The difference is Terri's friends weren't nice to him, even before the abuse started. Rick wasn't bipolar he had anger management problems. Him wanting to leave was a result of Paige saying something that hurt his feelings, and when Terri decided to stay he took that as her not caring about his feelings. The kid needed help not others picking on him. And I think Terri realized that which is why she gave him a second chance, but she didn't actually know how to help him.
