Bipolar disorder is a disorder where you have reoccurring episodes of depression or mania. Because these mood states are opposite in nature and say they have opposite polarity. It’s like a magnetic pole that has opposite ends of attraction. At one pole of the mood disorder spectrum, you have depression which is a low mood state and low energy state and at the opposite pole is mania where you have a high mood and high energy state. In this video I discuss how unipolar mania behaves differently from bipolar mania and how the treatment may be different.

My mania has always been used as a tool to get shit done. I managed to start a successful small business the ground up with mania just because I had a manic moment where I thought hey I can do this. Manically worked on it for 3 months crashed at the end. Still worth it.

@@addysaw very true! I mentioned hypomania because the original commenter described channelling "mania." It didn't occur to me that someone would describe their happiness and relief after a depressive episode as "mania", but I guess it's possible!

Wow. I love how you explain everything in layman’s terms. Bipolar disorder is definitely the toughest condition to treat. As someone who has fought depression and anxiety most of my life, I know how terrible depression can be to get out of. I live with someone who is quite anxious and I have to get away from her often because it triggers my own fears. Thank you 🙏🏻

I don't think mental illnesses should be ranked by which is worst to treat. One may have any mental illness that never responds to treatment.

I’ve read that people with bipolar disorder may not want to get medicated because the manic state is so pleasurable and almost addictive. I wonder if there are a lot of unipolar manic people out there who feel even more strongly about their condition. Thank you for your very clear explanation of this!!

If only mania could stay pleasurable. Some people may start out with grandiose, happy mania but as they get older it eventually turns into more irritability. Also as DMG said, even grandiose mania usually produces some level of destruction.

@@dmgsoultogetherness6667 You would have lots of energy and it sounds pretty manageable. Without the crash of depression, it could be nice so long as you don't get too crazy. Although, it might make you really annoying. We have a couple of bipolar in our family and the mania is usually worse than the depression for the people around them. For the person with the disorder, they like mania, at least until it lands them in an institution.

Hyperthymic. My roommate was like this. He'd get 4-5 hours of sleep, was constantly bouncing around, working out, energetic, super sexual constantly, and sometimes got a little agitated at people. But man, he was a machine. I guess as with many things in psychiatry, it comes down to how much it affects people. ADHD and anxiety can definitely go hand in hand with these symptoms.

Thanks for making this video, Tracey. I've been diagnosed with bipolar type 1, but I present with unipolar mania as you've described. I've been hospitalised on three separate occasions for very serious mania with psychosis present. I've never had a major depressive episode (lasting 2 weeks). It's closing in on 4 years since my last relapse and I've stopped taking medication over 2 months ago (I was on a very low dose of clopixol depot). I am indeed a highly optimistic and friendly person, but not so much high energy imo. Also, I don't think anybody would be able to tell I have this condition without me being manic, I'd have to tell them.

Oh wow - this is so helpful. I have a close friend who had their first manic episode with psychosis earlier this year and was diagnosed with bipolar but the diagnosis didn't make sense to me because this person has never suffered from depression. They also have high energy, talkative and genuinely is the most optimistic/positive person I have ever met so the bipolar diagnosis felt weird. I've questioned the psychiatrist on this but it feels like there's not much info on just mania so thank you, thank you, thank you for making these videos Dr Marks. Your teaching and impact is deeply appreciated!

Unipolar hypomania/mania is traditionally diagnosed as bipolar disorder according to the DSM-V

Another great video! Thank you! I know someone who we suspect has unipolar mania. It was always confusing what her diagnosis was since she only appeared to get manic and not have depressive episodes. Unipolar mania would explain it. The episodes are getting worse and worse over time. I hope the person can eventually get diagnosed and get proper treatment before it is too late.

I’ll explain more. He’s constantly go go go, never relaxes. Workaholic. But obsesses over projects and hobbies, spends thousands of dollars on a whim. VERY quick to anger. And believes in a lot of CRAZY conspiracies and hides guns and swords in the walls of his house.

@@Imtrying_girl I don't know it father, I'm just here to provide information for comparing/contrasting symptoms 💙 Several of the symptoms you've stated can also be ADHD symptoms if it feels like it's never ending up-beat, but he still sleeps. The lacks of needs for sleep is a sure mania symptom. Being naive and believing conspiracy theories because he wants to feel special is also an ADHD (among others) symptom as well. However, if your father is delusional, and makes up his own conspiracy due to self referencing thoughts, that's a manic symptom that doesn't overlap w/ADHD.

Thanks so much for explaining the hyperthymic temperament! I always thought I might be bipolar coz of that. But when I watch your other videos explaining bipolar disorder in detail, I don't relate. New doctors always suspected it at first but then discarded the hypothesis afterwards.

There are a lot of folks who may experience no depression symptoms, including myself most all of my life,☺but fall into the trap of self diagnosing "mania" or bipolar disorder. Or even ADHD... and just have a hyperthymic temperament! Thank you for the great info, Dr! I did not know how to conceptualize the "manic" symptoms being described that did not meet criteria for mania or hypomania and are life long with no clinical depressive Sx.

@@DrTraceyMarks Hello Dr. Marks, I have watched your video about dysthymia and all videos about depression but I still can't seem to find mine.I had my first depression 4 years ago and since then I kept having those episodes regularly. I take antidepressants for my OCD that's rooting from my childhood. Most of my obsessions are gone thanks to CBT and antidepressants but It doesn't help with my constant depressive state. Like my episodes are just its own thing. Not coming out of anything. I don't know how to open up about it to my doctor and therapist.I self harm a lot and have all the symptoms to depression or related illnesses. I have so much hate towards myself that I can not get over. Why do I have so many depressive episodes but slightly less severe ones when most people have just a few major episodes? I'd be really greatful if you help me.

This sounds souch like me! Early onset BP1, and probably 2 or 3 extended depressive episodes in 44 years, otherwise hyperthymic, and hypo/manic. The risk taking gets dialled right up though and ive been in quite a few serious accidents that nearly cost me my life when i was manic. Stabilising on Lithium now, but needing a decent dose due to quick metabolism. Thanks for sharing the video... Helps me feel im not alone.

I was literally JUST pondering this. My manic friend seems to be having a MUCH easier time with HIS disorder, plus the doctors do not make him jump through hoops for medicinal treatment [he can get away without seeing a therapist - I can NOT]. I struggle with chronic chemical depression [sometimes diagnosed as major], anxiety and ADHD. I'm envious of the fact that he isn't treated like a guinea pig for modern medicine. He seems to be doing just great chemically, and I'd finally come to the conclusion that his illness must be easier to treat - though I automatically thought that it would be the other way around.

My husband had unipolar mania. He was originally diagnosed as bipolar and took lamictal religiously for years. I always thought it was strange bc he was literally NEVER DEPRESSED. He was the happiest, funniest, most positive person ive ever known. He was so damn energetic ALL the time and he never got much sleep. Hed start pacing before 7 am every day. Eventually his doctor told him he thought there was something else going on and that he should probably be rediagnosed as manic (except there isnt an official diagnosis obviously). Regardless, he was a wonderful person...i miss him so much. I lost him 6 years ago and theres not a day that goes by i dont miss how much hed annoy me sometimes

@@senseofmindshow I hate it. Being hypo manic is quite pleasant, if anything bad happens you just briefly feel down and then the world is bright and wonderful again. Pure mania however is just scary. One of my coping mechanisms is exercising, but when I’m manic I take that to extremes, I go on walks that lasts for hours and keep walking even when my feet starts bleeding. One time in summer I was swimming in a lake and I didn’t stop until my mother came and got me, I had been down there for hours and when I came up I was shaking so badly I could barely walk, I would like to say I would have stopped before I drowned but I’m not sure. Three days in to a mania my body hurts so much that every step I take feels like my last. My mind is exhausted but the thoughts keep going no matter what I do. It feels good in a way, but the complete lack of control over yourself is just terrifying. It’s hard to explain to someone who’s never felt it.

I was never diagnosed with bipolar I or II , I just know that my depressive episodes are long and terrible . My manic episodes short . Thank god Im being “normal” for the past 3 years , taking 4 medications…. But I know depression , anxiety or manic episodes are there they will show up , eventually. My life have change so much trying to keep my balance , I don’t drink , smoke , don’t go out , I’ve separated myself from situations where I can go back to negative thinking, rumination of ideas , conversations , separating myself from toxic people. It’s difficult to keep yourself sane but is better than being miserable all the time .

I think my brother has manias. He’s always been so careless about money (which he doesn’t have), about drinking, you get the picture, but has never been depressed as far as I know. Only now that he lost everything and is sick and needs surgery, he’s depressed but who wouldn’t ? Whereas me who’s diagnosed bipolar 2 have been depressed most of my life. Once he asked me “what’s depression?” And he laughed. Of course he’s never been to a psychologist because there’s nothing wrong with him (that’s what he thinks) he’s a lost cause

Those designations apply to someone who has dissociative symptoms but does not have enough symptoms to qualify for the other dissociative disorders like DID, depersonalization/derealization, etc.

You never let us down each week :) Would you be able to do a similar style post on BPD/EUPD and what some people call "QUIET borderline", I know it isn't an official term, but over over years and trying to understand my EUPD more, I discovered that I tend to fit into a quiet borderline type as I do not (or vary rarely) have outbursts or arguments in public, or become angry, I tend to hold it all in until I'm alone where I self harm or have an angry moment and punch a wall. In the moment thats making me uncomfortable or triggering me, I find that I tend to dissociate when pushed to the limit. I find that BPD has a stereotype that I still fit but just in such a different way to what the public recognises. There's not much content out there about quiet bpd and I'd love to have something I can share with my family and friends to help them understand me a big better. Your videos are so helpful. With your help I decided to go to a physiatrist and was diagnosed as BP 1. I have stopped medication due to sexual dysfunction which I previously never had. its been about 4 months since I quit and still have problems, so I'm curious if you have any suggestions other than just sildenafil. Its become a big problem and up until my medication, not one a remotely dealt with before. Anyways that you for all your informative videos. Your a beautiful person and honestly should have a side gig as a fashion model. Just a thought. ☺

This is exactly what I've had for years and years, yet I've never heard of Unipolar disorder. Most medications just make me feel like a zombie and I can't focus. This is 100% describing my family. I wish more doctors would know how to manage this condition.

I was diagnosed with bipolar type 1 a few years ago, after many different episodes of mania. I told my most recent psychiatrist that I am hardly ever depressed. I never really experienced the depression side of bipolar disorder. However, along side when I have these episodes I also experience what some would say would schizophrenia. I don’t know if it’s intertwined, but I noticed that I have symptoms when I start an episode: less sleep, overthinking thoughts, voices, (sometimes I even talk gibberish-like another language) and then I think it goes into psychosis State where I feel like I’m living in a different reality, and I mix up where I am currently at. Sometimes the mania makes me think that I am bigger than I am, such as FBI, CIA etc. There has been episodes that I can recall where I become destructive, there have been episodes where I want to save the world. There have been a few episodes where I get lost,. I’ve had episodes where I start one thing and never finish it, and I usually get the cops called on me, and end up hospitalized. I’m currently on lithium carbonate schizoaffective disorder, and I haven’t had an episode since may of 2021. Stress can be a big trigger for me, but more often than not I think it occurs when I don’t take my meds. There have been times though however, when I feel like I am more than just one person living in my body. Sometimes I feel like I am a boy, and a girl. I don’t know if that correlates with my disorder, Anyway I’m rambling however I really like this question and in kind of help me think like maybe I’m not just bipolar and I think help clarify

Thank you so much for this insight! I'm not diagnosed with bipolar, but I could relate to a portion of your experiences. I've never been hospitalized, but sometimes I'm convinced I'm better than I actually am. I'm better than everyone. I also get paranoid a lot and I can relate to not feeling real and not feeling like a human. There are times (more frequently now) where I would burst into energy and be very talkative. Stay up late and make plans for my future, as well as starting doing things that I've eventually had given up on. Also seeing ghosts or spirits or thinking someone is going to kill me in my sleep. In the end, it comes crashing down and I'm lying in bed crying or have self harming thoughts. I'm sorry for writing so much. I hope you're alright now. Thank you again for sharing your thoughts. This makes it easier to understand what you go through and I understand your pain

Hi Dr Tracey I have a question: are there any differences between unipolar depression and the depression in bipolar disorder, for example is it possible to spot someone with depression who is potentially bipolar before they've even had a manic or hypomanic episode? I have a tangentially related question which I’ve asked on an older video but wanted to reiterate here to see if maybe you’re more likely to respond: I’ve suffered from full major depressive episodes for several years at this point, and though they have some defining points of recurrence (e.g. one extra-long episode per summer), I also go through cycles of major depression AND periods of several hypomanic symptoms; perhaps even enough symptoms to qualify for full hypomania, but never significantly enough for it to raise concerns with my counselor. When I found out about cyclothymia, I resonated with the diagnosis on several levels, and wondered if it could describe me. However, upon watching one of your videos on it, I found that because I had experienced full major depression before, that I may not qualify for such a diagnosis. So, here’s my questions: If you’ve experienced full major depression, is a cyclothymic diagnosis out of the picture? Should I potentially look into bipolar 2 diagnosis if the answer is yes to the former? Is it possible to have very cyclic depression without a bipolar or cyclothymic disorder/temperament? If you read this and respond, thank you so much. Happy new years!

When I was 25 I was put on an antidepressant called Nardil an MAO inhibited for a severe depression I was experiencing for the previous 2 years. Within 4 days I had a drastic change and I felt better than I ever have. I was on it until I was 40 years old. It definitely put me in a state of Hypomania for all of those years and looking back I I did exhibit a considerable amount of risky Behavior but not to the point of destructive Mania. Never took a nap in 15 years and would only need 5 hours asleep each night. Amazing feeling but not knowing i was walking a tightrope. She was very correct an antidepressant can put you in a State of not Mania but Hypomania

I have had anger issues, meaning I can fly into a rage really quickly, my whole adult like. I hated that I did that but couldn't stop. I went thru one very serious depression with suicidal ideation. That's the only serious depression I've ever had. My doctor said I was bipolar. After some discussion we were able to figure out that when I flew into rages that was mania and it's still bipolar without the depression. I would never have thought that rages could be considered mania but it all make sense to me now. I would be interested to know if other people deal with this. With medication I am very even and don't get angry.

For me anger comes out when something is not fair , when someone comes after me for no reason . I get angry , I throw a tantrum , I have to leave the situation immediately, breathe , go for a walk o cry . If not becomes a real meltdown.

I had heard there were several such as Quiet BPD, and High functioning BPD, and It gets confusing from there, so I was hoping you would explain them or at least the first two.

Thank you for the definition of "mania mission" and the focus on sleep aside from sedative talk. I think I've read about bipolar disorder but still need to review the difference between hypomania and mixed states other than hearing "mixed states are when you have destructive negative moods AND the energy to carry out a self destructive agenda."

I really dont think it is the description seems to fit a friend of mine. She always had the most hyper energy would talk to anyone all the time, it just led to her getting far too heavily involved in drugs and alcohol, taking on more projects than someone could really handle, she rarely ate, she struggled to wind down without weed, in the end she burnt out and seemed to have a breakdown. I dont see her much any more but the last time i did she was stick thin and talking nonsense. You cant talk any sense into her either.

That may fall under the group of "resistent" where even after having tried most of the antidepressants and mood stabilisers,your condition lingers most of the time..

Thanks for the question. The antidepressants are only about 60-70% effective in most cases. So not everyone will respond to them even if they try more than one.

I know this has been answered, but I'd like to add that sometimes, depression or presentation that looks like depression can be caused by trauma or other environmental factors like chronic stress. If the environment does not change, treatments like medicine and therapy will only go so far. Therapy will usually try to help an individual cope with or change their environment, but this is not always completely doable. Therapy and medication can also help with trauma and chronic stress, but in some situations the person may not feel able to talk about these factors for some time, or may not realize they have even experienced them and so not know to address them with therapy. Wrt treatment-resistant depression, there are other treatments like electroconvulsive therapy, clinically administered ketamine, and more, but these are not always available or accessible (e.g. prohibitive expense, no clinics offering these in the area, etc.)

I wanted to know what advice you might give to a person who is struggling to take care of themselves because of mental health stigma. I personally find it hard to stick to a strict routine because the people around me in my life don't have to. What would you suggest? I have never had depression (I’m 55) but my doctors absolutely refuse to believe me. I have had lots of hypomania and an episode of mania but they say I am lying about the lack of depression. It has completely destroyed my relationship with the health service. I want help with reducing my meds safely but they just say I am lying. I am not in denial, I am a professional who has never had time off from work and have been stable for 5 years. I am left with no option but to do it on my own. I feel like if I want help then I have to pretend I have symptoms that I don’t have. I am so frustrated (but not depressed)😂

Thanks for sharing your knowledge. I’m not a Dr but I enjoy reading about pychiatric nosology (maybe that’s in itself a new nosological category!). Anyway, it looks confusing to me why bipolar disorder includes 1) classic mood psychoses but also 2) cyclic psychoses without affective content. Example of 1) is someone who is depressed and eventually develops mood of grandiosity such as being Jesus, or irritability, who can evolve to psychosis. Example of 2) a woman I know who has profound religious beliefs. She can be perfectly fine for months but from one day to another she starts to hear voices calling her ve nasty insults (like the word that starts with bit and ends with ch (in case youtube bans my comment)) after which she is admitted to hospital and treated with Haldol. She doesn’t really have depressive episods and her delusions are not about grandiosity, nor shows signs of pressure speech, high energy, etc. She is diagnosed with bipolar disorder but her family has cases of both bipolarity and schizophrenia... Another example: a man who suddenly starts experiencing prosecution delusions and kills himself, also diagnosed with bipolar. It seems odd to me that Kraepelin and followers decided to get rid of cyclic/dual psychoses (without affective patterns) and put them all together with truly bipolar (case 1)) and on top of that, add recurrent depression to the same entity “Manic-Depression”. Why is the case that case 1) and case 2) above are both considered bipolar I ?

Hi Dr.! Thank you for this video. I couldn't help but wonder about the inverse of this, unipolar depression. I've fought with depression for decades, and not once have I had any experience with mania that wasn't drug (caffeine) induced. I've also tried antidepressants for the depression but they only made things worse. I have read xomments about the productiveness of mania... I wish I knew what the productivity was like.

Dr. Marks, would you be willing to share insight on a couple of questions? 1) seeing improvement with a low dose of a medication, but worsening/returning to baseline when the dosage is increased 2) why ADHD stimulants sometimes put people to sleep 3) falling asleep as an anxiety/fear/stress response i.e. beginning to have a panic attack or a flashback and falling asleep before it peaks These have been my experiences, but I've heard that 1 and 2 happen to other people, too. No one else seems to do #3 :p I did ask my psychiatrist; he said re: #1 that I'm sensitive to medication (I agree), re: #2 that it's normal but we don't really know why it happens and re: #3 that he'd never heard of that before Thanks! (Not asking for medical advice - just find these things weird and am curious)

You're welcome Daphne. I've only seen a few of their videos. I think their content is similar to what you would get on a blog where the articles are written by a health writer who does research, but is not clinically trained. They do great storytelling, but I don't know if their intent is to deliver clinically accurate information vs keep you interested in psychological topics.

I would love to hear more about the benefits of receiving an injection of antipsychotics vs taking it in pill form. I’m on Invega in pill form and I think it’s working some, but not completely. I believe this is due to having gastric sleeve surgery and Invega being an XR drug. I would also like to know of any other benefits you might know about. Thanks!

I have noticed that my bipolar started as Bipolar 1 then it became ultra rapid cycling and now I feel ony unipolar hypomania. I am in a very good chapter in my life with barely any stress and it feels like because there is no depression or anxiety I get stimulated by anything during the day. Does this mean I need to change medication for unipolar mania?

I had a quick question for you. I am a retired physician my spouse male is definitely bipolar one but primary mania. He is being treated with geodone 20 mg. q hs It seems when he has been off of the me for a wk he responds better when he has a manic episode. Is it true you need to stop the medicine for awhile to get better results during a manic episode?

What's your opinion on Endoxifen as a medicine for treatment of manic episode. In india, it comes under the name Zonalta, and doctors have started prescribing it for manic patients, even though the medicine has not completed it's phase 3 trials. Would love to know your opinion on it. Or anybody else having any experience regarding this, do share!

Can you have a mix of unipolar, mpd, and/or bipolar ?? With someone who suffers from fibromyalgia/cps could that be a possibility along a with diagnosis of ptsd, adhd, autism, depression and bipolar. With multiple manic episodes that differ on pain levels?

So I'm not sure about the did (mpd) diagnosis, but if you're bipolar, you are also depressed. So if you're thinking "could I have bipolar and unipolar disorder?" Then that would contradicting itself, because the unipolar part, as in major depression, is included in bipolar disorder. You're manic depressed
