Gabe’s 6 goals for improved compliance/best outcomes night stretching AFOs-
- 1. custom orthosis design to correct and protect subtalar joint to ensure that a stretching brace is not doing damage. -I never recommend pre-fab night stretch for chronic neuro/ortho kids.
2. Adjustable stretching tension improves compliance AND results by allowing you to increase the tension to improve stretch OR decrease the tension if needed to improve patient comfort.
3. Dorsiflexion limit provides a “finish line“ for parents to know when they have achieved success and how far they have to go. Also provide safety/patient protection ensuring that the orthosis will not stretch patient’s ankle joint beyond the anatomical range of motion.
4. Tension release or tension lock that removes the stretching tension from the orthosis to allow for proper donning- so that the patient’s foot can be properly, easily seated into the orthosis, in the correct alignment and strapped into position easily.
5. The dynamics of the dynamic tension band allow for spastic release AND for progressive stretching throughout the night as the tension band continues to apply mild and adjustable stretching pressure to hold to patient at their end range even as it moves a few degrees during the night.
6. Included goniometer so that everyone(parent, PT, orthotist) can understand the patient’s current range of motion and to measure progress towards the goal +20 dorsiflexion.
