His knife broke. It was all he had left from his brother who died. So he got a panic/anxiety attack due to it Actor portrayed it well too, those attacks are hell on earth to experience My mother had em real bad in the past, stopped breathing once due to it. Had to save her life. Not a great experience.

He does have autism, he is a surgeon. The pocket knife was all he had left of his brother I believe. The knife is broken. I think his brother is the one who believed in him and encouraged him to become a doctor despite his struggles. The show is called The Good Doctor.
