The metropolol one makes me laugh because my primary care doctor (now more than) once gave me a two hour lecture on blood pressure medication options before making a selection. There was even a chalk talk. I am the patient. I do not care.

Actually, from my perspective (endocrinology), I must respectfully disagree. My favorite patients have always been those who wanted as complete an explanation as possible to back up any medical recommendations. My least favorite patients patients (exaggeration: "You want to remove my liver - anything you say doctor") were those who just wanted to follow directions on "faith." I've always found it a pleasurable challenge to adapt my explanations to fit the patient's level of expertise. My absolute best "fun" practice experiences were when I could care for an academically-trained endocrinologist. Hardest part was tearing myself away to do other things.

This is so true! I am nurse and have experience from internal medicine. The rounds usually took forever just because two doctors argued like this about the patients. Sometimes I just grabbed my cup of coffee and told the other nurses "hey, this is taking forever, can you look after my patients while I sit here while we are doing rounds?"

I've been watching quite a bit of Dr. Glaucomflecken since I just found him, and I conclude that most likely his videos are so not ordinarily received due to how not unlike reality they are, and not only for the medical people who know the tropes, but for everyone because he sees so sharply what's so comedic about the stereotypical traits and his acting of personal characteristics is so particularly skillful. Like if he would do one on my job or hobby, I would probably laugh at how many little funny details I haven't realised and that get repeated almost daily. Such great observational skills. Thanks for so many helpful explanations on what's going on in hospital life, behind the curtains and daily job. KZfaq algorithm is also funny, I didn't even know that the medical youtube existed, but after some glaucomflecken I'm seeing every single medical practicioner reacting to his content and I learn that there are so many doctors doing youtube. Which is kinda nice, because I know so many professionals in my fields of interest or hobbies do it too. It's like having open university AND more practical schools available at the same time. And apparently so many professionals are also having fun doing youtube outside their job and educating people in what they maybe love or at least are great at. In fact youtube is possibly the first place I search information on topics that aren't my forte, and at times when the topic is my forte, I might just know where in youtube to look for... And afterwards I can have the ego of a professional without knowing more than the surface and advice people on things I have no idea about in reality.

Definitely would like to see you reacting to more of the specialties to help understand some of the jokes that are over the heads of people who haven't worked in a hospital. I love his videos on rural medicine because I completely understand coming from a town whose 'hospital' was a ten bed nursing station. Once they built the new hospital/long term care in the next town over it was converted to a vet clinic.

This is one of the best "sociologic" summaries of medicine ever! Everything from the endless, nonstop physiology reviews on rounds, to dreaded "curbside consults" and more! So amazing that in the 50+ years since I was in training, the "grammar" hasn't changed a bit. Oh, and as an endocrinologist, I also carried a tuning fork (for diabetic neuropathy) until I found that a smartphone could be programmed to vibrate at near 128Hz. And yes, medicine changes you, in some ways permanently. I'll bet 50 years from now, hearing a recording of a beeper beeping will raise the heart and respiratory rates.

This gave me a great 😃 today! I hope that if you use the term "nursing informatics" you've looked into what this certification/degree program means. You must go to school to become a registered nurse, go through nursing clinicals, (graduate) and take nursing state boards. Most nurses work in their field for a few years before going back to school for their nursing informatic certification or their Masters degree in Nurse Informatics. Best wishes if you truly want to move forward with this intense program.

This made me laugh out loud. I have an amazing and *very* thorough internal med doc and I feel like she has done most of these things with me hahaha. Definitely not complaining! I am super grateful to have her, her consult notes are usually always at least 2-3 pages long. I recently had an MRI to try to confirm a myositis diagnosis and I noticed on the report that it was also ?cysticercosis. I’m in Canada & haven’t travelled in at least 5 years lol. Edit: Oh god especially the "goes to therapy" sketch 😂 I literally insisted my psychiatrist watch that one so he could see how ludicrously it matches me (& then I apologized to him about how often I've effectively curbsided him with my quick questions about peers or public health populations I'm fascinated with lmaooo)

Talking about being specific with your words reminds me of grad school for curriculum and instruction for math and science. I had a professor who stressed knowing exactly what you're saying. For instance, when multiplying 5 x 10, you add a zero, not thinking that what you're actually saying is 5+10. I used to do it to my students when dealing with decimals 🤣

Thanks for sharing your thoughts and insight into what it takes to be an Internal Medicine! 😊❤️ Since Dr. G. gave the example of a patient with heart failure, would it be a good idea (Not Unreasonable idea 😉) to first ask what has already been considered by the knowledgeable internal medicine doctor? Or perhaps the patient's history thus far? This way, the resident med student doesn't feel like they're stumbling around in the dark, and they have an opportunity to learn about the patient's history before jumping to conclusions too quickly. This might just satisfy both parties. Thoughts? 🤔 😊

So I was a resident many years ago. Imagine this, we had a pulmonologist who normally did ICU rounds who got assigned rounds on the floor, but he was used to ICU rounds. Teaching rounds started at 8 am, at 12 pm we were half done but didactics started at noon. He dismissed us but told us we’d reconvene after to finish rounds. This lasted until 4 pm! A great deal of the time was spent deciding to involve this consultant and that consultant but by that time they had all gone home. The next day he would get angry because we had not gotten anything done!

as a student, my longest ICU round I can't even remember how long. we started at 9am, was able to go get my computer, had a code went to that, came back and they moved to one patient so far. stayed, got hungry, was told I could go to lunch and come back after. I think we were 3 patients in. did our treatment rounds, and check vents (I'm going for respiratory) and by the time we were done, I had one more round before being done with clinical. Most of it was talking about history of what and why as well as insurance so even though it was long, I got to see a different aspect of the medical field.
