I get to do an HCP (Health Care Professional) Trial of the NEW Medtronic MiniMed 780G insulin pump with Guardian 4 CGM in SmartGuard Auto Mode! This is my overview of the 780G system - the next video will have my opinions on the system for my own diabetes management.

Ive been with medtronic since 2017. I got the 780g upgrade in the summer of 2023. Im very happy. My a1c that i got 2 weeks ago came in at 5.4. That is the lowest a1c I've ever had. I normally run 5.7 to 6.2. I do have to say that while their new algorithm and sensor 4 work very well, their sensors leave a lot to be desired. Lots of change sensors well before the 7 days and the size of the sensor are my biggest issues with medtronic.

I'm with you! The problem is that the 4 is the 3, repackage. Plus, they made it so you can't extend it, but I'm in the process of getting around that. It's great to hear that your a1c is that good!! I used to be in the mid 6's until I started using the Sensor 4. The Simplera will be the game changer when they get it compatible with the 780g. Go figure, they made a new cgm that's not compatible with the best pump on the market. Granted they'll send you a replacement, but I hear that they are starting to cut back and give out a certain amount now. Depending on circumstance. Good luck and all the best!!

The 780g is amazing I have used Medtronic Pumps for probably 12 years, My last A1C from1/4/2024 was 5.8 I was shocked especially with Christmas, My previous A1c was 6.0.

Thanks for this info. I Just upgraded to the 780g with the sensor yesterday, had the 770g for year and a1c went from 9 to 7. So hopefully this gets it down even better. Hope everyone else does great to.

Medtronic sensors never worked for me so was in manual mode until i was able to switch. I enjoyed Animas and Tandem over Medtronic, I'm now trying the Omnipod but just wish they made a larger reservoir pod as its going great so far.

Dexcom/Tandem are better for CGM but I prefer the design and interface of the Medtronic pumps- battery operated, no annoying touchscreen that you can't use in the rain, easier to fill and replace the reservoir and an overall tougher design.

I'm so excited to hear your experience with it!! Where I live, omnipod and medtronic are the only pump options, and dexcom/freestyle is the only cgm option, so this is helpful :)

I just received mine in the mail today. I'm very excited to use it, but sadly I have to wait until I meet with my diabetic educator in two weeks for proper training

Started 780g 3 weeks ago. A1C with Tandem T- Slim was around 7.8. Lost the T- Slim pump while still under warrantee but Tandem wanted $5,100 to replace it. Switched to Metronics. My time in range for the 3 weeks so far was 95% . This was without using a preset bolus. Consistently averaging 135 to 140mg for the daily average = A1C of 6.5. I never could get it that low while using the T-Slim.. Unfortunatly, I am using the Guardian 4 and I agree that it is a little out dated and much harder to apply than the Dexcom G-6. I will be switching to the extended wear Infusion set and am looking forward to new Simplicity CGM sensors when they hit the U.S. market.

Hello , i have the medtronic 720G and i will upgrade to 780G or the accu check Solo (Tubeless) . I probably will gonna change to the tubeless. I have a group with a big group with 780G and the sensor are always updating or disconect.

thanks for update, I just received a 780 and only thing i dont like is i have all these sensor 3 that are not compatible with 780g. I didn't know about the shortcut for down, right and up buttons from home so big ups to you for that KT. Great to hve a well educated ,,,,educator

Hey mate I’m sure you can use the G3’s with the 780, it what we used here in Aus until June last year as that’s all that was available. G4 began in July. Only difference is you have to calibrate it but it will live happily in Smartguard no problem.

Danica, I love my 780, the only complaint is my sensors only last 3 to 5 days on average. I wear a freestyle and have no problem getting 14 days. Any tips on getting closer to 7 days with the guardian4

My 780 insulin pump will alert me when my blood sugar is at 250 which is what it's suppose to do. Then it says it's gonna administer a correction of insulin and after that the load screen after being administered the bar shows going from left to right. Showing that the insulin is being delivered. However, after that's done and I go back to my safe guard screen, I have no active insulin in my system and no insulin was given. Medtronics is unable to see this in their system. Has anyone else had a similar situation? Please let me know. Thx!

Why must they be so intrusive/large compared to Dexcom and Libre? I'm just not using mine despite loss of features due to that.

I have had type 1 diabetes for almost 10 years now and I've always injected myself with a pen refill, never been on a pump before. I'm now going onto this pump. do you have a video which shows more in-depth like you actually using the menus on the device? like setup or when you're having a meal and entering your BGL's? tia

If using this exact same pump, etc, you tube is a great place to find videos from others & even the brand itself. Medtronic also has videos on their website that can help with your questions.🙂

Most curious to hear your experience with the Guardian 4 sensor. Can it be calibrated? It would also be interesting on how they got the extended time on the infusion set. Three days has been the so called "standard" since day one of infusion sets.

yes it can be calibrated and when you put one in make sure you do because it differs from the real bloodsuggar

can’t comment on the guardian 4 as i don’t have mine yet but i’ve been using the extended infusion set and all i know is it just feels a lot more sturdy (not sure if that’s the right word) and the materials just feel better.

@@bombayjake just as sturdy as the same ones in white that last 4 days, if you can go without changing the insulin for 7 days it works, if you have to change it every 4 days it is not worth the extra cost

Yes, I have a few times! I’ve been diabetic for 30 years now. Been on an insulin pump for about 25 of these years. It gets frustrating & overwhelming to say the least! Talk with your doctor to have fast acting & long acting insulins scrips on hand at your pharmacy. This way if your son feels he needs a break, and you & his Dr. agree, this is one of many options I have used. It may only be for a few days or a month. You still have to be very diligent about testing your blood glucose. Unless he is ok with just wearing the CGM, such as one that will send info to a mobile phone also. Especially being in the teen/younger years, I can understand how burn-out can especially make living life much harder. ❤

@@JMyoutube1From experience, it’s where you get ‘burned out’ of having to constantly stay up to date on your insulin needs. Even with a pump, it can get overwhelming at times. Guessing carbs for amount of insulin needed, pump site changes, reservoir refills, accidental removal of your sites being pulled off or having to remove one because it isn’t working. Constant finger pricks for blood sugar levels… stress in life can & will affect your A1.C. Having support from family, friends & medical staff is important. I hope this helped answer your question.🙂

Can I use the G4 sensors with the 670G? I got off the pump due to 2 recalls and went back to pens. I still have a brand new 670G in the box. I can it exchange for an upgrade, it will knock -500 off the new unit. I’m trying to determine if I’m gaining anything with a new pump.

Balony! Medtronic’s C/S at least for getting a 780g/G4 setup has been a nightmare. I have worn Medtronic pumps exclusively for ~35 years. (508 on) This time around the move from a out of warranty 670g to the 780g is impossible. I am still being told “By the end of the month” for several months now. And it isn’t the insurance dragging their feet for approval. The delay put me in the hospital with a +1000 glu. a month ago. Had to wait for a loaner 670g to replace a failed 670g pump for 4 days (the pump was 4yrs, 3mo old and just out of warranty). Medtronic won’t ship to hospital’s so I had to go AMA to wait for the UPS driver to show up at my home to get the loaner pump I am using now. Not fun times. Making me look at other options closer to a DIY closed loop system for this go around.

Hey I watch your video but never comment. I have a question when you’re low and you treat that low. And eat a snack for 15 grams do you cover that snack with insulin?

With your example, usually 15 grams will raise the glucose level 50 points with no insulin. So if you are at 60, the 15 grams will bring it up to around 110. The one thing I have found, being on a pump that is controlling basal insulin, when going low the pump reduces and even stops basal insulin to keep you from hypoglycemia. When you start treating the low there is very little to perhaps no insulin on board. In which case I may add 1/2 unit of insulin so I don't spike.

If using an insulin pump & in auto mode, or even not on the pump, I still do not give myself insulin to cover the carbs eaten. You don’t want to give insulin, even after eating, if you are low. Unless you test later and are now high or eat more than your low blood glucose would need.

I have the Minimed 670G, an irregular lifestyle, and do sport 2x a week. Unfortunately, the blood monitoring - 670G CGM sensor is not good enough for me to activate smart guard mode. How much better is the sensor in the 780g? Is anyone (780G user) experiencing high differences between blood sugar and CGM sensor readings in certain daily situations?

I have been using the 780g since the start of summer 2023. It is very accurate. However, keep an eye on how you fell when you are very active as it does have a slight delay on where you truly are and what your pump is showing. I've had very few lows that lead to any issues with the upgrade and when I'm going to be very active I always bring a snack that will fix a low quickly.

I have been using 780G and Guardian 4 for about a year now. I have been on Medtronic pumps for many years, usually in conjunction with Dexcom CGM. I was initially very skeptical about the Guardian 4 CGM since it has the same form factor as the earlier models, and it is quite difficult to insert (I cannot do it without help from my wife). I also had some initial trouble getting used to the closed-loop setup, maybe for a couple of weeks. But once I got adjusted, it has been a life changer. I don’t have the nightly lows anymore, and my A1C has improved from 7.5 to 6.8. I am looking forward to their next-gen CGM (Simplera), which is supposed to be much easier to use (though still lasting for 7 days, and not 10).

Tandem tslim x2 and the omnipod. In my opinion the tslim x2 is a very good pump. There might also be another pump manufacturer working with dexcom now but but I'm not sure who that is.

Honestly didn’t realize Medtronic was still trying to do CGMs….I was on a Medtronic for years and years but once CGM technology started, Dexcom blew them out of the water so quickly in features, reliability, etc. Their CGM tech was laughable to me. Omnipod+Dex for me 💙

I am a 68 year old man. Two years diabetic type 1. I still work as a plumber and have to crawl around and get banged around. In addition it get wet by dirty water. How does this effect the pump?

Hi. Fellow Type 1 here. My job causes me to be all over the place & who knows what each day I run into. Make sure to purchase a study case. They have clips on them or not, depending your preference. There are many after-market out there that are great! As for your infusion sites, make sure to put the clear covers over them. Medtronic sells them or after market also. These can be clear with a sticky base that adheres to the site itself & your surrounding skin. Others come in many colors but are of thicker (not necessarily better) type of coverage like that of a thick band-aid? like covering. But make sure when u apply that there are no openings to the outside edge on your skin. From experience, I use the clear kind as I can see if there are air bubbles when applying & these are meant to last through showers, light pool time. Just keep in mind that using these ‘covers’ makes one unable to disconnect from the pump. But pumps now days are much more water resistant then they were when I started mine 25 years ago. 🙂

Im a 62 year Type 1 - On the Medtronic 670G pump for almost 8 years (just upgraded to the 780G) For me, I have the black silicone case for some protection while in my pant pockets. I sometimes remove my pump and "suspend delivery" when under my cars or home construction activities. My decision that limited time suspended is NOT the end of the world and i typically have normal to low glucose levels while active. The pumps are somewhat water proof to a degree.

I've had the 780g for a little over a month - I hate the sensor!! It does NOT last 7 days - lucky if I get 5 days out of it. And what's with the complicated way to put it on? Used Libre 3 for 6 weeks and loved it - though I was giving myself 4 shots a day. Would love a happy medium...

Hello, it's been a while watxhing your content. Unfortunately this info might as well been in giberish. I for one have no FAITH in Medtronic. For many reasons. Like their poor manufacturing quality control to their pump insulin delivery being dangerous. IE their sensor seeing 40mg/dL and insisting on micro bolusing. Then their Tech support reps and supervisors insisting that isn't a "bug" but actually a "feature." I can't imagine their sensor actually being any better than the garbage Guardian 3. I also am not happy that because of their poor QC they forced a recall on MY perfectly fine pump. Then the "new" pump died right after warranty expiry date. That was 2 negative in one. I converted to Tandem and have reached the sub 6.0 HA1c for the 1st time. Next option, if needed is Beta Bionics and their iLet. Rant over.😂😂😂😂

After the 670G that was harmful to my son's physical and mental health, we are very afraid of trying again, You are stuck due to insurance that won't pay for a new one for 4 years. The transmitter was a SPEAR, We left and I am afraid to go back.

They guarantee the sensor for only 5 days. I’ve only had one sensor that lasted the full 7 days. Dexcom and Libre sensors are really not much better. The weakest link in the system is the sensor

Medtronic made a big mistake by not making a deal with Dexcom. Medtronic and Dexcom would have been a perfect CGM pump combination. I tried all their Guardian junk and its not accurate or reliable. Shooting their harpoon thick guardian sensors into my skin will never happen again. I have boxes full of their guardian sensors thats going into the trash.

Guardian 4 sensor is super awkward, leaving rush and scar marks. Very annoying, you can’t put it yourself on your hand. The plaster is to big. Have to wait 3.5 years to change the pump and get Dexcom. Dexcom is the best. Or Libre is much better. No Guardian 4!!!!!!!

After using the medtronic system for 7 years I upgraded to tandem paired with the dexcom. When the tandem pump is properly programed and you use the dexcom with it consistently it is a great setup.

Agree. I used the Medtronic 670G for over 4 years. Its programming was faulty. It did not even try to keep blood sugars in a normal range. And it did not try to bring high blood sugars down to normal either. And the CGMS was extremely inaccurate. Tandem is fairly successful most of the time in keeping blood sugars at a normal level. And Dexcom 6 is very accurate, except the first day. Tandem still has problems and is not perfect. But you at least know its trying and most of the time it does succeed.

This pump and cgm are not worth it. The 770G is much better. The sensor for the 780G lasts 24-48 hours
