The common reasons you can’t get rid of acne
1: not taking a well-rounded approach to how you actually treat acne
- Ingredients that decrease free radical damage and oxidative stress that damage the pores and lead to inflammation (antioxidants)
- Ingredients to decrease C. Acnes bacteria in the pores (salicylic acid, benzoyl peroxide, sulfur, succinic acid, topical antibiotics) that lead to inflammation
- Ingredients to help remove the dead skin cell buildup/ clogged pores (retinoids, salicylic acid, succinic acid, AHA’s)
- Ingredients to decrease the production of oil (Niacinamide, succinic acid, retinoids)
- Lifestyle factors to help decreases stress, regulate hormones, improve hygiene)
- Antiinflammatory ingredients to put out the c.acne fire
#2: Relying on your cleanser to treat your acne:
Your cleanser is to prep and prepares your skin to better accept the active ingredients mentioned above. Gentle yet effective is the way to go.
- In a recent research study double cleansing was linked to worsening acne
- Overaggressive cleansing can damage the skin barrier which will impair progress
- Aggressive exfoliation can irritate sebaceous oil glands causing them to pump out even more oil
#3: Using too many actives at one time :
Instead, focus on using a simple AM and PM routine that doesn’t pair the same active ingredients on top of each other that can lead to irritation.
#4: You may not have acne:
- there are many acne mimickers like rosacea, folliculitis, milia, etc. seeing your board certified dermatologist for an accurate diagnosis can save you lots of time and money
- Your acne may be linked to other undiagnosed medical causes like PCOS or IBS or medications. Again your derm can help.
#5: You are not being consistent enough. Clearing acne takes time! How much time can range. Please see the chart above and please don’t discontinue your medication once your acne clears
#6 Your diet sucks: Processed foods, foods with a high Glycemic Index( sugar content) and Dairy can worsen acne. You’ll want to rethink your diet if you eating poorly and you are serious about clearing your acne.
#7 Your stress level is out of control. Find something to help decrease your stress levels. Elevated cortisol can wreak havoc On the skin and worsen acne.
#8 You are not giving your skin what it actually needs. Don’t get in the rut of always using the same cleansers and moisturizer because it’s what’s on the counter. Listen to your skin's needs. Just because your skin was oily last month doesn’t mean it’s still only today.
#9 Check in with your hygiene practices:
- Stop touching your face all day and please not picking use a pimple patch instead!
- When’s the last time you washed your pillowcase? Your face mask?Your glasses? Your headphones? Your face towel?
- How long does it take you to wash your face post-workout?
#10 Don’t chase acne! Focus on preventing acne instead of chasing it. When your skin clears up continue using good hygiene practices and a good skin routine and do not rely on spot treatments. Get in front of that spot so it never shows.
*No skipping sunscreen! Sunscreen can help prevent scarring and hyperpigmentation. I prefer a mineral-based (zinc oxide) for acne patients. Those with acne have been linked to zinc deficiency. In addition, zinc has anti-inflammatory and antibacterial properties.

Really needed this. I never had acne in my teens. At the most I'd get one bump the week before my period that would heal and go away quickly and not leave any long lasting PIH. Now at 27 the struggle is real.

Same! I never had acne in my teens and only occasional flare-ups as an adult...until perimenopause. Then my face became worthy of a dermatology textbook. Menopause--the gift that keeps on giving!

I had acne from ages 11-23 with varying levels of severity. The only thing that finally got rid of my acne reliably was double cleansing. I didn’t know about actives or chemical exfoliates at the time. This was around 2012. I oil cleansed with a mix of grapeseed and castor oils and followed with a gentle daily cleanser and daily moisturizer by CeraVe. I would splash my skin with lukewarm water in the morning and just used a thinner layer of moisturizer. I used a different wash rag every night. It took several weeks of doing this to see results but totally changed the outcome of my skin as well as my sister who had the same issue. The thing I didn’t do was wear sunscreen which I regret. Please wear sun protection! Now that my acne is under control I only double cleanse as above when I wear makeup and I am using more actives. She hit the nail on the head with the picking. Do NOT pick the acne!!!!! Do NOT use the baking soda, lemon juice,scrubs etc.

I have tried every product on a walmart or Walgreens shelf and now i decided to try changing my diet and that actually worked. I have acne on my face, back and chest. I gave up gluten, dairy, processed sugar and junk food. My skin as been clear for a week now which is rare. Try it for your self guys

@@dominikastrauss5208 just won't eat bread or anything else with gluten. I also had to google what might have gluten besides bread. It helped not only my acne but my digestion as well

@@QuirkyBoy I'm from the UK- So I signed up to a company called 'Skin+ Me' , it's similar to Curology or Dermatica. My prescription is 0.021% tretinoin, 1 % clindamycin and 4% azelaic acid. I have a basic routine of using Cerave hydrating cleanser, use cerave moisturiser and use spf 50 during the day.

Your videos have helped my skin so much! I am normally oily, so I was scrubbing the heck out of my face like 2 sometimes 3 times a day but my skin kept getting oilier and oilier and went from bad to worse! I could not for the life of me understand what I was doing wrong but I found your winter skincare video and I made some of your recommended changes including NOT washing as much, using the Tula milk cleanser and adding in niacinamide as well as cutting out so many actives at once and it completely turned my skin around! I also have started wearing sunscreen when I have never ever worn sunscreen. I now feel like a complete idiot because I feel like I should’ve known better, but I just assumed that my skin getting oily meant I wasn’t cleaning it well enough and that was so so wrong! Thanks for sharing your knowledge with us for free :-)

I love your videos so much! Honestly ever since I started back into college on the science/medical side I have totally changed my opinion on aestheticians and KZfaq skin experts and focused my time and attention on medical doctors like yourself. There really is a science behind things that a lot of people don’t realize. I do believe that some aestheticians are very talented and knowledgeable but I don’t think after years of school they know more than a doctor. Currently I’m a respiratory therapist that’s taking classes towards physicians assistant. I’d like to stay in the respiratory field but Dermatology also excites me

Omg your skin dr is AMAZING 🤩. Also please could you recommend some good drugstore affordable moisturisers please. I have combination/oily/dry/acne prone skin with hyperpigmentation. My problem areas are mainly my chin and jawline areas. I have no idea what to use. The ones you have recommended in the previous video are hard to find in the uk and some are a little pricey or out of stock. Thanks Dr

I use to go to the dermatologist, but I am no longer able to. When I did, they had a screen saying that you can’t get acne from what you eat or dairy, but I have noticed that when I stopped drinking milk and don’t eat sugars, my skin was a bit easier to clear. I still have issues with the cystic pimples that hurt, but it’s gotten a bit better. When I sleep, I use T shirts on my pillows. It’s easier to change and I have noticed that my skin doesn’t break out like it use to. As a matter of fact it has gotten better. I use my sunscreen, and I’m starting to take zinc pills, and have gotten better with washing my face every day twice a day. I hate sweets, but when I have a taste for something sweet, I have to remind myself about my skin. I’m just worried that when I go back to college, I may get stress acne.

I have had IBS for around 10-12 years and acne for much longer. Never before have I heard about acne being linked to IBS (though it does make sense with the links to gut health and inflammation). When I think back, the time my skin was indeed the most clear was after I had treated my acne with prescriptions but before my IBS symptoms started. I simply figured that the acne treatments hadn't really worked and there was no reason to go back to the dermatologist. Since then I've tried all sorts of skincare as well as lasers, with no real improvement. This has given me renewed determination to treat my IBS properly, plus has made me very happy that I just made a phone appointment with my family doctor to get a referral to a dermatologist.

Great advice. I finally got meds and topical treatment from a derm. I am 37 and been struggling since my teen years. They are helping me address hormonal acne. Along with these, I am still doing all of the other things she mentioned in the video consistently. The biggest change in 2020 I learned is SUNSCREEN!!! I never used it before last year, I was so ignorant. Now I know!!!

What I found works for me is taking care of your body from the inside. I use to be the type that loves chocolate. I drink coffee probably twice in one week. As soon as I went cold turkey on the both, I noticed a drastic change on my skin. I don’t suffer from extreme acne. Only when I get my period. And I’m irregular so it can last more than a week. I break out around my cheeks and chin area. What I also learned is Tea, there are certain ones/flavors that helps with acne or the skin in general. Roobios, Hisbicus, Green Tea & Chinese Violet Leaf is good for the skin. I alternate drinking all of what I mentioned for 4 weeks straight and it did wonders to my skin. All decaffeinated by the way, just my preference. Water Water Water and lots of Water! I also take Pre-Probiotics (50 Billion). Food intake, I cut down on so much. Lots of fruits and vegetables. I pretty much went Pescatarian. I’m not big on red meat. Lots of smoothies and juicing. Thank you for sharing this video 💕

Foods really do make a difference. I noticed a huge change in my skin when I started intermittent fasting 4 years ago. Now the only thing I do is wash my face with a warm cloth and treat my skin at night with CeraVe resurfacing retinol serum. My skin looks better now than it has in years

Doc, this cheat sheet is great! It makes things simple. It would be wonderful if you would add ingredients that aren't only for acne for the next vids. Idk like a vid that shows the most common ingredients and what they do in a table form like this. Anyway I hope you have a great day!

I have to disagree about double cleansing. When I don't double cleanse, my face gets very congested. I can tell a difference in when I double cleanse and don't. I have pcos and extra oily skin so it is helpful for me.

Dr. first of all; you are beautiful! Everything that you say - I follow! I have had acne and hyperpigmentation/ Melasma all of my life! I recently decided to get in accutane, now that I’m 56 yrs old. But, before I get on it I wanted to try one more time to stop my outbreaks and “wallla” your information has been a bible for me (I’m spiritual)! My skin with cyspera (expensive) but worth it! I also have paired ordinary products and, Dr. it’s working ! I finally woke up this morning without a breakout - thank you ! My melasma and hyperpigmentation is a slow process - and that ok, because I’m smooth and hydrated - love you

I’d love to see more on the link between double cleansing and acne. The study at 3:42 shows a correlation between oil cleansing and acne, but doesn’t show a causal link. I know I tend to use more cosmetics when my acne is worse, which then requires more cleansing to remove. I wonder if relationship between oil cleansing and acne doesn’t go the other way.

Would love to hear more about this new information on double cleansing! What were they using to double cleanse (an oil cleanser and then a water? Or just their regular cleanser twice?) I had heard it was good to get off makeup and since leaving makeup on the skin overnight can cause acne I would think it’s good! Dr. Alexis mentions double cleansing in her video, "You Are Washing Your Face Wrong". Per her recommendations, one should only double cleanse if wearing heavy makeup, and use an oil based cleanser, followed by a water-based cleanser. It's about four and a half minutes in.

Great Video....What I learned is that my skin Loves Sulfur, BP, Salicylic acid, and I need exfoliation! I also like Alpha Arbutin by the ordinary and the Urban skin rx Super glow serum. Still looking for a good gentle exfoliation product!

This is the video what I need, blc inspite of doing and trying everything , I have on and off episodes ko acne breakouts and its vicious cycle I had acne it will heal n leave brown mark and then I will spend next 1 to 3 months clearing that mark up and then boom when mark is about to go a new pimple will appear and again I am on same cycle .

For what it's worth facial massage worsened my acne. The best thing I have done is keep my hair and hair products off of my skin and cleanse my skin with a foaming cleanser after washing my hair.

In 2020 i was breaking out with the WORST cystic acne on my cheeks for the first ever and I watched your past video on foods that cause acne and i found out whey protein was causing it. My skin is now acne free but the pigmentation is still, do you have any advice for red pigmentation (hyper-erythema)? Thanks Dr. Alexis!

I had a very similar situation. Multivitamins and Whey protein supplement sent my skin into a frenzy with acne. Once I stopped taking them, the constant cystic acne subsided. My dermatologist prescribed Retin A .05% to help with the hyperpigmentation.

Also for colored cosmetics does that include tinted sunscreens? I had learned that they help prevent hyperpigmentation from the visual light spectrum. But do they make acne worse because they are a colored cosmetic? Would love to hear more about that -thank you dr Alexis!
