Asthma Review - Annual Review of Asthma Control with RCP three questions - Dr Gill
Poorly managed asthma is a regrettable cause of death. Something that should easily be preventable with regular, annual asthma review, checking a patients inhaler technique, their peak expiratory flow rate, and the effect of asthma on their day to day activities using the RCP Three Questions
Ensuring Adequate asthma control is hugely important, not merely for mortality, but also to improve a patients quality of life. If poor asthma control is affecting sleep, exercise, and causing a strain to the patient, there are things that should be addressed in the annual review.
Similarly, a personal asthma management plan should be agreed upon, allowing the patient to be able to adjust their care and improve their home management

Dr Gill and Abbie, this was a great video. If Abbie is acting she's doing very well, if not this is a great insight into a "real" appointment where the patient is answering honestly (stumbling a bit, being a bit vague sometimes but with a genuine problem she needs to communicate) which is kind of different to previous examination type videos where the patient always seems OK.

I’ve had asthma since I was 3, currently 24. It has gotten much better as I’ve gotten older but still comes back under certain circumstances

Same here! It was really bad when I was a kid but now I'm 23 and it only flares up when I have allergic reactions to dust, car hair and pollen. Really grateful that I don't have asthma attacks in the middle of the night anymore!

Honestly no. We confirm things like drugs, but other than that, just run with it - it’s a well learnt skill at medical school, bring able to be a patient, as it helps you learn the conditions

I hope this is understood since English is not my mother tongue, I have been using an inhaler called Butocort HFA since I was 11 years old because I suffer from bronchospasm, I never asked my doctor since he was a pediatrician and I haven't seen him in years but I would like to know if it can be completely cured since today I am 29 years old, the internet is a confusing place and since I am a big fan of yours I ask you this question, much love from Argentina!

I was diagnosed with chronic asthma in 6th grade, I would think the reason why the doses for asthma medication is so low is because the nature of asthma attacks on a general basis is that they come and go so frequently and the reasons why they happen are often so small in frequency so they wouldn’t do high doses for something that is so fleeting so often.

The doses of asthma medication are because of that action of the drug on the local surface of the tissue Oral medications have higher doses, as they have to be absorbed in the stomach and moved around the body By targeting a tissue directly you can use a smaller dose of a drug

Night time can be a bitch. It can also be a bit psychological too I find. Especially if you are low on Ventolin. I have also been on becotide 250 for a good few years now and I find it was a game changer for me. Seems to have really boosted my immune system. Made me feel bulletproof for a long time. Exercise does help. Weights and walking.

I'm curious why medicine was the only treatment plan discussed, especially with such severe symptoms. Are there no options for breathing exercises to improve lung capacity and function? Should an allergist be consulted to see if there are unknown airborne triggers? Etc.

I was not diagnosed as asthmatic until I was 22 and was trying to join the RAF. It ruled me out of serving. I didn't start showing symptoms until I was around 13. Please get tested. Uncontrolled asthma is a killer.

Been prescribed and taking blue inhalers for asthma, triggered by certain triggers, since childhood for over 20 years. Tried to order a simple repeat prescription for expired inhalers last week. Instead of 2 inhalers was given 1 and then when I asked GP why, they said its because I don't have asthma...? I mean tbh thats good news... But ive had to take my inhaler several nights in a row last month due to having a bad cold and the breathlessness and tight chest meant i could not sleep until i took it. It was definitely needed. So what is the solution here? I mean you go in to the GP when your feeling well... They do a peak flow test or something and they decide your breathing is ok so you're not asthmatic and don't need inhaler. Then what happens two weeks down the line when you catch a flu and it triggers your asthma? Would they take the responsibility for their negligence?

I have been using ProAir for years and the hand tremors never stopped for me. I'm not a doctor, so I can't give you a definitive professional answer. This is just my own experience.

Whilst this IS a role play, I’ve been seeing patients face to face throughout the pandemic, and continue to now There are MANY things that can be safely treated on the phone, and there we do so, but there are also MANY things that for safety need to be seen face to face

In the scenario she is already on a steroid inhaler, and the short acting beta blocker, so better to try and move to a MART therapy if the patient is able to follow that protocol

If you are concerned about your breathing, I’d strongly suggest talking to you doctor, and if it is new, getting a PCR test too

Yeah I was wondering that too because it seems very realistic and natural. But looks like she is a medic and even has her own channel and Dr. James said in another comment that they don't rehearse it so I imagine she comes up with the answers on the fly based on her knowledge. Very realistic though it had me convinced she had all these ailments.

As one of the medical students, she’s well versed in “being a patient” and being able to quickly craft a history for practise. It’s a good skill, as you need to KNOW the conditions to role play the patient
