Holy cow! Check out this guys pricing!!! I went to a first rate Newport Beach plastic surgeon for deep plane facelift, neck lift and upper bleph. Including surgery center with 1 overnight stay the total cost was $16k but this was 5 years ago. Dr Karem charges $140k for same thing. Inflation is bad but it’s not that bad. His pricing is just astronomically crazy. Who can afford this?

People think Hollywood, but his customers will be families that own businesses, maybe 10+ McDonald's or car dealerships. In 2021, annual wages for the top 1% reached $819,324. Income inequality means he'll never lack for paying customers.

As aestheticism, I have only had some Botox. I am now 75 , have used active ingredients you speak of as well as Red light treatments, vitamins. I am now 75 and people tell me they don't believe it. Also exercise is important

In a couple of months I’ll be 69. I am eerily disciplined when it comes to skincare. I’ve used prescription tretinoin every night from my forehead to the base of my chest, and from my fingertips to my elbows for many years. I’m very diligent in keeping my skin hydrated and moisturized. I never skip a morning or evening when it comes to my dedicated skincare regimen and I’m fiercely protective when it comes to sun exposure. I know topicals can only go so far, eventually I do think some form of surgery intervention will be required, but I am very happy with the look and condition of my skin. I don’t want to look younger, I simply want to be a woman on the threshold of 69 years old who looks well kept for her age. Let’s be realistic, younger to me would be 65! I’m not trying to turn back the clock, I’m just trying to look like I care.

Such an encouraging video 🥰 you’re so right that it’s about looking like we feel inside. It’s not vanity. I’ve worn sunscreen since the 90s :) I started religiously when I was 16. And at my sister’s second wedding the photographer thought I was the youngest in the age lineup. Back in the 90s it was recommended in the magazines to do it, so I did. I started with neutrogena chemical and just kept going from there. Worth every penny and every bit of time it took to blend it in lol from those super early days of cast. I use Omnilux almost every day now, and take collagen am and pm. Slowly working in tret once a week now in my mid 40s. And keeping my weight in check. Personally I think I still look 20s in the mirror. Might be delusional lol but it feels good to see it pay off.

The section on Understanding your Anatomy was truly eye-opening. It helped me understand that flawless perfection does not make a youthful face. I have seen many youthful and beautiful faces that have nasolabial folds and tear troughs. You have helped me become more forgiving of these so-called "flaws" in myself. BTW I absolutely love the Trifecta! What a difference it has made in my skin!

You are totally right Dr.Karim. I am 58 yrs old and for 3 yrs now I have been using sunscreen, good skin care as you mentioned and I have been also using the contour mask LED for a year now and my skin has changed for the better. My skin is even better than my 40s. I am no longer lying in the sun like I did in the past. I use a self tanner. I will also do my micro needling once a week at home. I have been consistent and it paid off. I will never stop 😊 Thank you for the great video. You are amazing!

You're the same age as my sister. She never uses sunscreen or good skincare and still sunbathes. She still looks good in spite of the above. I've been using good skincare and sunscreen for the last 15 years. My skin is saggy, age spots and wrinkles. My age is 65 and I look it 😢. My sister must had received the youthful gene. It must get down to genetics.

A beautiful smile and a sparkle in your eye also makes someone look younger. Nobody looks good with a scowl on their face and sometimes we don't realize our stress is showing. I've trained myself to have a pleasant look and a slightly upturned smile to combat my mouth which tends to curve downwards. It really makes a difference

I absolutely agree with these !!! I actually have referred many of my clients to you for consideration of surgery because you preach comprehensive approach to the face and aging! I am not a fan of any RF modalities and encourage skin Rejuvination in other ways and nothing compares to surgical solutions. ❤

I agree with you, Mary Vollmert. His last section, "Understanding Your Anatomy" should be a real "come to Jesus" meeting for some. As time (decades) go on, people forget what a real face at 20 - 30 is supposed to look like. This was an eye opener for me. I forgot.....tear troughs are normal. Nasolabial folds are part of our normal anatomy. It's crazy what becomes acceptable. Thank you Dr. Karam, for this most excellent video reminder.

Thank you Dr ; I just turned 40 I don’t look like my age. I started doing microneedling twice this year. I have noticed my skin looks much better now. I used sunscreen also using retinol at night . Thank you for the information it really helps

I love your videos - thank you so much for taking the time to sit and be real with us. I wish I’d been more aware 3 years ago when I made the (desperate and dumb) decision to do Ultherapy. I was 40, I wanted to address the beginning of what I saw as laxity in the neck and jaw and of course “baby jowels,” and my industry was doing a number on my self esteem. The medspa told me it was going to encourage collagen, and the effects would last years longer than any other non-surgical procedure. It was the most PAINFUL experience I’d ever had and the recovery took weeks, where my face was swollen, and my skin was red and textured, like I’d had an allergic reaction. Years later, I cannot say I see any positive results. It was such a waste of money and time, and I wished there was more information out there when I was researching this procedure! Now at least I know where to go when I need some honest information. Do you have recommendations on how to choose practitioners?

I just turned 34 on New Year’s Eve, and I cannot tell you how happy I am to hear Dr.Karam reassure some of the practices that I’ve been doing for close to 6 years now. My skin looks great thanks to actives and sun care! I was recommended to do the Morpheus 8 RF treatment for some acne scarring and I didn’t due to what Dr.Karam states regarding fat loss. I was dead set on getting it done before doing my research. I’m happy that we are moving towards a more natural looking aesthetic and preserving what we have. I will definitely gift myself the facelift procedure when the time comes. Hopefully it will be with you. Love your content!❤

Thank you for pointing out the upper bleph fat pads. I have genetically horrible under eye bags since I was a kid and I’ve always felt like I look tired and sick now that I’m in my early 20s I wanna get a lower bleph but was thinking of just doing both since it would be cheaper and I could avoid another surgery if I don’t like my upper lids in the future💀 that’s would’ve been a mistake bc they still look youthful and literally nothings wrong with them

Thanks for another great educational video! I heard recently on a podcast that your face ages without your body realising as it happens so slowly. I think this is why actives and microneedling work so well it alerts your body to repair.

⁠@@jodyd5786if you don’t overdo it, it won’t be poofy and overdone looking 😁🤷🏼‍♀️ I’ve also had my tear troughs filled and we did it very gradually - in 5 different appointments. It was extremely natural and she said her preference was to under fill vs. over fill. It was one of the two things on my entire body that has ever bothered me and I think it really helped my appearance and my confidence. I’d definitely do it again 🙂

I was going to do some Kybella for the double chin...but after I heard your point of view about not removing the fat between skin and muscle I decided to address my diet and start exercising...I eliminated sugar and double chin reduced, still a bit there but I'm terrified of getting sagging skin if I do kybella...I rather continue improving my diet, sleep and exercise, it's the tough route

Great information!! So helpful! There are so many factors to consider when getting plastic surgery. Then once one decides, there's sometimes the fear of being under for 4-6 hours, the financial commitment for the procedure and the anxiety of how one will look after and how family and friends and acquaintances will view you. Sometimes that's the hardest part since others can be very judgmental and want to shame you or tell you to just except the aging process. I believe that if there are opportunities to live a better quality of life then more power to someone that wants to improve their appearance via cosmetic enhancement.

In my experience, the people who tell you to accept aging fall into one of three categories: younger women who think that they will accept aging (they have no clue), older women who either don't have money for a facelift or were never attractive even when young, and male relatives.

I'm hoping there a good number of plastic surgeons and aestheticians subscribed to your channel. Its so sad to see people ruin their faces by making poor choices, even informed by well meaning clinicians who just haven't really understood the consequences of their actions. As someone with a facial difference all I want is to look natural. Although I have had some surgery I am fairly risk adverse as I definitely don't want my face to change for the worse! I would love to hear from anyone who has any experience in treatment for facial differences x

This was my favorite video from all the videos you’ve done. Why? Because at 49 everything you said hit home for me. Thank you for being so frank and honest about how our skin ages, the anatomy of our faces and to be realistic about our expectations with surgery. Luckily I’m blessed with great skin and I have been using actives for years and years and I understand that only surgery can fix laxity and to fully rejuvenate the face. I will save my money and not buy into the fads of those machines ( Morpheus etc) like you, I feel like I’m 30 so aging is hard to accept. When the time comes and I’m ready I’ll definitely get surgery so the way I feel on the inside will match the outside 😊🙏🏽

This is really informative. I’m very lucky I went to a plastic surgeon for Rhinoplasty that explained comprehensive surgery (not in those terms, but essentially same thought process). The result was a surgery that has aged well in +12 years

I got upper blephroplasty last year. Heavy eyelids have been inherited from my mother’s side of the family. I lost a significant amount of weight which only made it worse. Because of the weight loss the sagging forehead exacerbated the problem. So I got a forehead lift at the same time. I regained peripheral vision and look younger. I love the results. People don’t know what’s different just that I look better.

I’m currently saving up money to remove my old fillers I had done regularly starting from when I was 17. I’m sick of the look and people’s comments. I haven’t filled them since 2020 and they still look pretty much the same. Just goes to show how they don’t really ever go away…

🔮 😂laughing out loud picturing you as a fortune teller! My favorite trends you mention are a “comprehensive approach” & “less stigma”. I’ve been questioned for having “so many in-office procedures”. I try to explain my 7 year rejuvenation plan was created by a dermatologist, who believes in treating the face neck and decollate as a whole … making tiny subtle changes that enhance, not transform. 😊

What do you think of bio- stimulators like Prophilo. It's meant to hydrate and stimulate collagen but doesn't stay in your face like actual filler. Thoughts?

The problem is, and I think most of your listeners will agree, is that 80% of us want the surgery but can’t afford it. I can’t even afford the machines. I’ve gotten surgery at teaching hospitals but I’m sure the results don’t come close to people going to surgeons like you. I would love to do what you recommend.

I'm 63 and have been considering facial rejuvenation surgery for a few years now. I have to get past what I know will be a lot of judgement from friends and family. I've avoided all cosmetic treatments and procedures so far including botox and fillers. Thank you for clarifying the importance of actives and collagen stimulation. Do you believe ingesting collagen as a supplement has any benefit for the skin? Everything I've read so far seems to indicate that it's a waste of time and money.

I love almost all of your advice and theories with the exception of doing away with upper bleth’s, since some people are saggers, instead of sinkers. I myself, am a sinker. At 26, my eyes looked extremely hollow. Now I always had large eyes and larger lid show, but as time went on, fat must’ve decreased in that area because I was looking extremely tired. I had a permanent tired eye look. Even when I did my makeup to fill up the space some, I still had that tired looking appearance, (think of Cher’s eyes). So I actually had filler placed in that region to make the eye look more sculpted and youthful looking. I don’t regret doing it now that it’s calmed down quite a bit but at first I will admit that it looked too swollen and puffy. But anyway, for saggers, like my mom, she would really benefit from a blethro. She has way too much skin overhang above her eye. Would you agree that it’s a procedure that’s only beneficial to saggers in that case? If not, what else would you suggest?

​@@dramirkaram so why don’t you like blepharoplasties? I know that you mentioned it can make you look older, but sagging above the eye can age you too. Also what would the point of getting a blepharo done if you’re a sinker? I thought the whole point of one was removing excess skin and fat from the eyelids. If you’re a sinker, like myself, the upper eyelid area would just be hollow, so what would you be removing ?

@@tranquilgray6244yeah, I would be curious too to know what would be suggested for “sinkers” since it’s not a case of excess fat or excess skin necessarily . Although I wonder if a just a tiny and I mean very slight removal of skin in the upper eyelid would mean less skin to “sink”. (Being extremely cautious to only remove a tiny bit so that there would be no trouble closing the eyes completely . I think the anatomy of the eyeball protrusion and the point of where it starts to fold inward when opening eyes would determine whether a safe but aesthetically satisfying result could be achieved by the slightest removal of eyelid skin.

@@SleeplessinOC yes! Exactly! Me too! See, I don’t get how removing any bit of skin for a sinker would be beneficial because a sinkers problem is fat loss in that area! So like for me, in order to fix my hollow eyes at 26 I had filler placed in that area so my eyes would appear more youthful and almond shaped.

I have always been very outgoing and I find myself getting more introverted and go out of my way to avoid bringing attention to myself. Great informative video. Stopped using fillers years ago and concentrated more on my skin. I’m 57 but do look younger. Dream is to have facelift some day but until then I’ll age as gracefully as possible and use Botox and occasional Sculptra in temple hollows. Gotta fight the fight! Love your videos- keep them coming

ALways love your videos. I would add, eating clean and exercise for the health of our skin. What we consume has such a great impact the health of our skin. Also Muscle is the largest anti-aging organ we possess and drastically deteriorates as we age. Keep not only your face youthful, but your body as well. Hope to see you when I turn 60 for a vertical lift

I am just loving ALL of your vertical lift (or whatever you call it, lol), and skin care videos!!! I am 52 and have hated my neck for about 10 years. I always said I wanted to get it done. And it was for me, about me. It wasn't about anyone else, or my husband. I was really scared of being judged, so I hardly told anyone before I got it done. BTW, I just had my deep plane face and neck lift done on May 20, 2024. I think I am postop day 12 today. I purposely didn't watch anything on KZfaq before I got it done. Now, I know that was a mistake. I wish now that I had seen your practice, and values. I am postop day 12, and still really nervous about my outcome, based on some of the things you mentioned. I am still pretty swollen and bruised, and just got my staples out yesterday, so I am really trying to stay calm and give my body time to heal. However, I really just wanted to come on here and tell you how grateful I am that you are making these videos for us. Now I know exactly what ingrediencies to look for in my skin care routine. I am going to check your line out. I am hoping you have a website, with maybe an online store. But, I also now know that other rejuvenation services would be right or wrong for me. Thank you again!

I hope everyone understands that you get what you pay for. If the price is low then you probably won't be happy with what you get. I find myself looking at people too and I can spot things that I've learned about watching Dr. Karam's videos. When watching TV with my husband I will point something out to him that I have learned from these videos. This is a great video Dr. Karam and I thank you for making it and educating us on what is good and not good. At least now we can make an informed decision when we want to do something. All the bad trends in cosmetic surgery and unrealistic outcomes. That youthful faces have lines and how you publicly study faces. I'm an artist and thats what artists do when training is going out in the general public and closely study facial anatomy of all different ages and ethnicities. Faces have always been my favorite thing to sketch. You sir have put artistry with anatomy into your practice to get the best natural outcome and it's refreshing! Btw i did my facial removal at the age of 40, I think that’s the reason why it worked perfectly for me, I look much younger , believe me. But for a younger person, I don’t think is the right procedure to choice!

I love that the trends you “predict” are what you hope would start happening around aesthetic industry rather than naming “trendy” procedures 😂 No I genuinely love that because most people don’t understand the outcomes of over-doing. Seeing a niche procedure on TT and immediately running to your nearest clinic should be soooo out in 2024…it’s giving obsession, body dysmorphia and self-destruction. I can’t imagine how much of a pet-peeve is that for you as a professional.

I'm wondering if Bio identical Hrt might start becoming the new norm, like 5-10 years from now, like once people turn 35 if it'll just be standard for every person to start bioidentical hrt (starting at a lower dose of course at 35, and dose changing to meet individual needs as they age), everyone would be living longer and looking better for longer

I have been on HRT since I was 58. Not only has it been a game changer in my mental health but knowing how it is helping my bones, brain, heart, and other organs in the body, I am sure it has been beneficial for my skin. I am 67 and I wish I would have gotten on it sooner. Because of the lack of knowledge I did not , and therefore suffered several years until I found a doctor who believed in it.

What I still can’t get is why comprehensive approach can’t be done step by step. Upper face and upper eyes first, then the mid section with lower eyes in a half year, lower face and neck in a year. I think it would be interesting to know more about that, including fear of 6+ hours narcosis and comprehensive complications etc
