set aside a whole budget and a legal team to tackle House's incidents. Dr Charles is not only more efficient, effective and kinder as a doctor, he's also a better deal.

Even for a psych consult they are suppose to do blood work, EKG and at least check your vitals. Ive never heard of that not being done first, while waiting for a psych consult in the meantime.

it’s actually happened to me. when I was 9 I went into the ER for a different medical reason, and when they were asking the standard questions they asked “have you had thoughts of harming yourself today?”. I was a kid so I stupidly answered honestly that yes, I had. they immediately sent me in and made me do the whole psych routine without checking my vitals or anything. they only got to what I was actually there for after I waited an hour to be cleared by the psychologist. my parents were furious, and I’ve never been honest with ER doctors about that stuff since.

Even in my experience, they have to do a full work up to make sure no preexisting conditions are to blame for psych issues, even with documented mental illness

I've been to the ER for psych issues, can confirm. At one point in my life the nurses commented on how I automatically held my arm up for checking when I was first seen.

This whole episode was a level in 'the writers have never been near a hospital or even watched their own episodes'. Because getting a legal guardian for impaired patients is also just standard operating procedure. It wouldn't even take that long and they specifically have a staff member who handles finding guardians and scheduling the hearings asap, iirc.

Falsifying consent in this situation is the sort of thing that get's doctors permently disbarred, and is really stupid because there is a very simple procedure - an appointed guardian that would almost certainly agree with the decision to perform surgery. The issue here isn't some massive ethical dilemma, it's a surgeon with a God Complex that can't be bothered with "silly" rules that protect patients from abuse by arrogant doctors.

They didn't have her informed consent, but it was still legal because two doctors agreed that the surgery was essential and signed off to that effect. I believe that's the procedure if there's no next-of-kin to consent.

The law over here is that if someone is not able to give consent then you can assume consent. That's where it gets grey

"Ollie" (Oliver Platt) has been my absolute favorite since the "new" BratPack of the '90s. What a gem of an actor , and from all accounts of a *person* as well.

I'd say that would typically be in a situation where they're likely to die if they don't get the surgery done immediately but I think the implication here is in this situation shes not showing signs of being in immediate danger and therefore it would be assault to do the surgery since it's not a dire situation

One common symptom of altered mental status (vague catch-all term) is that your memory is very poor, or you’re there but you’re not understanding and you don’t even know that you don’t understand, so you wouldn’t remember

Yes. Years ago, the pillow has been signed by family members and close family friends. I think it’s currently tucked away in storage. @@kellybeck4579

@@MrsCKane It's even better if you notice that you do "see it coming"...in hindsight. Throughout the season he has smaller and bigger moments struggeling, but it never feels like preparation for big "OMG"-moment

Well he was under a lot of pressure and severly depressed and self medicating with alcohol. I believe he later comitted suicide

Ich liebe Dr. Charles als Mensch weil er persönlich mit Menschen ins Gespräch kommt und sehr nett ist vom Charakter. Dr. Charles ist super als Mediziner und Psychiater. Was ich sehr positiv finde ist das Dr. Charles nicht nur psychiatrisch denkt und behandelt sondern sehr genau nachfragt bzw hinterfragt ist es psychisch oder wirklich eine ernsthafte körperliche Erkrankung . So einen Arzt hätte ich auch gerne gehabt.

I'm glad that in most of Europe in casess like this, decision goes to Doctors - because that patient could very much die. Dr, Charles was lucky his gamble paid off

Ideally they would still need to give informed consent. If she is incapable (not unwilling) to give it then it would be a relative or legal carer. If they are not available and its life threatening then doctors would be able to act in what they think is the patients best interest. The shrink has said she is incapable therefore it comes down to what a reasonable competent (preferable with a 2nd opinion) surgeon thinks is for the best. The shrink case is the most interesting here through, a patient is assumed to be competent unless clearly shown otherwise (ie unconscious, semi-conscious, formally diagnosed with a serious mental health issue, courts have ruled). A shrink saying not competent just like that is an invasion of a patient autonomy

If it isn't a "perform surgery NOW or she dies in the next hour" level of necessity, doing it without informed consent is illegal. And she's not in a position to give informed consent. Though really, this is precisely the sort of thing they get next of kin in for, don't know why they didn't do that.

Plenty of people don't have next of kin. My dad is 71 and when he dies my nearest relative will be an aunt in England, and no hospital is going to be able to find her without me telling them.@@dragon_toad

dr. charles wanted to start her on antibiotics first to see if that cleared up the infection. she wasn't at imminent risk of dying if they waited on the surgery, and she did end up improving on the antibiotics so surgery wasn't needed after all.

he’s exactly the kind of person who would do this. Any doctor, who actually follows their oath, would not do surgery on a patient that they did not consent to. But especially seeing as he’s a psych doc he would know better than anyone, whether or not a person is capable of giving consent. So it actually makes sense that he is the one calling it off.

Yeah like Alex said he's a psych doctor so he's very able to give advice regarding surgery if he's concerned the patient is too psychiatrically impaired to consent to it. Plus she was his patient at first

Are you talking about the cardiology bits? He went to medical school like any other doctor and has years of ER experience, he knows body medicine just as well as mind medicine
