Hi Jay. I'm a nurse and planning to take OET this year. I have seen some sample case notes where past medical history shows a list of medications for past medical problems related to the present medical problem. Is it important to list all those medications (there are a total of 5 meds)?

Jay ,how do we address an occupational therapist requesting for an assessment of the patient'ts workplace? Usually we write, I am writing to refer in other cases. But ,I don't know how to write to an occupational therapist?
