Board Certified Prosthodontist and Board-Certified implant Dentist, Dr. David McFadden, discusses the differences between the ever-popular “All-On-4” treatment and Implant Bridges.
He also discusses dental photography with “lips retracted”; revealing everything, not JUST the front teeth and lips, like we see in ads, or on dental website photo galleries. He challenges that with dental reconstruction “Before & After” cases, when they’re photographed to show you how beautiful a dentist’s work is, that it’s misleading to only show that part without showing the full scope of what’s “under the hood” so to speak, with the dental restorations they design and deliver in patients’ mouths; particularly with full mouth reconstructions. He argues that: If you cannot see the foundation or gum line areas of these full arch or full mouth restorations for a full close-up view, how can you truly judge the abilities of a dentist’s best work? (The work he/she’’s willing to put out there for all to see?)
He proposes that in order to be able to see a dentist’s reconstructive and cosmetic results, they should be willing to retract the lips in their photos and not ONLY show the front teeth and the smile, but the foundation, as well as.

the fact that these are the only kind of reviews we tend to see makes me scared about dental work. I don't need my dentist to call me. I don't need the receptionist to be friendly. I need the office to BE clean, but I don't need it to LOOK clean. I need long lasting dental work, not a day at the spa. It's not bad to be caring and empathetic, but let us know how his actual dental work holds up over the years

Can you please explain something to me. I can’t believe what happened. I had an implant bridge and it cracked. I had to save for a bit to get the 10,000 to replace it. I assumed he would just make me another implant bridge. My dentist put in an implant denture. It is awful. I went back and said I would wear the cracked bridge instead of this. He said the change he made is irreversible. I just sat there sobbing. He said he would make another replacement and would not charge me more. I just could not stop crying, I didn’t understand that this could even happen. Is there a way that what he did is irreversible? He did not cut my gums at all. Is there anything I can do. Thank you so very much.

I’m looking into getting All-in 4s. The dentist didn’t even give me an option about dental implant bridges. Now I’m confused. I thought all in fours were already pretty expensive saying $27,000.. And this guy is saying that the dental implant bridges are more expensive? Do all-in 4s last a lifetime?

Hello, Dr. thank you for sharing this valuable educational video. I have a question regarding implant bridges. Do you extract all the teeth before placing the bridge, or do you prepare the existing teeth and create a single piece implant bridge? I would greatly appreciate your insights on this topic. Every patient is different. But, we do NOT extract all the teeth. We extract selective teeth, place implants and measure their tightness. If the first four implants are tight (high insertion torque) we extract a couple more, place two more implants and measure their torque. For low torque, we leave teeth in to serve as abutments for the temporary teeth for 2.5 months, come back and extract the remaining teeth, place additional implants then convert the temporary to all implants. I call this a "strategic" approach.

Without x-rays or seeing you we can only give you a large range of $25,000 - 70,000. If you have dental insurance that covers either the implants and/or the bridges, you’d get your annual maximum from them. Typically $1,000-2,500 total. In some cases, when the treatment spans over two different calendar years, sometimes you can get two years worth of insurance benefits.

What is required for implant bridges if all upper teeth have been extracted?? Do All-on-4 screw in dentures always sit on stilts which causes the implants to be visable? When they show the denture it does not look like there is an intermediary piece between the denture and the ridge - it looks like it sits directly on the ridge. Please advise

🦷I am impressed with your expertise. I’ve been researching what is best. I have a Tori and need full mouth restoration with implants. I’m convinced all on 4 is not the best. The other option i found is 3 on 6. Which uses the natural gums. I am so grateful for how you have explained this in depth. 🌷thank you

WOW! I stumbled upon this video AFTER I completed my treatment plan, but the lips retracted pics DO speak for themselves! I had no idea that is what's going on behind the lips. 😬😲 I just received my 4-unit, implant bridge--a true implant bridge--and I love, love, love it! (not from this specialist, but Dr. Ramsey Edwards in Sierra Vista, AZ). Dr. McFadden is correct! Please do your research and don't just go for the cheapest choice. Go for the BEST, LONG TERM CHOICE! And, I'm confident that if I'd been in Dallas, TX, Dr. McFadden would've been my choice! p.s. I don't have dental insurance so I paid cash as we progressed, but it is $$$ well spent! 😁❤️

I’m so confused. What is the difference between the implant bridge and the all in 4s? I just had my consultation today with my dentist who wants to start the All-In-4s next week. Did they extract all of your teeth?

Great video - thanks so much. I have been quoted for all on 6 on the top arch. I have an existing 5 piece bridge whose supporting teeth failing - one adjacent tooth failing, a gap from an extraction, and a root cannaled / crown failing - so 8 adjacent teeth need handling. 5 good adjacent teeth on one side - I can't see why I can't get two implant bridges and leave the good teeth alone? I hate the idea of this artifial gum.

That all depends on the status of those teeth and the bone. Most patients with some teeth can have implant bridges and SHOULD NOT consider All-on-4 treatment for all the reasons I state in my videos. Good teeth should be saved until there are so few good teeth that they "get in the way" of the reconstruction.

Question: How do implant bridges differ from the 3-on-6 process? There seems to be a closer tie between the two than implant bridges and All-on-4.

Implant bridges and 3 on 6, (given the 3 on 6 is done with NO PINK porcelain or zirconium,) are the same. The difference being that one is a single unit and the others is 3 units. The biggest difference we are trying to show here is not necessarily whether the prosthesis is a single unit full arch bridge or three units, which esthetically look the same, but focusing on the fact that the pink tissues are the patient’s own gum tissues.

No, they are screwed in and only removable by a dentist. How long they last depends on several factors. Keep watching - I have several videos on this subject. They should help you understand all the variables involved, and different options available so you know just which questions to ask and which terms to use when discussing full mouth implant treatment options.

If there is lack of bone and you wanted 3 on 6 or fixed bridge, i assume they're essentially the same, then can a bone graft be carried out to solve this problem?

We can add width (thickness) but not height so there may be pink at the top of the prosthesis. Similar to 3 on 6 but a better version.

“Last purchase you make for your mouth.” But why do they NEVER mention the fine print in the needing to get these things cleaned professionally every couple of years for at least $1000 a pop each time? That’s where it gets upsetting. It’s not the last purchase.

2 years after replacing all upper teeth with an all on 4 with a different implant dentist, my partner has speech issues with lisping and pronouncing certain sounds like sh. Is it possible to correct this problem by shaving the teeth on the denture for example? Thanks

The prosthesis could be remade to fit more tightly against the gums, closing the open space. OR- *If the prosthesis is made of acrylic, (this would be the easiest, as instead of remaking it,) they could modify the underside of it by adding more acrylic to fill in the open space where the air is flowing now and causing the lisp. I actually have photos of this in one of my other videos. It’s on the “Buyer Beware: Problems with All-On-4” video. I had a patient for whom I modified the underside of his prosthesis because of food entrapment and not being able to properly clean between the prosthesis and gums. (Different symptom of the same problem as your partner.) Thank for for this video, I have a question: i had the multiple implants, and the true zirconium implant bridges (like in the last case you presented, NOT all in 4}. However mine don’t look like in that last pic. The teeth don’t sit on the gum but are more large and tall. What was the difference? I’m not satisfied with the job even though it was an expensive job which involved bone grafting etc. Can you guide me with what should have been done different, and if there’s something to be done in a case like mine? Thank you!

Hello Dr. McFadden, I had all on 4 done 3 years ago and I am very disappointed with the outcome (especially for 45 K). Could I have this converted to implant bridges? I have at least 1 angled implant resulting in a fairly large amount of acrylic on my palate and floor of mouth. My speech is terrible. I am at my wits end about this. You can see my implants when I smile which I don't feel like doing. Can you help me or am I stuck with this?

A couple of questions, Doctor: Did the patient at 7:27 have severe periodontitis? Why would she need to have here upper teeth extracted, if they could have been restored with crowns or veneers ? And , in the case of a patient with significant ridge loss, how would an all porcelain implant bridge look like? Wouldn't the teeth look extra long in order to meet the gums?

The main difference is placing implants to avoid angled abutments. We add pink zirconium if the patient has pre-existing bone loss. I do not remove bone in the maxilla and only when absolutely necessary in the mandible. Second big factor is creating a prosthesis that is convex and not concave. I hope this answers your question.

I'm turning 65 and starting Medicare soon. Can I buy a Dental Plan that would cover a small portion of your procedures like extractions, bone graft (if needed) ? And if so which insurer is the best one to go with? Thank you !

Unfortunately, I cannot advise you on that. It’s a tricky thing to win with as a private consumer. After all: The insurance companies are in the business of making money. And that ultimately means collecting more in premiums than they spend paying claims. The bigger plans, through large employer sponsored group policies (think IBM, for example) tend to have better coverage and are useful because you only pay part of the full premium and the employer pays the rest. Additionally, from our experience, private policies very often have waiting periods before “major” services are covered. You may find one to pay for the extractions during the first year, but not the bone grafts, and but the extractions are a minimal part of the cost… and it’s usually a wash, if not a loss. And the yearly maximums they pay haven’t changed much since the 79s. Most still limit com overage to $1,000/year, and then nickel and dime you to death on what they will pay for. If you do shop around, I’d advise you ask about “waiting periods,” for certain services and “missing tooth clauses”. (Meaning if a tooth was missing when you started coverage, there is no coverage at all for that tooth/those teeth, even as part of a bridge or other treatment that ultimately replaces it). And some, (like HMOs) dictate who you can go to and allow no coverage out of network. I forty’s my, it’s just not like medical coverage. It’s hard to come out on top :(

In the example of the implant bridge I see that there is no fake gum, but what are the other differences that make them a bridge and not a denture on stilts as it were? Is it not one long continuous piece like the all on four? Also, how does the gum know to grow around the prosthesis in that way? Is there any risk of the gum receding exposing the implants? Do you know how much the cost varies by market? I am in VA but being forced to move unfortunately to a very expensive area of CA that I can't afford-I am disabled and dependent and live with my parents and very scared of all the high costs of everything, but thinking this will impact my mouth restoration as well.

Thank you for the great explanation! This implant bridge looks so beautiful resting against the natural tissue. How many implants need to be placed for this type of bridge? What does the x-ray look like underneath? Over time, will the tissue eventually shrink away from the bridge causing a gap?

Typically, 6 or 8 implants. The post-op x-ray showed good bone levels around the implants. With proper cleaning and maintenance, the tissues will remain where they are after implant healing and final bridge placement (given that this has not been rushed, and the tissues were given ample time to fully heal prior to bridge fabrication and insertion). WITHOUT proper cleaning and maintenance, the tissues and bone can shrink, just like around natural teeth, so good homecare maintenance is VERY important!

@dentalimplantcenter, I am wondering so a full arch of 6 on 6 is not the same as a fixed bridge? Can you go more in detail please? Also regarding the lips retracted, i went to a locationa nd all their photos showed the lips retracted to show the before but not on the final look, so you mean this is a concern? Also do you do discounts for someone paying cash or take in consideration the living situation of the person to calculate the fee?

If you mean an All On 6(all teeth on six implants), not 6 on 6 as it’s typed: That depends on whether they use angled implants, angled abutments with bone removal, as so many of my videos mention. As far as the lips retracted question? Yes!! It’s a huge red flag. They show you lips retracted BEFORE pics but not AFTER pics? Run.

@@sandracano5910 it’s not the number of implants that determines the final look of the prostheses. I suggest having a talk with your dentist and asking him detailed questions about the final prostheses design and look. (Pink? No pink? Etc)… I go over this in great detail in several of my videos. You can learn about just which questions to ask by watching and gaining a more clear understanding of the overall process and variables involved.

So in order to avoid having an outcome like this where the implants are showing and looks pretty bad, we can opt to have it done in 2 steps, like extractions first and wait a few months and then have the implants placed?

I have a current bridge , 7 units total, 3 upper anterior teeth lost decades ago…my bone loss there is quite a bit and won’t be able to have implants on those 3. How much does it cost to have a zirconium dental implant bridge, 4 implants total of 7 units? I have gone to 2 consultations already here in CA and the tx plans were the same, 4 implants for my 7-unit bridge. I am scheduled to fly there in TX in less than 2 weeks, on a remote or virtual consultation, they are recommending All-on-4 as being more practical for me. I am not sure if I want that. Came across 3-on-6 or 4-on-8, that’s the one that almost the same as your protocol.

It's bone volume specific. Some people lose bone while they have teeth and even faster after they are missing. Other people lose teeth without losing bone. It's very individual.

We don't make the permanent bridge until approximately 4 - 6 months after the initial surgery. 98% of the shrinkage has occurred by then. We use more implants than most offices and this helps stimulate the bone and gums not to shrink.

The pink part is the biggest difference. Implant bridges don’t have all the pink tissue as part of the prosthesis.

@@cheferthetanker8038 Then it's possible that you are actually getting implant bridges, and your Dr. is calling it "3- on - 6". Some Drs. use different terms for marketing purposes. The 3-on-6 is very similar to our technique but it uses cement for retention instead of screw retention. We've known for at least 10 years that the cement used in Implant dentistry is toxic to the Bone and soft tissues. Screw retention is superior! (This is the main reason that 3 on 6 is less expensive.) I hope this helps.

@@shashisingh3934 this depends on several factors: how many natural teeth you have remaining, their location and amount and quality of bone surrounding them, whether or not you will need bone grafting… and how many implant placement surgeries your treatment will involve - to keep you in fixed temporary bridges throughout the healing periods. If you’d like an estimate from me, feel free to contact my office and arrange to send a current panoramic and/or CT scan of your mouth. Ray, it's a technique developed about 20 years ago that results in a "denture" placed on four implants. It has been made famous by Clear Choice whose commercials you see on TV.

Well that was a nice explanation. However with all you people you never quote the price of the work done. I know everyone is different. But it would be nice to know how much the patients were charged. Maybe even a ballpark figure will do. So that a proper determination can be made theoretically for each individual that is watching .

You didn’t really explain the differences other than saying lip retracted on all on 4 looks like poop, but you didn’t explain at all how the bridge is placed in. Quite deceiving

No deception, at all. You might want to rewatch this video or view some of our others. Implant bridges are screw retained bridges that do not require bone reduction. The tissues are maintained/ shaped by the temporaries and supported by the final bridges. Happy to answer more questions after you've done some more digging.

You could call it "beauty and the beast" in comparison. The All on 4 look like overpriced, morphed dentures but with lots of metal showing. A monstrous outcome. I'd be mortified to end up going into this treatment plan , only to have it look like this. When I think about the whole point is to return the mouth to a healthy, and aesthetically pleasing state... this would not be it. Isn't the entire goal to save bone and tissue as well as prevent further loss? When I first saw a photo of the bone that's removed, it sickened me. I'm desperately clinging on to what little I have left, fighting a losing battle and health suffering as a result. So many of my dental issues have been caused by poor dentistry and since watching the first videos from this clinic I have had a tooth literally drop out of my mouth. It was a crown placed on a root canal that had been repaired a couple years ago. Inwas aware of the decay but what I found was much more. The entire center was filled with cotton gauze, so only a thin shell was bonded to the root canal. With ongoing irreparable decay, I surely didn't need to see what was involved in this $1500 repair. May not sound like a lot of money but for someone whose disabled, it's money I could ill afford to waste. Why dental implant bridges aren't the standard in tooth replacement is beyond me

@@davidmcfadden878 I have indeed. I was convinced from your first, longer video. Being ultra sensitive to any excessive bacteria due to an extreme case of oral Lichen Planus, as soon as I saw the flawed concave design of All on 4, I saw what a disaster waiting to happen it would be based on 1 single flaw. Then the angled abutments. All this awkward, hard to clean space these dentures on stilts, why? Just aesthetically, smile a little too wide exposes a Frankenstein mess. I had not realized a patient could very well end up having to pop them out and use store bought products to temporarily cement them and hope for the best!!!! Dentures would be more preferable for the cost and results I won't ever be able to afford, what clearly are the only safe and healthy back teeth restoration. Once they're pulled, I'm forced to live without any due to this cursed lichen planus and the restrictions of a disability income. Yet I will continue to promote the only same dental restoration on the market today. I'm sure as time goes by, little tweeks will only improve implant bridges. I wish dentists would stop promoting permanent bridges that destroy healthy teeth and are doomed to failure as cement loosens and those natural teeth break down. Mine suggested it as a more affordable option but alarm bells went off with just some basic research. Hard to treat Lichen Planus impacts the health as it limits your diet. As a very young looking 57 year old woman, as I lose first one molar, then another....to see the cheeks start to hollow out robs you of your confidence. I've already retreated somewhat. I see what I'll look like with the rest pulled and despite decay.....I've delayed extractions. Both for vanity and to enjoy eating solid food while I still can. Keep promoting this healthy approach to dental restoration. Your videos are well presented and relatively easy for people to understand. An informed patient is best able to advocate for their health and realize that over the long-run, costs will be less
