I had my diabetes under control, the I started taking prednisone. No one warned me… going to go get some Chromium Picolinate. See if i can bring the blood glucose down to my normal range pre-prednisone

Dr. Megan....I can't thank you enough for putting this information out there. Zero doctors or ENTs have ever mentioned any of this to me and I've been stuck in these 10 day bursts of prednisone for well over 2 years. I hope my GP will at least write enough refills to allow me to taper off myself. It would be helpful to hear about what language to use with doctors who are particularly hard-headed and holding onto old, incorrect or simply missing information that affects their patients lives.

I started Prednisone about a month and a half ago. My last lab showed high glucose. I'm interested in the Chromium Picolinate aspect. Googled it a bit, and why does everything say you can't take Chromium while on Prednisone? I searched further, but nothing anywhere says why. Just curious what the reasoning would be.

When I was taking 60mg or prednisone in the hopsital, the nurses measured my blood sugar 4 times a day -- before each meal -- and if it was over 200, I would get an insulin shot before the meal. No insulin shot before bed even if blood sugar was over 200, since it could cause hypoglycemia at night. Prednisone definitely raises blood sugar. My HbA1c also rose from 5.0 to 5.7 (and probably higher now) over the months that I was on that high dose. Hopefully everything will come down as my prednisone dose comes down. I tried exercise (walking), avoiding carbs, getting more fiber and of course no snacking or junk food, but the effect of the prednisone was greater than all of those. I also tried taking chormium picolinate after getting released from the hospital, but I don't have a blood glucose meter to check to see what's going on with my blood glucose now, so I don't know if it is having any effect. I will say that I used to feel a crash after meals and I feel that less so now. I think that is a common phenomenon with diabetics, so maybe that is an indication of a spike in blood sugar after meals. Since I feel it less now, maybe that means my blood sugar is better controlled now, but I have no measurements to confirm that.

Hi - I'm in much the same position. Two things have made a positive difference; I've shifted the time I take Prednisolone to 4am and then I continue to fast through to 4pm; effectively having a very short eating window from 4pm to 6pm. The idea is not to eat during the 12hr period while the Prednisolone is active in the body. The other is Berberine which has seems to be working well to minimise hunger. End result is I have stopped gaining weight and my energy levels have improved dramatically.

@@philipwilkie3239 That's very interesting. Are you able to monitor your glucose levels throughout the day, either with a CGM or by pricking your finger and using test strips? Dr. Megan does say, I think, in one of her videos (maybe this one) that the "ideal" time to take prednisone is "shortly before you wake up" because that would mimic the body's dawn effect in which hormones are released that cause blood sugar to rise to give the person energy to get ready for the day. When do you get up? Do you go back to bed after you take your prednisone? As for me, in the 4 months after I wrote that comment, I was able to go from 60mg to 30mg and eventually taper off the prednisone completely. Now I'm taking other meds only. My HbA1c has gone back down to 5.0, and my insulin resistance also went back to "excellent." I'm not taking any supplements or medications specifically to deal with insulin resistance or high blood sugar, just exercising when I can and trying to eat healthy, although I'm not super strict about it. I hope your regimen continues to work for you!

@@karlint39That's excellent news for you - the ideal is to get off the Prednisolone altogether because it messes with so many systems not just glucose. I'm dealing with myasthenia gravis which means I have to be very careful with any tapering - 5mg per month is considered a relatively fast taper for MG and once you get down below 25mg it's even slower. And then I'll likely remain on something like 5 or 10 mg indefinitely - so I have to deal with this long term. I've always been an early riser so setting the alarm for 4am to take the Pred is not a problem, some days I'll doze back off for a few more hours, other days it's fine to get up and enjoy the early dawn. It takes about 3hrs for the Pred to start acting so it's not really kicking in until 7am. Yes I am tracking blood glucose with a finger prick meter and can confirm that it's usually around 5.5 mmol/L at 4am then without eating anything from about 7 am it will steadily rise to a peak of about 6.8 - 7.0 by midday - then drops quite rapidly to about 4.8 - 5.0 by 4pm. Urine test strips show no excess glucose at all, and slight ketosis at 4pm after 22 hrs fasting. So all of this data is consistent.

@@karlint39I find waking at 4am is OK - either I doze off afterward or I get up and enjoy the dawn. I am doing finger pricks and what I see is glucose rising at 7am from around 5.8 mmol/L to a peak of about 6.5 - 7.0 at midday, which then drops quite quickly to the low 5's by 4pm. Then I have dinner at 4 - 5pm, retest at 7pm in the mid 6's. Urine test strips show little to no excess glucose and mild ketosis at 4pm after 12 hrs fasting.

How long does it take for glucose levels to return to normal after tapering the prednisone? I was shocked to see my glucose level shoot up during lab work today. Thank you for this informative clip.

When I was in the hopsital on 60mg of presnisone, my blood glucose would be super high throughout the day, but relatively low in the morning before eating breakfast. This won't help with predicting when it will go down, but your HbA1c will tell you if it *has* been lower in the last 1-3 months. Years ago, I was on 5mg of prednisone for many years, and my blood glucose wasn't especially high. So I think the effect is definitely dose-dependent. I imagine the lower you get the prednisone, the less of an impact it will have on your blood glucose.

@@PrednisonePharmacist yes I have no adrenals. So probably the prednisone is using the pancreas as a carrier to spread doses throughout the body.

unfortunately my son had very high glucose when taking both this drug and methotrexate. He had to stop.

I have not heard that methotrexate causes high blood sugar. Did he stop the methotrexate as well, or just the prednisone?

@@karlint39 He was only on Methotrexate. No prednisone. I don't understand why the drug raised his glucose. He also gained a horrible amount of weight. Stopped the drug, glucose came down and lost the weight. Now he is searching for a medication that works as well. Very difficult.
